This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

Summary:
Works under the administrative direction of the Supervisor of Physical Therapy. Provides physical therapy treatments to patients under the clinical supervision of a licensed physical therapist. Clinical Duties:
* Assists supervising physical therapist in the overall assessment of the patient.
* Performs but does not interpret selected measured procedures such as joint range of motion, gross strength of muscle, and length and girth of body parts.
* Assess general mental, emotional, and cognitive status in frequently referred diagnoses.
* Obtains general subjective information to assist in the evaluation.
* Assists in the provision of patient treatment.
* Upon completion of the initial evaluation by a supervising PT, initiates treatment according to the prescribed plan of care. Alters treatment only after approval by the supervising PT.
* Utilizes subjective and objective results supplied by supervising PT to guide treatment approaches.
* Reviews chart and/or obtains current information before treatment session and identify information that may necessitate modification of PT treatment. Modifies treatment procedure if indicated by patient response and within the limits of the plan of care. Seeks guidance from supervising PT if a plan of care needs to be changed.
* Assesses patient response to treatment and reports unexpected responses to supervising PT. Documents incidences with assistance.
* Instructs patients, nursing, and family regarding the importance of participation in the PT program.
* Involves patient and family, as appropriate, in treatment and discharge planning.
* Recommends (for use while in the hospital and/or at discharge) necessary durable medical equipment or other therapeutic devices that promotes optimal functioning and patient safety. Seeks guidance from supervising PT for equipment selection and most cost-effective materials.
* Utilizes appropriate modalities that include but are not limited to heat, cold, light, water, sound, and electricity. Utilizes exercise equipment, general strengthening, and range of motion exercises.
* Can present rationale for selection of a particular procedure.
* Participates in patient care and team conferences, clinics, and rounds to coordinate patient care. Makes a verbal/written report regarding the patient's status when requested.
* Incorporates safety, infection control, and emergency procedures into treatment.
* Charts and authenticates progress notes and treatment summaries promptly.

* Has a thorough knowledge of policies and procedures. Frequently refers to Policy & Procedure Manual in carrying out job responsibilities.
* With occasional guidance, organizes their schedule to reflect a "Patient first" philosophy. Rearranges schedule, coordinates with peers, and asks for assistance when necessary.
* Carries a patient load that reflects the productivity standard of the unit.
* Demonstrates appropriate written communication.
* Is responsible for written documentation that includes but is not limited to progress notes, home programs, discharge summaries, statistical reporting, charges, and interdisciplinary forms.

The Physical Therapist Assistant program mission is to prepare diverse, active learners to serve their communities by enhancing quality of life as entry-level physical therapist assistants through quality instruction in a supportive environment. The Physical Therapist Assistant (PTA) program has an emphasis on experiential learning, students gain a strong foundation in theory and hands-on training to assist in implementing treatment plans, promote patient safety, improve functional outcomes, and support quality, and cost-effective care across healthcare settings. Instructional technology is integrated throughout the curriculum, including immersive experiences in our state-of-the-art Center for Interprofessional Simulation and Innovation, where students engage in realistic clinical simulation to strengthen critical thinking, teamwork, and patient care skills. Graduates are prepared for opportunities in a wide variety of practice environments, including hospitals, outpatient clinics, rehabilitation centers, skilled nursing facilities, home health, and schools. Accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), the A.A.S. PTA degree can be completed in 24 months and includes a curriculum focused on anatomy, kinesiology, therapeutic exercise, patient care skills, and clinical education experiences.
The successful candidate will be able to teach a wide variety of courses, including cardiopulmonary physical therapy, to students of diverse backgrounds and varying levels of college readiness using traditional and alternative delivery methods. We encourage interested candidates to familiarize themselves with our course offerings. This position teaches a standard load of fifteen (15) credit hours per semester (30 credits per academic year) physical therapist assistant courses.
Schedule Information: The successful candidate is expected to teach 30 contact hours each year in the Physical Therapist Assistant (PTA) program. In addition to teaching 30 contact hours per academic year, a full-time faculty member serves on departmental, divisional and institutional committees; holds six office hours per week (some of which must be on campus); and pursues professional development opportunities related to discipline and pedagogy. Evening and weekend instruction or meetings may be required. Instruction may take place face to face, online, blended or flex formats. Most of the PTA courses are in a face to face format.
Weeks per fiscal year: Faculty are covered under a 9-month contract period (August thru May) - We seek a full-time colleague to begin in August 2026. The physical therapist assistant program is a year-round program, additional course load taught during the summer is paid based on our faculty agreement summer pay schedule.

