Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.
Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
Certified Immunizer or willing to become an immunizer within 90 days of hire.
Preferred Qualifications

Assists the pharmacist by receiving new written prescriptions or refill requests. Following the completion of proper training, receives new prescriptions over the phone from doctors' offices. Gathers the following information: patient name and DOB, medication name, strength, directions, refills, prescriber's information, etc. Refers all doctor's medical questions to the pharmacist.
Accesses, inputs, and retrieves patient and prescription information into and from the computer and successfully, efficiently, and accurately bills insurance and generates a label for the medications.
Mixes pharmaceutical preparations, fills vials, bubble packs, and/or dads with the correct and right amounts of tablets and capsules, and generates labels for bottles. Alerts the pharmacist if a CII prescription needs to be dispensed as they fill all CII orders. Utilizes Kirby and AccuMed when available for increased accuracy.
Prepare all compounds to fulfill waiting orders; this includes but is not limited to weighing/measuring chemical, preparing equipment/supplies for chemical use; completing the compounding process for available chemicals and hormone supplies.
Some chemicals are hazardous and require protective wear, which will be supplied to every individual. Each individual is responsible for preparing themselves for the job.
Retrieves and files pharmacy prescriptions in the appropriate files.
Assists patients and customers by greeting customers, offering assistance, running cash register for over-the-counter and prescription sales, receiving payments, and providing other services requested by customers. Refers the patient to the pharmacist for any medical questions. Assists patients via drive up window to make service more convenient for the customer.
Bags and hangs prescriptions after the pharmacist has checked them. Prepares prescriptions for deliveries and mail outs as necessary.
Answers the telephone within two rings and handles requests within their job description including refill requests, new prescriptions, and general questions. Directs prescriber inquiries, prescription transfer requests, and patients' medical questions, etc., to the pharmacist.
Maintains store stock of medications by reviewing shipments for completeness, putting price tags on the products and placing the items in their appropriate shelf locations, excluding CII medications.
Order, label, and count stock of medications, chemicals, and supplies, and enter inventory data into computer.
Maintains inventory levels by following defined ordering practices for warehouse and approved outside vendors.
Maintains the pharmacy including performing the following duties: clean pumps within 24 hours of return, fill pharmacy supplies, vacuum, dust/face, wipe countertops, clean sink and settee area, straighten counseling area, empty trash and shred bins, clean bathroom, clean hood and anteroom per requirements, etc.
Keeps product shelves and counters neat and clean and “faces” shelves by moving items to the front and arranging and stacking them. Checks items' expiration dates and removes expired and short-dated products monthly.
Maintains regular and punctual attendance. Develops courteous and professional relationships with patients, customers, and co-workers.
Works with pharmacist to ensure proper Medicare DMEPOS paperwork is complete, provide appropriate product to the customer, and maintain accurate billing records.

EDUCATION and/or EXPERIENCE

I. Pharmacist-in Charge
· Ensures the legal operation of the pharmacy, including meeting inspection and other requirements of state and federal laws, rules, and regulations governing the practice of pharmacy.
· Maintains adherence to all departmental policies and procedures.
· Ensures all documentation is complete and maintained for the required amount of time.
· Oversees daily operations to ensure adherence to policies, procedures, and billing practices.
· Provides daily support to the River Crossing Pharmacy staff.
· Review errors and provides staff coaching consistent with a Just Culture environment.

