Job Description
Seeking an experienced Pharmacist in the Porterville, California area who is looking for a new opportunity with a prestigious healthcare company.
Candidate will be working at the hospital's in house pharmacy during the AM shift. Under the direction of the Pharmacy Director and/or the Assistant Pharmacy Director, assures safe and timely preparation and delivery of pharmaceuticals to patients and employees of the Hospital. Is responsible for the assessment, treatment or care appropriate to the ages of the patients served: i.e. Neo-Nate, Pediatric, Adolescent, Adult, and Geriatric. Demonstrates the ability to obtain and interpret information in terms of the patient's needs; possesses knowledge of growth and development; and has an understanding of the range of treatment needed for these patient groups. Provides requested information about drugs to physicians and other Hospital personnel.

Primary Accountability

Verifies merchandise against invoices upon delivery from vendors. Controlled medications to be counted and verified to match invoices.
Return outdated/expired, damaged, over stock, and recalled merchandise for credit and proper disposal.
Responsible for performing Medication Management Therapy for clinic patients per clinical protocols.
Responsible for functioning as a Pharmacist-In-Charge in the absence of designated Pharmacist-In-Charge (PIC).
Ensures Pharmacy compliance with all state and federal regulations as well as Family HealthCare Network policies.
Functions as a Pharmacy Tech or Pharmacy Clerk, as directed.
Performs other duties as assigned.
Description of Primary Attributes
Professional & Technical Knowledge:
Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in Pharmacy studies, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Pharmacy degree.
Technical Skills:
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.

This position requires onsite work at PACE customer center to further strengthen customer service between Grane Rx and the PACE center staff. This position will work full-time between onsite at SEEN Health and remote from the Team Member's home office.
KEY RELATIONSHIPS
The Pharmacy Technician Liaison reports directly to the Director of Pharmacy Services. This position works directly with center staff, nurses, providers, and leadership at the PACE center on a daily basis. This position also works with Grane Rx team members, including Pharmacy Operations team members, Consultant Pharmacists, Billing associates, and Inventory team.
Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES
On-site attendance at the PACE center on a routine basis as directed by Grane Rx leadership. Team member will primarily work onsite at PACE center, with the remainder of the work being completed at the team member's home office. Each week, team member will report to the PACE Center with specific days to be determined as collaborated between SEEN Health and Grane Rx leadership.
The team member will assist with the PACE center initiatives/questions.
This position will act as the point person for all initial questions or concerns for center and clinic staff. Team member will be responsible for escalating all quality-related occurrences to the Grane Rx Quality team, as well as escalating any service issues to Grane Rx leadership.
Team member will be tasked with the following directives on a continuous basis, including but not limited to:
Assist with daily pharmacy needs, including troubleshooting and resolving any quality-related occurrences;
Coordinate with White Glove Delivery management and couriers to assist with daily and cycle deliveries;
Train new center staff team members and conduct refresh trainings on pharmacy protocols;
Solicit feedback from center staff while observing PACE center protocols to improve upon mutual standard processes;
Act as a liaison for any customer concerns or escalations;
Process refill requests and tracking requests from clinic staff and participants;
Communicate with outside providers to support PACE participants' continuity of care;
Assist with reordering and maintaining center stock supply while supporting centers' protocols surrounding center/provider stock;
Obtain clarifications from providers for any clinical questions sent from the Grane Rx Customer Care team.
Team member shall maintain a Leader Standard Work report, which will be due to the Director of Pharmacy Services each week.
Identify and escalate priority issues to Pharmacist-in-Charge and/or appropriate department manager(s).
Follow procedures, guidelines/SOPs to ensure compliance is maintained.
Competent in software systems that include FrameworkLTC, FrameworkECM, Salesforce, and Automated Packaging System.
Additional duties as assigned by manager.
Qualifications
PREFERRED EXPERIENCE, EDUCATION, AND SKILLS
Certified, licensed pharmacy technician in the state of California preferred.
Preferred pharmacy experience.
Preferred experience in long term care or PACE pharmacy.
High School Diploma or equivalent required.
Proven customer support experience - passion for excellence with respect to treating and caring for customers.

The Pharmacy Technician assists in preparing medication orders; coordinating the medication supply chain; and maintaining records and medication inventory in accordance with policies, laws, and regulations.
Minimum Qualifications:
Possession of a valid certificate of registration as a Pharmacy Technician issued by the State Board of Pharmacy set forth in Section 4115 and 4202 of the Business and Professions Code is required.
Registration requirements shall not apply during the first year of employment for a person employed or utilized as a Pharmacy Technician to assist in the filling of prescriptions for an inmate of a correctional facility of the State Departments of Youth Authority or Corrections, or for a person receiving treatment in a facility operated by the State Department of Veterans Affairs.

Receives E-script orders from providers and new prescriptions from patients
Verifies patient specific information and enters/reviews data into the Pharmacy Practice Management System.
Processes labels for new and refill prescriptions.
Makes appropriate contact with prescribers to obtain approval or denial of refills and document refill orders accordingly.
Selects appropriate product from stock by comparing National Drug Code (NDC); counts, measures and pours medication in vials or containers.
Verify the entire information on the prescription label by comparing with original prescription, initial the label then affixes the label on vial or container.
Presents hard copy prescription, finished package, patient receipt, monograph and stock container together for final review.
Places filled and reviewed prescriptions on the shelves to await patient pick up
Completes and processes third-party documents and transmissions, authorizations, and TAR (Treatment Authorization Request).
Check and process phone or fax messages periodically.
Keep the Pharmacy work areas clean and organized.
Files paperwork and prescriptions under supervision of the Pharmacist-in-charge (PIC)
Operates cash register during patient prescription pick up, processes cash and credit card transactions, and obtains patient signatures for insurance log documentation.
Keeps Pharmacist informed.
Warning or Drug Utilization Review (DUR) screen appears during label processing.
Prescriptions requiring Pharmacist consultation with the patient.
Departmental policies and procedures and progress thereof.
Monitors prescriptions in the wait area.
Checks fill date.
Runs will call reports.
Removes all prescriptions over seven days old or as instructed by PIC.
Returns pulled medications to stock.
Reverses orders online.
Delivers medications and prescriptions to other sites and maintains accurate delivery logs.
Monitors medication stock levels as directed by pharmacist.
Processes incoming medication orders.
Compares items shipped to items ordered on invoice.
Affixes price stickers.
Places items on shelves or in refrigerator.
Checks stock expiration dates on a monthly basis and rotates or removes items.
Returns outdated/expired, damaged, overstock and recalled merchandise for credit and proper disposal.
Assists PIC with inventory process.
Performs other duties as assigned.
