We are looking for a full-time pediatric PhysicalTherapist or PhysicalTherapist Assistant to float between our NW (88th & Blondo) and SW (138th & Q) centers.
PhysicalTherapist will be responsible for:
Working with pediatric clients with disabilities, injuries, and serious health impairments to restore, maintain, and promote overall gross motor function and health.
Assessing the needs, analyzing risks, and problem solving effectively while using a high level of professional judgment to recommend and provide therapy services to our pediatric clients.
Making environmental modifications and training staff to manage physical needs at an appropriate level of supervision with direct supervision exercised over support personnel.
PhysicalTherapist Assistant will be responsible for:
Working with pediatric clients with disabilities, injuries, and serious health impairments to restore, maintain, and promote overall gross motor function and health under the supervision of a physicaltherapist.
Integrate and collaborate with other disciplines in order to effectively carry out individualized programs, provided by physicaltherapist, based on improving deficits in areas of gross motor function, adaptive equipment, and self-advocacy.
Assess and teach clients who have limited capabilities to develop abilities needed to foster independence and positive interactions in their home and community environments.
A pediatric physicaltherapist assistant works under the supervision of a physicaltherapist to treat children from birth to age 21 at CRCC. A PTA will treat problems such as injuries, pre-existing conditions, and issues caused by illnesses or diseases.
PhysicalTherapist Essential Functions/Responsibilities
Plans, coordinates, directs, and evaluates physical therapy treatments, procedures, and programs from date of evaluation through date of discharge while adhering to professional, ethical, and legal standards of practice.
Reviews client records in order to determine necessary physical therapy services (screen, evaluation, and continued treatment).
Evaluates client to determine intervention strategies and frequency of physical therapy treatment necessary using appropriate tests and measures.
Makes recommendations regarding the use of adaptive equipment, orthotics, assistive devices, and adjunct sessions with trained staff or family members.
Administers physical therapy programs and intervention strategies.
Provides direct supervision to physicaltherapist assistants every 30 days per Nebraska Physical Therapy Practice Act.
Modifies intervention when deemed appropriate.
Collaborates with school therapists in order to provide consistent and effective care.
Trains staff in safe implementation of physical therapy needs in the classroom.
Records all pertinent data in the clients medical records.
Provides and record all relevant paperwork and client information (evaluations, re-evaluations, daily notes, plan of care updates, and discharge summaries) per policies and procedures for the therapy department.
Assists with the recruiting and pre-screening of therapy clients.
Provides proper documentation for ordering necessary therapy equipment, supplies and assessment tools as needed.
Provides weekly schedule to Rehab Services Coordinator by Wednesday at noon each week or enters weekly schedule into the EHR system by the end of the last working day, the week prior.
Respectfully communicates with parents/caregivers and serves as a resource for information relevant to client care, using Tadpoles.
PhysicalTherapist Assistant Essential Functions/Responsibilities
Observing patients before, during and after therapy.
Noting the patients status and reporting it to a physicaltherapist.
Helping patients do specific exercises that the physicaltherapist has put in the plan of care.
Treating patients using a variety of techniques, including, but not limited to, stretching, equipment training, functional mobility, etc.
Helping children to learn to move their bodies correctly.
Assessing patients and monitoring their progress.
Collaborate with school therapists/classroom teachers in order to provide consistent and effective care.
Train staff in safe implementation of physical therapy needs in the classroom, as directed/approved by physicaltherapist.
Record all pertinent data in the clients medical records per policies and procedures for the therapy department.
Respectfully communicate with parents/caregivers and serve as a resource for information relevant to client care.
Attend monthly team meetings and all other meetings necessary for continuity of care for clients.

Minimum Qualifications for PhysicalTherapist
Graduate of an accredited Physical Therapy School with any of the following degrees or combination of these degrees: BS, MS, MPT or DPT.
Current active license in state of Nebraska as a PhysicalTherapist.
Current active BLS/CPR certification required.
Experience with pediatric physical therapies preferred.
Able to meet agencys conditions of employment regarding health status and clearance with the Nebraska Child Abuse/Neglect Central Registry and/or Adult Abuse/Neglect Registry and the Nebraska State Patrol.

What You'll Do
Evaluate and assess patients' physical therapy needs, creating individualized treatment plans.
Provide direct therapy interventions to restore function, reduce pain, and improve mobility.
Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated care.
Educate patients and families on therapy plans, exercises, and home programs.
Document therapy services accurately and timely in accordance with regulatory and company standards.
What We're Looking For
Graduate of an accredited program in Physical Therapy.
Current Nebraska Physical Therapy license (or eligibility to obtain).
Strong communication and interpersonal skills.

Outpatient or geriatric rehab experience preferred, but not required.

1. Evaluates, plans and administers treatment to patients in accordance with the healthcare provider order, and accepted standards of professional practice.
Establishes a system to provide timely and accurate rehabilitation assessment.
Conducts a thorough evaluation according to referral script.
Utilizes evaluation skills to assist in determining an accurate diagnosis and plan of care.
Provides evidence-based treatment services to restore, maintain or improve patient functions including gait training, strengthening, etc.
Develops treatment plan in collaboration with patient, family support systems and members of healthcare team.
Evaluates patient's treatment goals for needed changes according to patient condition.
Performs therapy treatment and modality procedures ethically per APTA and clinic guidelines.
Re-evaluates patient on a regular basis and adjust treatment accordingly using subjective and objective information.
Utilizes space and equipment in cooperation with other staff.
Integrates both manual and non-manual therapy skills in treatment of patients where indicated.
2. Evaluates treatment efficacy and patient progress and makes any necessary adjustments in the care plan to optimize patient care.
Complete treatment plan to meet regulatory guidelines.
Incorporates plans for discharge in treatment plan in collaboration with patient and support system.
Educates family and/or support system to functional needs and equipment/resources to meet these needs.
Utilizes outcome measurements to objectively assess effectiveness of treatment interventions.
Evaluates patient's treatment goals and adjusts plan of care according to patient condition.
3. Performs and maintains documentation.
Documents accurate and ongoing assessments of patient status in a timely manner that reflect physical therapy interventions, patient responses, patient teaching and status of outcomes at discharge.
Completes accurate reporting of billing in a timely manner.
4. Communicates effectively.
Communicates and collaborates effectively with other health team members regarding patient condition, physical therapy plan of care, patient satisfaction needs and recommendations for meeting identified outcomes.
Promotes and maintains positive inter- and intra- departmental communications.
Follows approved shift report format when communicating patient information.
Identifies and recognizes abnormal symptoms/changes in patient condition, established priorities, and takes appropriate action. Appropriately reports condition changes to medical provider.
5. Promotes and ensures patient and staff safety in all aspects of responsibilities.
Administers medications, treatments and procedures in a timely and safe manner according to physician orders and hospital policy, including appropriate performance of standard precautions, using two patient identifiers, and performing patient risk assessments every shift.
Utilizes proper body mechanics, transfer/lifting techniques to minimize fall risk to patient and injury to self.
Uses National Patient Safety Goals (for critical access hospitals) to ensure patient safety.
Implements infectious disease controls as appropriate including isolation procedures, proper use of hand washing and gloves.
6. Performs other duties as assigned.
