Job Summary
Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients/residents.
May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Will be interacting with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. Will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership. Performs other related duties as requested by manager.
Based on facility needs and assigned locations, may focus on those receiving intervention in the post-acute, long term care and home health care environments.
Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.
When applicable, may require valid driver's license.
Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

About this position
The Physical Therapist is a member of the Therapies Center team who evaluates the physical and/or cognitive needs of a customer, establishes a care plan and provides or directs the individualized care of a customer within the specific scope of practice. The desired outcome of care is typically to prevent further impairments, improve function and the ability to participate in activities in order to promote greater independence and reduce the need for further care. The Physical Therapist also serves to educate the customer, their caregivers or family about their role in the care plan. The Physical Therapist functions as a part of the North Memorial Health interdisciplinary care team, collaborating as needed with other clinical and non-clinical team members in order to provide unmatched customer service. The Physical Therapist is capable of and willing to function in all locations and areas where care is provided by the Therapies Center. The Physical Therapist participates in training, orientation and education as required to provide care to specific diagnostic groups, units or locations as needed, and is competent to provide at least core or entry level skills with typical customers. Education
* Baccalaureate or Master's degree from an accredited school of Physical Therapy. Doctorate preferred.
Experience
* A minimum of diagnostic group, hospital based acute care, inpatient rehabilitation care or outpatient neurological based clinical experience during training, residency or fellowship for new graduates. Demonstrates entry level competency and skills.
* A minimum of two years clinical experience for practicing therapists without experience in the Therapies Center diagnostic groups, units or locations of care.

Hours: Full-time - Days, Monday - Friday | There will also be a weekend rotation. Dependent upon staffing levels, work one weekend approximately every 6 weeks.

Territory: The Physical Therapist will have a general territory within our service area where they will provide one on one in-home care to the patients. The office is in Roseville, MN and the general territory for this position would primarily be in the southwest section of the Twin Cities metro area.

A Few Must-haves for This Position:
Currently licensed as a Physical Therapist in the state of Minnesota.
In the last five (5) years, two (2) years of experience as a Physical Therapist that is relevant to the patient population assigned preferred.
Able to travel within geographic area serviced by office. Valid drivers license, reliable transportation, and proof of auto insurance is required.
CPR/BLS certification
Some functions of what you will do as a Physical Therapist:
Assists the Physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs as well as in developing the resulting plan of care/treatment including appropriate long and short-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Regularly assesses changes in the patient's status since the last visit/treatment day, and determines whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the Physician as indicated.
Advises and consults with the family and/or other caregivers to promote the patient progress toward mutually agreed upon goals and planning for discharge.
Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote the coordination of patient care and planning for discharge.
Completes and submits timely, legible, accurate, relevant and sufficient documentation
