Job Description
Under the supervision of a provider or registered nurse, participates in the assessment, planning, implementation, and evaluation of care. Performs nursing process while providing direct patient care in a safe and effective manner that promotes quality patient outcomes. Functions as a member of the interdisciplinary healthcare team. If working in non-provider based, physician practice sites, utilizing the medical model of care delivery, patient assessments, plan of care, and discharge plan are performed by a physician, nurse practitioner (NP), or physician assistant (PA). Under this model, the LPN assists the provider in providing prescribed care or treatment to the patient.
