Position Summary
The COTA Certified Occupational Therapist Assistant provides direct rehab care in occupational therapy to patients upon physician referral under the supervision of a licensed Occupational Therapist (OT) in accordance with all applicable laws, regulations, and Life Care standards.
Education, Experience, and Licensure Requirements
NBCOT certified upon hire, but renewal is optional going forward
Graduate of an accredited program in occupational therapy (ASOT)
Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
One (1) year experience in post acute care or related setting preferred
CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Participates in community health matters/projects as appropriate
Proficient in Microsoft Word, Excel, and e mail
Liaisons with patients, families, support departments, etc., to adequately plan for patient needs
Demonstrate good body mechanics at all times

Implement and interpret the programs, goals, objectives, policies, and procedures of the department

Maintains confidentiality of all proprietary and/or confidential information
Understand and follow company policies including harassment and compliance procedures

What will you do as a Certified Occupational Therapy Assistant at Symbii?
Participate in the ongoing evaluation of patients' functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) as delegated by the OT. Participate in the ongoing evaluation of the home environment for hazards or barriers to more independent living as delegated by the OT.
Participate in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury.
Maintain appropriate clinical records, clinical notes, and reports to the OT any changes in the patient's condition.
Follow treatment program and goals for improved patient function as established by the OT. Document patient response to treatment plan and progress toward established goals.
Maintain contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.
Attend and contribute to in-service, case conference, and other meetings as required to ensure coordinated and comprehensive plans of care for the patients.
Identify patient and family/caregiver needs for other home care services. Consult with the OT and assist with necessary referrals.