The Physical Therapist Assistant treats patients for difficulties with mobility, function, strength and positioning and scope of practice as outlined by APTA and licensure laws in respective state under the supervision of the Licensed Physical Therapist. The PTA may also assist in conducting in-services and training for facility staff on an ongoing basis in restorative and functional maintenance programs. The Physical Therapist Assistant reports to Licensed Physical Therapist and/or Director of Operations and is responsible to Therapist Program Manager. The PTA communicates and collaborates with families, physicians and other health team members and maintains impeccable, professional documentation of services in the medical record. CRI's PTs model and promote competent, courteous and professional services to internal and external customers to develop and maintain a competitive advantage.

Requirements
ESSENTIAL FUNCTIONS OF THE JOB
Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical
Therapist.
Communicate with the Physical Therapist, the patient and/or family and facility staff any pertinent information concerning patient progress, problems and plans.
Participate in Patient Plan of Care Meetings and Interdisciplinary Team Meetings if asked by the Physical Therapist.
Participate in in-service training program for other staff in the facility.
Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedure.
Instruct patients' families and/or nursing staff in maintenance programs under the supervision of the Physical Therapist.
Participate in discharge planning under the supervision of the Physical Therapist.
Achieve or exceed the minimum productivity standard established by CRI.
Recommend any needed equipment to the Physical Therapist which would facilitate improved delivery of clinical services.
Report any problems with department equipment to the appropriate personnel.
Act as clinical trainer for affiliating Physical Therapist Assistant programs for area Universities and Colleges.
Comply with the Physical Therapist Assistant Licensure and Certification Standards.
Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.
Consistently portray the mission, vision, core values, cornerstones and professional image of Concept Rehab, exercise good judgment in the performance of the job.
Special projects and other duties as assigned.

REQUIRED QUALIFICATIONS
Associates Degree in Physical Therapist Assistant.
Licensed in the state(s) where services are being provided as a Certified Physical Therapist Assistant.

Job Description: The Physical Therapist provides services on a visiting and intermittent basis in the client's home in accordance with the client's plan of care.
Responsibilities
1. Assesses client's physical therapy needs.
2. Develops and implements plan of care appropriate to the assessment findings and plan for discharge based upon the client's needs.
3. Participate in the periodic revision of the plan of care.
4. Provides physical therapy to clients at home or in community settings.
5. Perform client tests, measurements and evaluation such as range of motion and manual tests, gait, functional analysis and body measurements.
6. Documents all clients care on appropriate and approved form and submits completed documentation to the Director of Clinical and Professional Services within established parameters.
7. Participates in therapy and other in-service programs.
8. Attends rehabilitation department staff meeting and case conferences.
9. Participates in the utilization review and quality improvement activities of the organization.
10. Demonstrate a high degree of commitment to customer service and the provision of quality care
11. Complies with home health agency's policies and procedure manual.
12. Complies with federal, state and local regulatory agencies.
Qualifications:
1. Licensed as Physical Therapy Assistant in the State of Illinois.
2. Graduate of a Physical Therapy Assistant School approved by the American Physical Therapy Association with an Associate Degree in Physical Therapy Assistant.
3. Ability to work well with others and demonstrate good communication skills, both oral and written.
4. Must possess CPR certification.
5. Functional abilities such ability to stoop or bend and able to lift and carry weight to perform all therapy activities effectively.
6. Must own a care with the required insurance coverage and a state driver's license to be able to travel to the client's residence.
