Diagnostic information shall be in a narrative or ICD-9 code format and valid only if legible. 4. If a patient is a Medicare patient, you must review the National Coverage Decisions (“NCD”) and Local Medical review Policies (“LMRP”) for the tests ordered on the requisition form to determine if an Advance Beneficiary Notice (“ABN”) must be completed. If the form is needed, correctly complete the form with the patient. 5. Ensure that the Medicare Secondary Form (“MSP”) is completed when necessary. 6. Accurately collect all specimens necessary to fulfill the clients' lab orders. 7. Provide accurate communication to clients and their staff regarding the proper ordering of lab tests. 8. Ensure that draw station where you work is kept clean and adequately stocked with supplies every day. 9. On a daily basis, inform your Operations Supervisor of your job performance issues, concerns and schedule. 10. If you work for UWL and are located within a physician's office, you are not permitted to do any work for the physician and/or the physician's medical practice, your sole responsibility is to collect specimens to be sent to a UWL client's laboratory.
