Provides physical therapy services and interventions under the direction and supervision of a physicaltherapist and in accordance with the physician approved plan of care.
Observes, records and reports the patient's response to treatment and any change in the patient's condition to the supervising PT, physician and other appropriate clinical staff.
Provides resources for future needs and promotes wellness and prevention.
Includes documentation of supervisory activities in the patient's medical record.
Monitors and documents the patient's response to physical therapy intervention.
Participates in patient care conferences, in services, chart audit activities, quality improvement activities including OBQI and OBQM and peer-review activities.

Graduate from an accredited physical therapy assistant educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
One year related physical therapy experience for PTAs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
Current CPR certification.

6. Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
7. Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient's clinical record in prescribed time frames.
8. Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.
9. Performs other duties as assigned.
POSITION QUALIFICATIONS:
1. Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.
2. Current certification or license as required by state law.

General Summary: The PhysicalTherapistAssistant (PTA) treats the patient with a specific treatment program designed by the physicaltherapist, under the guidance of the physicaltherapist and physician. 1. Treat the patient in accordance with a specific program designed by the PhysicalTherapist and coordinate this with other team members.
2. Provide education and training to patients and family regarding the therapy plan, home exercise program, home safety, and diagnosis/illness.
3. Take measurements related to patient's condition; reports and coordinates these and any other changes to the supervising physicaltherapist.
4. Maintain a level of professionalism consistent with the code of ethics of the APTA and HHP and report any unethical situations to the PT Board or appropriate personnel.
5. Provide specific documentation of patient treatment and progress to the physicaltherapist and Agency.
6. Participate in face-to-face patient conferences with the PhysicalTherapist as required by state regulations.
7. Participate in mandatory case conferences.
8. Complete appropriate documentation in a timely manner to assure compliance with company policy.
9. Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
10. Meet mandatory continuing education requirements of the Agency/licensing board.
