License Requirements
Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
Current CPR certification is required.
Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
The Home Health Physical Therapist Assistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the physical therapy assistant.

Summary:
Provide Physical Therapy and related services for patients under the direction of a licensed Physical Therapist (PT) while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of physical therapy and within Reliant Rehabilitation guidelines. Provide quality patient care including ongoing treatment per a plan of care, supervision of Rehab Techs (where required), and case management responsibilities. Maintain professional conduct that represents Reliant Rehabilitation's commitment to quality and service in all interactions with clients, including positive levels of interaction and rapport with facilities and clients. Adhere to all state and federal regulations regarding physical therapy.
Essential Duties and Responsibilities:
Under the direction of the licensed physical therapist, optimize the functional abilities and skills of patients.
Build a strong physical therapy program.

Responsibilities:
Actively participates in patient care, notifying the supervising Physical Therapist of matters concerning the patient's condition.
Follows the Plan of Care established by the Physical Therapist and practices within state practice guidelines.
Exercises sound judgement, care, professionalism and skill in the performance of duties.
Instructs and counsels patients effectively in exercises and recovery.
Identifies patient's needs and exhibits compassion and caring.
Conveys potential modifications ensuring patient's progress, safety and comfort to the Physical Therapist.
Completes documentation in the EMR to support the delivery of physical therapy services
Under direct supervision of and with co-signature by a PT, records daily notes.
Provides information to patients by answering questions and requests.
Helps treat patients by applying ice or heat packs, helping patients onto exercise equipment, monitoring motion; tracking walking time and distance; performing prescribed exercises and strengthening techniques as requested by PT.
Educates patients by demonstrating proper use of equipment and exercise routines.
Maintains patient confidence and protects operations by keeping information confidential.
Maintains safe and clean working environment by complying with procedures, rules, and regulations.
Enhances physical therapy department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments, is a team player.
Reports any issues needing resolution to Physical Therapists.
