Ensure that requisitions are completed properly, and the patient demographic, diagnosis, insurance and physician information are included on or attached to the requisitions. Diagnostic information shall be in a narrative or ICD-9 code format and valid only if legible. 4. If a patient is a Medicare patient, you must review the National Coverage Decisions (“NCD”) and Local Medical review Policies (“LMRP”) for the tests ordered on the requisition form to determine if an Advance Beneficiary Notice (“ABN”) must be completed. If the form is needed, correctly complete the form with the patient. 5. Ensure that the Medicare Secondary Form (“MSP”) is completed when necessary. 6. Accurately collect all specimens necessary to fulfill the clients' lab orders. 7. Provide accurate communication to clients and their staff regarding the proper ordering of lab tests. 8.
