We are primarily an outpatient physical therapy company. However, we have partnered with a few healthcare providers offering rehabilitation services in various settings.
We currently have an opening for a Per Diem (PRN) Physical Therapist to join our Inpatient rehabilitation team at St. Anne Hospital in Burien, WA. This opportunity is in an inpatient acute setting. St. Anne Hospital (formerly Highline Medical Center) is a 159-bed community hospital located in Burien, Washington. St. Anne offers a full range of medical and clinical services, including a birth center, oncology and orthopedic care, and a specialty stroke care unit. We are looking for someone who is determined and committed to providing patients with the best treatment experience around and someone who is ready to help grow our organization and the field of physical therapy.
Position Summary:
Conduct comprehensive evaluations of patients to assess their physical condition and functional limitations.
Develop customized treatment plans based on assessment findings and in accordance with physicians' orders.
Administer therapeutic exercises, manual therapy techniques, and modalities to improve patients' strength, flexibility, and mobility.
Monitor patient progress regularly and adjust treatment plans as needed to achieve rehabilitation goals.
Educate patients and their families on proper techniques for at-home exercises and activities to support recovery.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure a holistic approach to treatment.
Qualifications:
Graduation from a physical therapy curriculum approved by the American Physical Therapy Association.
Current State licensure for the practice as a Physical Therapist

The Mobile Clinic Coordinator plays a key role in ensuring a smooth and welcoming experience for patients in the mobile health setting. This position coordinates and supports mobile health services at community sites to reduce barriers to healthcare access and strengthen community relationships. The role oversees daily operations of mobile medical, dental, and vaccine clinics, supervises support staff, and maintains strong partnerships with site partners through effective communication, logistics coordination, and agreements or memoranda of understanding (MOUs). Primary responsibilities include patient registration, scheduling, referral coordination, and medical record management to ensure continuity and quality of care. The CCC also supports program compliance, reporting, and data tracking to help evaluate service delivery and improve outcomes. Job Summary:
The PACE Clinical Services Manager is responsible for the overall management and operation of the PACE nursing team, the PACE clinic at Ron Chew Healthy Aging and Wellness (RC-HAWC), and the PACE Home Care program. Works closely with the PACE Center Manager and reports to the PACE and Senior Services Director, and secondarily to the PACE Medical Director. In conjunction with the PACE Center Manager and PACE and Senior Services Director, responsible for the continued development and expansion of PACE Home Care services. Serves as the Clinical Manager for PACE home health services. This position is a backup member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), and meets applicable state licensure requirements to represent the Registered Nurse and Home Care Coordinator roles.
Responsibilities:

Under the supervision of the PACE Center Manager, and as a member of the Program of All-Inclusive Care for the Elderly (PACE) interdisciplinary team, the Day Center Supervisor assesses the needs, interests and capabilities of participants and works with Recreational Therapists and other team members to develop individualized therapeutic recreation plans. This role provides leadership, supervision and support to the Recreational Therapy team. As a Registered Nurse /Licensed Practical Nurse is responsible for providing clinical expertise, supervision, and support to the staff. This includes direct clinical care, family/caregiver education, oversight of Caregivers, and working collaboratively with an Interdisciplinary Team (IDT) to provide holistic care for our participants. This position is a member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), as the Day Center Supervisor and backup for the Personal Care Attendant Representative. As an RN or LPN, Day Center Supervisor is responsible for day center coverage, services, managing risk, medication management, and overall programming.

The Mobile Clinic Coordinator plays a key role in ensuring a smooth and welcoming experience for patients in the mobile health setting. This position coordinates and supports mobile health services at community sites to reduce barriers to healthcare access and strengthen community relationships. The role oversees daily operations of mobile medical, dental, and vaccine clinics, supervises support staff, and maintains strong partnerships with site partners through effective communication, logistics coordination, and agreements or memoranda of understanding (MOUs). Primary responsibilities include patient registration, scheduling, referral coordination, and medical record management to ensure continuity and quality of care. The CCC also supports program compliance, reporting, and data tracking to help evaluate service delivery and improve outcomes. Additional duties include ordering and maintaining inventory of office and medical supplies (including vaccines), coordinating vehicle maintenance and readiness, and collaborating with ICHS programs and community partners to promote services and outreach to patients, families, and the broader community.

Job Summary:
The PACE Clinical Services Manager is responsible for the overall management and operation of the PACE nursing team, the PACE clinic at Ron Chew Healthy Aging and Wellness (RC-HAWC), and the PACE Home Care program. Works closely with the PACE Center Manager and reports to the PACE and Senior Services Director, and secondarily to the PACE Medical Director. In conjunction with the PACE Center Manager and PACE and Senior Services Director, responsible for the continued development and expansion of PACE Home Care services. Serves as the Clinical Manager for PACE home health services. This position is a backup member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), and meets applicable state licensure requirements to represent the Registered Nurse and Home Care Coordinator roles.
Responsibilities:
Lead and manage the clinical services team, including nurses, therapists, social workers, and other healthcare professionals, to ensure effective and compassionate care delivery.
Develop, implement, and monitor individualized care plans in collaboration with interdisciplinary teams to meet the unique needs of each participant.
Ensure compliance with all federal, state, and local regulations governing PACE programs, including documentation, reporting, and quality assurance standards.
