The Phlebotomist III position at an inpatient medical center involves greeting patients, performing blood draws, labeling and centrifuging specimens, recording maintenance data, and updating patient information. Responsibilities
+ Greet patients as they arrive at the lab.
+ Perform blood draws using straight needles and/or butterfly needles.
+ Label, centrifuge, split, and freeze specimens according to test orders.
+ Ensure specimens are stored at the required temperature and placed in the appropriate collection containers.
+ Match laboratory requisition forms to specimen tubes.
+ Check test requisitions or computer labels against scripts to ensure accuracy.
+ Package specimens for transport, maintaining the integrity of the samples.
+ Comply with OSHA and DEP regulations and attend annual department trainings.
+ Answer telephone calls and provide laboratory results to inquiries.
+ Prepare collection sites, handle insurance and billing, and process samples.
+ Provide excellent customer service to patients.

Required Education

Department Description
The laboratory team at Santiam Hospital has the responsibility for testing samples that may include blood, body fluids and tissue to aid in diagnosis of diseases, illnesses or conditions that may need medical attention. Lab tests are processed for hospital inpatients and emergency room patients twenty-four hours a day. In addition, testing is performed on outpatient and clinic specimens. Job Description
Provide service to Hospital and Medical Clinics patients by identifying optimal methods for specimen retrieval, preparing such specimens for laboratory testing, and performing screening procedures.
Prior to specimen collection, verify test requisition information via applicable Hospital staff and patient identification.
Using aseptic technique, obtain blood specimens and buccal, nasopharyngeal, sputum, and urine samples.
Track collected specimens with accurate and thorough labeling technique, in accord with Laboratory Service protocol.
Perform tests and document controls for testing waived in the Clinical Laboratory Improvement Amendments.
Perform urinalyses using Service equipment.
Perform routine, manufacturer-recommended maintenance on Service equipment, including centrifuges and urinalysis machines
Prepare and send specimens for external analysis and communicate with entities that provide such analysis.
Provide patients and patient families with indicated educational materials and instruction about patient specimen collection methods, including those for fecal occult blood, gastrointestinal panels, twenty-four hour urine output, and clean catch and non-clean catch urine samples.
Monitor inventory of supplies used to execute functions of position, and properly notify designated staff when such supply requires restock.
Perform office tasks, including mailing chain of custody documentation, scanning testing results, and faxing documents.
Participate in Service quality control activity as directed by Laboratory Director.
Respond tactfully and appropriately to evaluation that results from Service quality control and Performance Improvement activity, applying conclusions of such evaluation to improve personal performance.
Annually demonstrate competency in all pertinent proficiency and competency testing.
Perform other duties assigned by Phlebotomy Lead and Laboratory Director.

St. Charles operates outpatient clinics in three Central Oregon counties. Lines of clinical service including Primary Care, Urgent Care, Medical Specialties, Women's Health, Post-Acute Care, Hospital Medicine, Emergency Medicine, Behavioral Health, Cardiovascular Services and Cancer Services. POSITION OVERVIEW: The Phlebotomist at St. Charles Health System processes laboratory orders and performs phlebotomy procedures/collections with as little discomfort as possible to patients of all ages. This position performs phlebotomy procedures following the standards of practice as outlined in System policies and procedures and while working within the boundaries of the Phlebotomy Scope of Practice. This position does not directly manage other caregivers, however may be asked to review and provide feedback on the work of other caregivers.

Processes and tests samples using various instruments and processes following laboratory standards. Performs POC testing, including CLIA waived testing, moderate complexity testing and QC.
Navigates through multiple complex computer systems to find MD orders, update, and verify patient information, research billing questions, place orders, and meet regulatory requirements for Medicare and other payers.
Thinks critically, multitasks, and maintains accuracy during periods of frequent interruptions.
Facilitates the distribution of results via fax, phone, or delivery, as designated or as requested by clients.
Assists in special projects as needed.

Additional Duties for Laboratory Phlebotomists:
Greets clients upon arrival at the outpatient draw station or area. Greets patients appropriately according to Hospital surroundings, situation, and time of day.
Assists the RN and MD as requested in the collection of blood samples and during specialty procedures. Including, but not limited to:
* Adrenal Study blood collections
* MRI pediatric blood collections
* Lumbar Puncture blood collection
* Bone Marrow collections
* Legal blood collection with law enforcement
* Arterial blood collection.
Responds quickly to codes, traumas, rapid responses, and can maneuver in tight quarters. Performs AHA BLS when asked to participate in the rotation by an RN or MD.
Manages multiple patient types including but not limited to confused, combative, upset, intubated, restrained, psychiatric emergency services on-site and off-site, traumatized, and nonresponsive individuals in varying situations including some that can be visually disturbing.
Has a clear understanding of how to accurately select the correct patient and assign an encounter number.
Registers all patients for laboratory services as ordered by their physician and in compliance with St. Charles Health System policies and procedures. Includes collecting payments from patients for specialty kit draws. Selects the correct insurance plan, and sends and reviews eligiblity responses.
Researches tests and diagnostic codes according to billing and insurance requirements, mainly pertaining to Medicare and other government funded insurance programs, according to Medicare Compliance Act; informs patients regarding expenses beyond insurance reimbursement and allows the patient to make decisions as to whether to follow through with lab services based on current guidelines before tests are drawn.
Has a clear understanding of all downtime processes.
EDUCATION:
Required: High school diploma or GED
Preferred: Participation in college level science classes.
LICENSURE/CERTIFICATION/REGISTRATION:
Required: AHA Basic Life Support for Healthcare Provider certification.
Preferred: Phlebotomy certification through a St. Charles Health System approved accredited agency is highly preferred. The current approved agencies are the following: ASCP, NCA, NCPT, and phlebotomy specific military training programs.
EXPERIENCE:
Required: N/A
Preferred: Previous direct patient care experience (preferably Lab experience) and/or college science background.

