Manages, coordinates, facilitates and supports the members of the interdisciplinary team in provision of hospice care to patient/families of VNS Health Hospice Care Program, including After Hours. Ensures the implementation of clinical/quality improvement initiatives for hospice services in compliance with regulatory requirements. • Ensures the use of an interdisciplinary approach to assess the medical, physical, social, emotional and spiritual needs of the patient. Ensures that support and direction to patient's families are provided in accordance with the patient's Interdisciplinary Plan of Care/Hospice Care mission, objectives, policies and procedures.
• Participates in Hospice Team meetings, IDG management meetings, and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training.
• Ensures full regulatory compliance with Hospice Conditions of Participation and standards through review, monitoring and audit (ad hoc / regular) of files, processes and procedures.
• Collaborates with Quality Improvement/Education in the development and implementation of quality improvement and educational activities to ensure professional practice standards are consistently met for each member of the team. Participates in the design, implementation, evaluation and modification of quality initiatives and educational plans.
• Initiates, coordinates, performs and assists in the analysis of clinical, regulatory and fiscal practice. Assists leadership in achieving Hospice Care goals, objectives, and fiscal targets by providing support with compliance and/or innovation to achieve improvement.
• Manages the day to day operations of the department, which includes effective caseload or visit management, achievement of productivity and revenue/expenditure targets for Hospice Care.
• Completes all work assignments, which includes audits, reports and projects.
• Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, and terminates staff and recommends salary actions as appropriate.
• Facilitates and manages the interdisciplinary group for provision of an effective and comprehensive hospice services to patient/families of VNSNY Hospice Program.
• Participates in special projects and performs other duties as assigned.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred
Education:

What you will do:
Follows established physical therapy plan of care, including physical therapy interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice. Documents all care provided and maintain records per VNS Health policy and standards.
Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy.
Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. PT will re-assess patient every 6 visits or 30 days (whichever comes first).

Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.

The Patient Financial Representative is responsible for the accurate and timely verification of insurance and benefit information for patients receiving inpatient medical, inpatient psychiatric, observation, ambulatory surgery and/or outpatient procedure related services. Plays a key role in the organization's financial health by obtaining or ensuring that insurance authorizations or pre-authorizations are on file and accurate prior to the service being rendered. This position must also ensure patient demographic and insurance information is correct, resulting in accurate claims for reimbursement. Core Job Responsibilities
For designated services, this position is responsible for ensuring that each patient account has accurate insurance information entered in the correct billing order and that each insurance listed has been verified as eligible for the designated date of service range. For each insurance, benefit information is obtained and documented. Verification and benefit information can be obtained via electronic or verbal method but must be completed prior to services being rendered. Position must have or develop excellent working rapport with surgeons' office staff, as well as hospital nursing staff.
For pre-scheduled services, this position is responsible for verifying that authorization is on file with each of the appropriate insurance companies and that authorization is accurate based upon location, CPT code, service type, surgeon, date range and any or all other necessary elements to secure payment for services rendered. For emergent or urgent services, this position is responsible for accurately and timely requesting that each verified insurance company has been notified of patient services being rendered and also request authorization for requested services. What You Will Do
Provides clinical supervision to staff including assigning, monitoring and evaluating cases for clinical team(s). Conducts regularly scheduled team meetings. Provides back-up coverage for program leadership as required.

Job Requirements
Responsibilities:
Perform consults with patients by reviewing health and medical histories; questioning, observing, and examining patients.
Evaluate patients using chiropractic diagnosis, functional movement assessments, and examination procedures to assess neuro-muscular and spine-related conditions.
Perform Chiropractic adjustments to the spine, or other articulations of the body, in order to correct the musculo-skeletal system, where necessary.
Practice a variety of therapeutic modalities including chiropractic, progressive rehab, spinal decompression, Class IV laser therapy, nutritional support, and custom orthotics.
Support the clinical operations of the clinic, including taking and reading x-rays.
Lead, direct, and support patient rehabilitation activities and exercises.
Educate patients on the quality of life and benefits of chiropractic care. Provide a recommended course of treatment.
Maintain accurate case histories of patients.
Obtain and record patients' medical histories, as indicated.
Conduct patient chiropractic care and education
Build positive doctor-patient relationships
Maintain accurate and timely patient records
Recommend and explain details of the care plan
Perform community outreach
Build referral relationships with businesses and other healthcare providers
