Essential Functions & Responsibilities
The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
Physician must be able to supervise collaborating nurse practitioners and physicians assistants

SCOPE OF ROLE
Reporting to the Chief Medical Officer, and under the direction of the site-level Associate Medical Director, and the organization's Medical Director of Psychiatry, a Psychiatrist at Signature Health, Inc. provides compassionate, supportive, patient-centered psychiatric medical care to our patients. Our Psychiatrists work diligently to help improve the daily lives of our patients by providing evaluations/consultations, diagnosis, monitoring of progress, and pharmacologic and other appropriate psychological health treatment services. Our Psychiatrists work collaboratively with Signature Health staff, including our primary care physicians, counselors, case managers, and other outside sources to provide quality holistic care for our patients.
HOW YOU'LL SUCCEED
Providing ongoing consultation and management for assigned patients as requested or as necessary in accordance with accepted standards of psychiatric practice.
Providing psychiatric services on the days and at the times established and scheduled by Signature Health. Our expectation is that the Psychiatrist's schedule will be maximized during his/her clinical schedule in order to best serve all our clients in need and to maximize access to services for new clients. This may include providing services on a same-day basis or providing care to patients using a walk-in service model.
Utilizing the electronic medical records system to timely and accurately fulfill all administrative responsibilities attendant to his/her performance of direct patient care, including but not limited to, entry and completion of progress notes detailing all patient encounters, services provided and treatment plans recommended; electronic order entry of medications and diagnostic tests; and other required documentation in accordance with the policies and procedures of Signature Health and/or other relevant accrediting or regulatory bodies. Psychiatrist must ensure compliance with Signature Health's note completion policy.
Preparing and attending to all reports, claims, and correspondence (including email) necessary or appropriate to the performance of the professional services. Email is a regular form of communication within Signature Health and therefore prompt response, generally a response within 24 hours (or sooner as medically/business appropriate) is required.
Referring patients to other services within the agency, other physicians, including specialists and other health care providers, when and if needed.
Maintaining competence in his/her area of specialty by attending professional conventions or seminars as well as participating in educational activities offered by professional societies on a reasonable basis.
Providing coverage when other physician employees/contractors are unavailable due to vacations, holidays, illness, other personal or professional absences, or as required by Signature Health.
Be available to take calls as assigned by the Agency.
Timely reporting incidents, including major unusual incidents, and other information required to appropriate governmental authorities and as required for Agency reporting.
Participating in Agency required quality assurance, peer review, compliance, and credentialing/privileging activities.
Attending administrative meetings, as needed.
Serving as a collaborating physician, as needed, and/or may provide supervision to Psychiatric Nurse Practitioners, residents, interns or others.

13 infusion chairs in two separate rooms (6 or 7 in each room), and 5 private rooms for long term infusions. We also handle non-infectious OP infusions and blood transfusions on our floor. Average infusions/injections is between 15-20/day. This is in addition to the consultations and follow up patients. They are treated and seen in the same area.

Psychiatrist role is to oversee the mediation prescribed. They continue with their current psychiatrist and the Psychiatrist, at this time, does not write for any of the medication. If med issues do arise, the collaborater is to contact the psychiatrist but rarely speak with them. Usually a message is left with their nurse recommending a change in current meds. Sometimes they listen, sometimes they don't. Psych evaluations (all ages) are required but they are not managing medication. The clientele is very stable and the provider would only be doing 1-2 evaluations every 2-3 months

Essential Functions & Responsibilities
The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
Physician must be able to supervise collaborating nurse practitioners and physicians assistants

SCOPE OF ROLE
Reporting to the Chief Medical Officer, the Physician, Dermatology will execute the delivery of high-quality, patient-centered, and community-based care in dermatology. The Physician, Dermatology will provide dermatologic services to Signature Health patients, including evaluation, diagnosis, treatment, and ongoing follow-up at our Willoughby, Painesville and Ashtabula locations. The Physician, Dermatology will refer patients to specialists and to relevant patient care components as appropriate.
HOW YOU'LL SUCCEED
Conduct physical examinations of patients to assess skin, hair, and nail conditions.
Diagnose and treat common dermatologic conditions, including but not limited to acne, atopic dermatitis, psoriasis, infections, and benign and malignant skin lesions.
Order and interpret laboratory and diagnostic tests; conduct specimens for pathologic exam.
Provide counseling and education to patients regarding skin care and disease prevention.
Perform minor surgical procedures, such as simple excisions and closures, biopsies, administration of local anesthesia, cryosurgery, curettage, cautery, intralesional injections.
Document all services, communications, and medical decision-making in the electronic health record.
Monitor patient progress and adjust treatment as needed.
Provide consultation to Signature Health clinical providers, providing staff presentations as needed.
Stay up to date on the latest advancements in dermatology through research and continuing education.
Requirements
KNOWLEDGE & EXPERIENCE
Valid unencumbered Doctor of Medicine or Doctor of Osteopathic Medicine in the state of Ohio is required.
Board Certified in dermatology required.
3+ years of experience required.
American Heart Association (AHA) Basic Life Support (BLS) certification required.
Experience in a Community Health Center preferred.