Job Description
The Patient Services Representative II (PSR II) represents the face of our company to patients who come to Quest Diagnostics, both as part of their health routine or for insights into life-defining health decisions. Job Accountabilities (Responsibilities)
1.Collect specimens according to established procedures. This includes, but not limited to: drug screens, biometric screening and insurance exams.
2.Administer oral solutions according to established training.
3.Research test/client information and confirm and verify all written and electronic orders by utilizing lab technology systems or directory of services.
4.Responsible for completing all data entry requirements accurately including data entry of patient registration; entry of test order from requisition or pulling order from database; managing Standing Orders.
5.Enter billing information and collect payments when required, including the safeguarding of assets and credit card information.
6.Data entry and processing specimens including: labeling, centrifuging, splitting, and freezing specimens as required by test order.
7.Perform departmental-related clerical duties when assigned such as data entry, inventory, stock supplies, and answer phones when needed.
8.Read, understand and comply with departmental policies, protocols and procedures: (i.e. Procedure Manuals, Safety Manual, Compliance Manual, Automobile Policies and Procedures, Employee Handbook, Quality Assurance Manual); and ensure that all staff members follow instructions.
9.Perform verification of patient demographic info / initials including patient signature post-venipuncture to verify tubes were labeled in their presence and that the name on the label is correct.

We are seeking a dedicated Phlebotomist I to join our team. The Phlebotomist will play a crucial role in providing excellent customer service and performing essential laboratory tasks. Essential Skills
+ At least 6 months of phlebotomy experience.

+ Familiarity with medical terminology and specimen processing.

Qualifications
Job Accountabilities (Responsibilities)
1.Collect specimens according to established procedures. This includes, but not limited to: drug screens, biometric screening and insurance exams.
2.Administer oral solutions according to established training.
3.Research test/client information and confirm and verify all written and electronic orders by utilizing lab technology systems or directory of services.
4.Responsible for completing all data entry requirements accurately including data entry of patient registration; entry of test order from requisition or pulling order from database; managing Standing Orders.
5.Enter billing information and collect payments when required, including the safeguarding of assets and credit card information.
6.Data entry and processing specimens including: labeling, centrifuging, splitting, and freezing specimens as required by test order.
7.Perform departmental-related clerical duties when assigned such as data entry, inventory, stock supplies, and answer phones when needed.
8.Read, understand and comply with departmental policies, protocols and procedures: (i.e. Procedure Manuals, Safety Manual, Compliance Manual, Automobile Policies and Procedures, Employee Handbook, Quality Assurance Manual); and ensure that all staff members follow instructions.
9.Perform verification of patient demographic info / initials including patient signature post-venipuncture to verify tubes were labeled in their presence and that the name on the label is correct.
10.Assist with compilation and submission of monthly statistics and data.
11.Maintain all appropriate phlebotomy logs in a timely manner and based on frequency, such as maintenance logs and temperature logs.
12.Complete training courses and keep up-to-date with the latest phlebotomy techniques.
13.Travel to Territory Manager meeting if held off-site or off normal shift.
14.Participate on special projects and teams.
15.Stay-up-to date on company communications and assist with the distribution of technical information to the work group.
16.Perform Point of Care (POC) testing at those sites where needed and the complete training/competency evaluations per Standing Operating Procedure (SOP).
17.With appropriate training, act as mentor and resource for new employees, assisting with transition into the PSC work environment and the familiarity with established procedures.
18.Assist with periodic inventory counts, report shortages and problems to group leader or supervisor as they occur.
19.Assist with the preparation of schedules for the assigned work group or PSC's.
20.Communicate professionally with clients to resolve or refer, and document problems, prepare problem documentation and report critical issues as they occur.
21.Ensure staff is following all safety precautions by wearing a clean, button lab coat, gloves and face shield when required.

23.Ensure facilities are neat, clean and in good repair, takes appropriate action to advise Group Leader or Supervisor of required repairs and maintenance.
