Description
BAYADA Home Health Care is currently seeking an experienced PhysicalTherapistAssistant, PTA, for a per diem opportunity performing home health visits for our Greenville Visits Office. This office services adult and geriatric patients on a per visit basis in territories throughout Greenville, Pickens, Anderson and Spartanburg counties in South Carolina.
As a home health PhysicalTherapistAssistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed PhysicalTherapist.
Each PhysicalTherapistAssistant (PTA) must:
Have current licensure or certification in the state of South Carolina.
Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
Have two years of appropriate experience as an PhysicalTherapistAssistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
Have at least one year's work experience under the supervision of a qualified PhysicalTherapist.

Responsibilities
Provides physical therapy services and interventions under the direction and supervision of a physicaltherapist and in accordance with the physician approved plan of care.
Observes, records and reports the patient's response to treatment and any change in the patient's condition to the supervising PT, physician and other appropriate clinical staff.
Provides resources for future needs and promotes wellness and prevention.
Includes documentation of supervisory activities in the patient's medical record.
Monitors and documents the patient's response to physical therapy intervention.
Participates in patient care conferences, in services, chart audit activities, quality improvement activities including OBQI and OBQM and peer-review activities.
Maintains confidentiality of information relating to the physicaltherapist-client relationship.
Makes recommendations for follow-up care in coordination with the agency that admitted the patient and utilizes other agency care providers to assist with referrals to community resources.
Performs other duties as assigned.

* Current license to practice as a physical therapy assistant, specific to the state you're assigned to work in.
Graduate from an accredited physical therapy assistant educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
One year related physical therapy experience for PTAs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
Current CPR certification.

Maintains communication and collaboration with the evaluating therapist, to promote maximum benefits from therapy. Summarizes patient performance according to established goals.
Implements education plan as outlined by the evaluating therapist, determines additional educational needs of the patient/client, family, caregiver or significant other as they arise and in collaboration with the evaluating therapist provides education to meet those needs.
Follows plan for discharge and discusses with evaluating therapist any additional discharge concerns which may arise. Assiststherapist in goal reassessment and discharge planning.
Communicates/collaborates with patient, family, caregivers, significant other and other members of the healthcare team to promote maximum benefit of care.
Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient/client populations served.