II. Controlled Substance Management
· Maintains accountability for controlled substances in accordance with federal and state laws, Iowa Board of Pharmacy regulations, and departmental policies.
· Ensures accurate, timely, and complete documentation of all controlled-substance activities, including receipt, storage, dispensing, returns, waste, and inventory adjustments.
· Oversees the perpetual inventory system and conducts required reconciliations; promptly investigates and resolves discrepancies, documenting findings and corrective actions.
· Ensures secure storage, restricted access, and proper chain-of-custody workflows for all controlled substances within the pharmacy.
· Serves as the designated DEA Registrant responsibilities for the site, including management of CSOS certificates, power-of-attorney assignments, and record retention requirements.
· Provides oversight and coaching to staff to ensure compliant controlled-substance handling and facilitates training for technicians, pharmacists, and new employees.
· Ensures readiness for regulatory inspections by maintaining audit-ready controlled-substance documentation and responding to inspection findings with timely corrective action.
III. Therapeutic Drug Monitoring (TDM)
· Completes a comprehensive patient assessment which includes evaluation of the patient's medications, and an evaluation of the patient's medical history.
· Evaluates all prescription orders to ensure the appropriateness of the following:
o Indication for use
o Dose
o Route of administration
o Drug interactions
o Dose adjustments for organ dysfunction
o Physical and therapeutic incompatibilities
o Drug-induced side effects and adverse reactions
o Potential drug misuse or abuse
o Use of the most cost-effective therapy
o Duplicate therapy
· Ensures pharmacokinetic assessment of drug dosing on targeted patients through:
o Clinical assessment of patients and desired therapeutic endpoints
o Preparation of formal dosing recommendations
· Communicates with prescribers about patient medication needs.
· Conducts review of patient profiles and updates as needed based upon findings at time of patient visit.
· Provides appropriate drug information to other healthcare professionals.
· Confirms/clarifies (assesses) all patient drug allergies noted in the medical record.
· Participates in collecting and documenting medication histories on patients.
· Evaluates patient compliance with prescribed medication therapy and identifies problems that may lead to partial or noncompliance.
· Utilizes appropriate resources and coordinates the resolution of financial concerns/burdens with department financial counselors.
· Participates in the facilitation of continuity of care for patients when transitioning from one setting to another.
IV. Documentation of Patient Care Activities Performed
The pharmacist provides pharmacy-related services through sharing of information with other members of the healthcare team via documentation within the patient medical record and other on-line reporting systems. Examples of documentation may include:
· Patient drug allergies and patient-specific height and weight information.
· Medication histories.
· Medication counseling to patients.
· Interventions with prescribers and other healthcare providers.
· Patient interactions - those that occur in the clinic and when the patient is located elsewhere (e.g., at home, as an inpatient, in the Ambulatory Care Pharmacy).
· Drug dosing recommendations made to prescribers.
· Drug Use Evaluation Program activities.
· Steps taken to identify, evaluate, or avoid adverse drug reactions.
· Prescription billing information, including prior authorization status.
V. Patient Communication and Interactions
The pharmacist must provide a high level of customer service and educate patients on the pharmaceutical care services offered including the following:
· Medication history.
· Patient's drug allergy history.
· Patient teaching classes (where appropriate).
· Medication counseling, including in-person, by phone, and at the drive-up window.
· Medication synchronization and adherence packaging.
· Communicating patient information to local healthcare providers involved with the patient's care to ensure appropriate continuity of care.
· Providing support for prescription billing (e.g., benefits investigation, prior authorization, claim processing, etc.).
VI. Enhanced Patient Care Services
The pharmacist assumes the role of patient advocate for all concerns associated with drug therapy. The pharmacist shall position themselves to work to positively influence therapeutic decisions which will affect patients and their care. Participation in hospital-wide programs intended to improve care (e.g., the Drug Use Evaluation Program and Adverse Drug Reaction Reporting Program) serves as a valuable quality-of-service surveillance tool and promotes healthcare provider education.
· Follows-up on any medication system problems, such as:
o Unclear or inappropriate prescription orders
o Cost effective drug therapy
o Prescription for non-formulary products
· Carries out, as appropriate, Medication Use Evaluation Program initiatives through active prescriber intervention and education.
· Serves on intra- and interdepartmental committees, task forces, and teams as required.
VII. Educational Services
University of Iowa Health Care is a teaching institution; therefore, the pharmacist is responsible for the education of others about appropriate medication use and billing practices and should serve as a role model. The pharmacist will participate in education services, which may include:
· Serving as preceptor for pharmacy students and pharmacy residents.
· Presenting in-service educational programs to staff nurses, physicians, and other colleagues on the healthcare team.
· Presenting at and/or participating in other educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UIHC or UI departments, and state and national organizations.
· Conducting educational classes for pharmacy students and pharmacy residents.
· Conducting patient education classes about the proper use of medications.
· Serving as mentor and role model for other pharmacist practitioners within the department, particularly those who provide ambulatory care services.
· Assists in the training of new staff members.
VIII. Other
· Completes and maintains compliance with all competencies and educational requirements.
· Assists with the evaluation and implementation of new products/technology, policies, and procedures.
· Performs other duties as assigned.
Education Requirements:
Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy.
Current licensure by the Iowa Board of Pharmacy Examiners is required. Includes those eligible for obtaining this licensure.

Job Requirements include but are not limited to,
Bachelor of Science degree in Pharmacy or PharmD Degree from an accredited school of pharmacy.
Current registration with the Iowa Board of Pharmacy as a Licensed Pharmacist.
Maintain Iowa Pharmacy Medication Therapy Management (MTM) certification.
Minimum of 2-3 years of experience in oncology pharmacy practice.
In-depth knowledge of cancer pharmacotherapy, chemotherapy protocols, and drug interactions.
Completion of a PGY-1 Pharmacy Residency with Oncology experience (preferred).
Completion of a PGY-2 Oncology Pharmacy Residency (preferred).
Board Certification in Oncology Pharmacy (BCOP) (preferred).
Familiarity with electronic medical records (EMR), computerized physician order entry (CPOE), and oncology-specific systems (e.g., Beacon, Epic).
Must possess excellent interpersonal and communication skills.
Ability to work well with a team and interact with patients, physicians, and other team members.
Required to be current in BLS (Basic Life Support) by the standards of the American Heart Association.
Will be required to show proof of having completed Mandatory Reporter course at the time of hire or attend within 6 months of the start date.
Must adhere to all training requirements as mandated by Federal, State, and Professional licensure and regulatory standards.

As a Pharmacist you will:
Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
