Job Description
Provides direct Physical Therapy (PT) treatment under the direction of the Physical Therapist in accordance with the physician's orders and as outlined in the PT Plan of Care.
Monitors patient's status and home environment making sure patient is successfully completing treatment as delegated by the registered Physical Therapist.
Reports to registered Physical Therapist any changes in patient's condition and/or needs.
Maintains communication with other Agency personnel involved in patient's care to promote coordinated care.
Prepares, submits and maintains accurate, relevant and timely clinical documentation per Agency policy.
Attends mandatory agency staff meetings, care conferences and educational offerings.
Perform administrative tasks such as keeping records, writing reports and making telephone calls.
Maintain current CPR certification, Tuberculosis and all other required licensures.

Holiday Closures: SBBC closes for the following holidays each year: New Year's Eve, New Year's Day, Martin Luther King, Jr, Washington's Birthday, Good Friday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and the following Friday, Christmas Eve and Christmas Day.
Additional Time Off: 5hrs accrued for every 150hrs worked
Mileage: Fuel between the first location of the day and the remaining locations will be reimbursed based on the current federal mileage rate. Miles need to be documented and submitted no later than a week before pay day.
Retirement: SBBC will match 100% of employee contributions on the first 3% deferred, then 50% of contributions from 3% to 5%.
