Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.
Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
Certified Immunizer or willing to become an immunizer within 90 days of hire.
Preferred Qualifications

Job Description
A Pharmacist's primary responsibility is to ensure patients receive safe and appropriate medication. Pharmacists fill prescriptions based on the information and dosage provided by the doctor. Pharmacists must ensure that the dosage is correct for the patient and their condition. SPECIFIC PHARMACIST DUTIES:
Expert use and safety of medications dispensed.
Work alongside physicians, nurses, and other healthcare professionals.
Supervise pharmacy technicians, interns, and ancillary help with filling, dispensing, labeling, and packaging of medication.
Perform the final check on all medications.
Manage inventory and profitability. (Invoicing purchasing, labor, etc.)
Protect health information (HIPPA).
Prevent fraud, waste, and abuse.
Prepare sterile solutions according to USP 797.
Prepare Enteral feeding and nutrition.
Prepare computer-generated forms and reports.
Maintain the legal aspects of pharmacy practice and regulations.
SPECIFIC PHARMACIST CONSULTANT DUTIES:
Perform Nursing home chart reviews by the 15th of each month. Report irregularities and recommend therapy changes to the medical director and the director of nursing, including:

Embraces Soleo's Quality Assurance and Performance Improvement initiatives, and plays an active role in supporting these programs
Assist in orientation and training of new pharmacy staff
Supervises activities of therapy care specialists and other support personnel
Ensures all pharmacy activities and documentation comply with state and federal law, accreditation body standards, Soleo Health policies and procedures, and payor, manufacturer, and referral source requirements
Obtains additional state pharmacy licenses as requested
Schedule:
Two schedules available: Monday-Friday 8:30am-5:00pm or 10:30-7pm
Rotating weekends
On-Call
Requirements
Prior experience in home infusion and/or specialty pharmacy highly preferred
Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience
Post graduate residency and/or training
Applicable active state licensure
Additional specialty certification or training may be required
Prior experience with CPR+ pharmacy computer system preferred
Excellent clinical and customer service skills
Basic to intermediate efficiency with Microsoft Excel, Word and Outlook

Qualifications:
Pharmacist
• Active, unrestricted Pharmacist license in any US state or territory
• Bachelor or Doctor of Pharmacy from accredited school

• IV admixture and immunization certification preferred
Pharmacy Technician
• Active, unrestricted Pharmacy Tech certification
• Minimum 1 year of technician experience
• Current BLS certification
Key Duties:
Pharmacist
• Review patient charts, process orders, check prescriptions
• Educate patients on medication therapy
• Collaborate with providers, answer drug therapy questions
• Administer immunizations (with certification)
• Handle controlled substance audits and IV admixtures as needed
Pharmacy Technician
• Fill and compound prescriptions
• Maintain inventory, prepackage medications
• Dispense refills, assist with inspections
• Support inpatient and outpatient pharmacy operations
Submission Requirements:
• Up to 6 CVs (Pharmacists and Pharmacy Techs combined)
• Include current resume, work history, copies of licenses, certifications, BLS, and education
• List years of healthcare experience, academic medical center or IHS experience if applicable
• Must meet IHS security clearance (fingerprint and OIG check) prior to start
