Position Summary:
The Formulary Operations Pharmacist - Medicare is responsible for providing operational support for the evaluation, implementation, and maintenance of Medicare Part D formularies and utilization management. This individual supports creation and maintenance of formulary and utilization management lists, preparation of CMS formulary files and member formulary drug lists, preparation and review of updates for implementation, and other formulary and utilization management related tasks and processes. The role requires strong cross functional collaboration and communication skills, and an understanding of Medicare Part D formulary management processes and related CMS rules and regulations.
Position Responsibilities:
Maintain standard and custom formularies for the Medicare line of business in accordance with CMS Medicare guidelines
Support development, implementation, maintenance, and quality control of Medicare Part D formularies, utilization management, and clinical adjudication drug lists
Lead and support preparation of monthly and annual files for CMS formulary submissions, member formulary drug lists and look up tools, and utilization management criteria
Evaluate drugs/drug classes, make formulary positioning and utilization management recommendations, and update formulary management strategies and associated adjudication requirements to operationalize
Evaluate appropriateness of and operationalize custom client formulary and benefit change requests within the formulary and adjudication platform
Support comprehensive testing of client formulary and benefit elections
Provide cross functional support for claim troubleshooting within the adjudication platform
Monitor and analyze regulatory and legislative requirements for Medicare
Support development of project plans to meet new CMS requirements, and support cross functional teams with implementation of programs to meet requirements
Lead formulary change processes within URAC/NCQA/CMS guidelines
Pharmacy and Therapeutics (P&T) Committee participation as an ad hoc member
Conduct formulary reviews and develop drug monographs based on a rotation of clinical pharmacists
Maintain and update the Capital Rx pharmaceutical pipeline, as needed
Develop clinical criteria for review as needed
Support CMS formulary administration audits
Support quality improvement projects as needed
Support Request for Information (RFI) and Request for Proposal (RFP) submissions, as needed
Support general business needs and operations, as needed
Responsible for adherence to the Capital Rx Code of Conduct including reporting of noncompliance.
Minimum Qualifications:
Doctor of Pharmacy (PharmD) Degree from an accredited institution, with record of strong academic performance
Current, unrestricted registered pharmacist license(s)
Residency/fellowship preferred
2-3 years of formulary experience working for a health plan or PBM preferred
2-3 years of Medicare experience preferred
Knowledge of formulary development and maintenance processes
Knowledge of highly managed specialty medications/strategy

As a Pharmacist you will:
Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
Document all communications and clinical interventions in the patient's medical record, including interactions with families and caregivers.
Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
Educate and support facility/agency staff on safe and effective medication handling procedures.
Perform other pharmacy-related tasks and responsibilities as assigned.

Bachelor's Degree in Pharmacy, Pharm D preferred
One year experience in long term care
License/Certification(s): current and valid pharmacist license in the state of practice
Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
Proficiency in electronic medical records and pharmacy information systems
Strong clinical judgement, and effective written and verbal communication

Pharmacist:
The Pharmacist serves patients by dispensing prescribed medications, drugs, and other pharmaceuticals for patient care. This position requires traveling between Sunrise's Loveland and Evans, COPharmacies. No Weekends or Nights.
Position Summary:
With a Quality, Customer First, and Compassionate approach, The Pharmacist will:
* Prepares medications by reviewing and interpreting physician orders, detecting therapeutic incompatibilities.
* Dispenses medications by packaging, labeling pharmaceuticals and verifying NDC's.
* Controls medications by monitoring drug therapies, advising interventions.
* Completes pharmacy operational requirements by organizing and directing technician's workflow, verifying their preparation, and labeling of pharmaceuticals, verifying order entries, charges and inspections.
* Provides pharmaceutical information by answering questions and requests of health care professionals, counseling patients on drug therapies.
* Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
* Enters data, such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge system, and inventory.
* Reports prescription errors immediately and adheres to company policy regarding medication incidents.
* Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
* Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by maintaining records of controlled substances, removing outdated and damaged drugs from the pharmacy inventory, supervising the work results of support personnel, maintaining current registration, studying existing and new legislation, anticipating legislation, and advising management on needed actions.
* Protects patients and technicians by adhering to infection control protocols.
* Maintains safe and clean work environment by complying with procedures, rules, and regulations.
* Ensures billing for in house plans and runs monthly reports.
* Completes other pharmacy reporting as requested.
* Reviews and submits outstanding Medicaid claims.
* Oversees inventory process.
* Resolves operational issues.
* Manages oversight and compliance of Patient Assistance programs.
* Assists in development of Policy & Procedure manual, PAP online application, coordination of medical screening, and pharmacy/EMR interface.
* Assists in interviewing and hiring support staff.
* Completes new hire on-the-job training.
* Schedules and supervises technicians to best utilize coverage. Supervises Patient Assistance Program Specialist team.
* Manages oversight of 340b policies and procedures
* Dispense and administer vaccines per protocol and CDC guidelines.
* Process provider refill request according refill protocol
* Completes 24 hours of continuing education either online or by attending workshops to maintain Coloradopharmacist license.
* Other duties as assigned.
Minimum Qualifications:
* Bachelor of Science (B.S.) Degree in Pharmacy or Pharm D.
* Current license to practice pharmacy in the State of Colorado.
* Minimum 5 years' experience, or equivalent combination of training, education, and experience.
* Bilingual English/Spanish preferred.
