Organizes and schedules a variety of major and/or complex computer system tasks, including programming, in a timely and proficient manner.

EDUCATION/EXPERIENCE
Bachelor's degree in Pharmacy, or Doctor of Pharmacy preferred.
Completion of an ASHP-accredited PGY2 Pharmacy Residency in Pharmacy Informatics preferred. Proficiency in Microsoft Office (Excel, Word, Outlook). Excellent verbal, written, and interpersonal communication skills. Experience as an Epic Willow Inpatient analyst preferred but not required. Epic experience in an inpatient healthcare setting is preferred but not required. Work experience may be substituted for formal education requirement. Strong analytical, technical, organizational, and problem-solving skills. Will be required to successfully pass the Epic pre-assessment test.

Performs outpatient distributive functions related to entering prescriptions orders, selecting the correct medications for new and refilled prescription orders, and coordinates delivery of medications for patient pick-up or delivery.
Clinical Duties and Responsibilities
Ensures safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols.
Reads, extracts and interprets information in patient medical records accurately.
Detects and reports suspected adverse drug reactions accurately and in a timely manner.
Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
Provides accurate, adequate and timely drug information to the hospital's pharmacy associates.
Coordinates pharmacist drug education to patients and their families per institutional protocol.
Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.
Conducts designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)
Baseline departmental standards for reporting interventions for clinical productivity as defined by departmental goals.

The Pharmacist adjusts and dispenses medication to patients and drives the compliant and efficient operations of the pharmacy. This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
Responsibilities
Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.
Other duties as assigned.
Qualifications
Bachelor's degree in Pharmacy or PharmD required.
Licensed, “good standing” pharmacist in the state of Texas required.
IV certification required.
In-patient/hospital pharmacy experience strongly preferred.
Flexibility to work various shifts and on-call as needed.
Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals.
Strong customer service skills.
Manual dexterity required for occasional reach, lifting and holding of small objects.

We are currently recruiting for a PRN Pharmacist to work as needed covering PTO/vacation/sick time and on-call after hours. The facility is open Monday through Friday 8:00AM to 4:30PM.
The Pharmacist adjusts and dispenses medication to patients and drives the compliant and efficient operations of the pharmacy. The Pharmacist verifies physician orders using the pharmacy information system, makes adjustments according to protocols for a given medication, and provides patient education. This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
Responsibilities
* Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
* Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
* Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
* Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
* Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
* Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
* Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.
* Other duties as assigned.
Qualifications
* Bachelor's degree in Pharmacy or PharmD required.
* Licensed, "good standing" pharmacist in the state of Texas required.
* IV certification required.
* In-patient/hospital pharmacy experience strongly preferred.
* Flexibility to work various shifts and on-call as needed.
* Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals.
* Strong customer service skills.
* Manual dexterity required for occasional reach, lifting and holding of small objects.
* Computer savvy to learn and perform successful operation of multiple pharmacy information systems.
* May require vendor credentialing.

Anticipated hourly range: $50.10 per hour - $71.70 per hour.

Responsibilities
Coordinates and schedules the preparation of sterile mixtures and home medical equipment orders with the assistance of the Pharmacist
Completes all calculations on product preparation as necessary
Compounds sterile mixtures and non-mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit
Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards
Documents all activities on appropriate logs
Participates in branch management, department and intra-departmental meetings and quality improvement activities
Adheres to all accreditation, OSHA, FDA, state, local and federal regulations, and USP 797 and USP 800 standards relevant to infusion pharmacy and home medical equipment
Documents all temperature logs, pressure and humidity monitors, records all sterility testing done in the clean room in compliance with company policies and procedures, and USP 797 and USP 800 standards
Documents all activities on the appropriate logs

Qualifications
Successful completion of an approved/accredited Pharmacy Technician training program; IV certification preferred
Registered Pharmacy Technician license in state of practice per state regulations
National Pharmacy Technician Certification as required by the state board of pharmacy in state of practice
1-3 years of related pharmacy experience in a home care setting, retail or pharmacy setting, preferred
At least 1 year of aseptic preparation of infusion-related pharmaceuticals in a hospital or home infusion setting preferred
General knowledge of drug terminology, pharmaceutical calculations, drug use and aseptic techniques
Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis
Effective mathematical and communication skills required
Basic pharmacy knowledge of reading and interpreting prescriptions
Ability to work independently, accurately and efficiently
Valid driver's license
Driving Position: Yes-Minimal driving for patient deliveries may be required
**To perform this role will require constant sitting and typing on a keyboard with fingers, and ocassionally standing and walking. The physical requirements will be the ability to push/pull and lift/carry 21-30 lbs**
