Overview:
The Staff Physical Therapist upon the referral of a patient by a Medical Staff Member, evaluates the patients functional abilities, develops functional treatment goals and times frames, establishes the patients rehabilitation potential, and develops a treatment program which will achieve the treatment goals. The incumbent shall asses in an ongoing manner the patients response and effectiveness of treatment, modifying or discontinuing treatment as needed, conferring with the referring physician as necessary. The incumbent shall provide home treatment education and program development, written documentation of initial patient evaluations, progress notes, discharge notes, and other reports as requested. The incumbent shall safety and effectively provide professional patient care services. The incumbent shall address appropriately the age specific needs for all patients. The incumbent will treat patients and families with respect and dignity. Patient's pain will be addressed and treated according to the department's pain management policy. It is the philosop Mercy SEMMC to optimize patient's comfort while they are in the hospital. The incumbent will identify and report any patient safety issue when identified. The Physical Therapist shall provide technical assistance and direction to the Rehab Tech, and coordinates the workload in the department as directed.
Qualifications:
Education: The Physical Therapist shall have a minimum of a BS degree in Physical Therapy.

* Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.
* Test/screen the patient's physical strengths to assist the physician in evaluating the patient's level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.
* Develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
* Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.

Summary:

What you will do:
This role provides physical therapy evaluation and treatment services to children age 0-21 years as prescribed by a physician.
Collaborate with administration and other disciplines involved in the child's care to promote consistent and quality services for children and families.
Evaluate children using appropriate testing protocols, tools, as well as other means, to follow funding guidelines for each particular child.
Create appropriate goals/objectives to be turned in with each evaluation.
Schedule children into caseload by contacting the parent/guardian.
Provide therapy services to children based on the recommended number of units from evaluations and prescriptions, and/or funding source approvals.
Provide therapy services in a variety of settings including home, daycare, schools, outpatient clinic, community, as well as other settings.

Kids Unlimited Learning Academy (KULA) is an early intervention day treatment clinic for children with special needs. Once children are referred to KULA by their primary care physicians, they must receive a developmental screening. Then, they may be scheduled for therapy evaluations at KULA or bring recent therapy evaluation reports done elsewhere. To receive Early Intervention Day Treatment habilitation services, a child must have a documented developmental disability or delay, as shown on the results of an annual comprehensive developmental evaluation and qualify for at least one therapy service. Those services include (but are not limited to) fine motor delays, sensory disorders, gross motor delays, speech-language delays, hearing disorders, feeding disorders, developmental delays, cognitive delays and medical needs.
Children are placed in treatment rooms based on their developmental needs and ages. Our treatment rooms are adequately staffed with teachers and teachers' assistants and are designed to celebrate the cultural differences of the children and families we serve. KULA has many licensed occupational therapists, physical therapists, and speech-language pathologists on staff to provide needed care to the children who qualify for therapy services. And, KULA has nurses on staff to serve children with medical needs.
KULA's services also include developmental/psychological testing by a licensed psychologist, nutritional evaluations and consultations, and audiological evaluations. Our whole treatment team works together and with our patients' families to make sure each child's needs are met.

How you'll contribute
We're seeking a passionate and skilled Physical Therapist (PT) to join our dedicated team of rehabilitation professionals. Perform thorough initial and ongoing assessments to evaluate patient needs and functional status.
Design and implement customized, medically prescribed physical therapy programs to reduce pain, restore physical function, and prevent future injury.
Educate patients on injury prevention, ergonomics, and rehabilitation strategies to support long-term wellness.
Carry out targeted treatment plans using evidence-based techniques appropriate for each patient's age, condition, and capabilities.
Administer therapeutic modalities including moist heat, ultrasound, ultraviolet and infrared light, and other physical agents.
Evaluate and fit prosthetic and orthotic devices, recommending modifications to improve function and comfort.
Supervise and mentor physical therapist assistants, aides, and students-ensuring quality care through task delegation and clear communication.
Set measurable goals and regularly document progress, reevaluating and adjusting care plans as needed.
Minimum Requirements
Degree from an accredited Physical Therapy program
Physical Therapist License in State
Basic Life Support (BLS) obtain within 30 days of hire
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.

As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, you will work to help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
Primary Responsibilities:
* Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care
* Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team
* Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician
* Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
* Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals
* Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy
* Completes all patient evaluations and develops the PT plan of care within state specific guidelines
* Reports outcomes of evaluation, goals, and anticipated projected frequency of care

Required Qualifications:
* Current Physical Therapy licensure in state of practice
* Current CPR certification
* Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation

Lead Therapist

The Lead Therapist is responsible for: * Work a maximum six (6) hours each week per client. * Of these, four (4) hours MUST be in customer's home. The remaining two (2) hours may be worked outside the client's home AS NEEDED * Ensuring the treatment plan is implemented as designed.
* Ensuring proper implementation and effectiveness of the treatment plan.
* Reviewing all data collected by the Line Therapist and parent/guardian.
* Knowledge of evidence-based therapies including but not limited ABA, Discrete Trial, Pivotal Response Training, and Natural Environment Training.
* Should be able to make adjustments to client objectives/targets as needed and have an understanding of prompting procedures, data collection, and error correction procedures.
* Model all therapies/objectives the client is working on and teach the line therapist how to implement all programs.
* Should be able to think on your feet and make suggestions and/or corrections as needed relating to client programs, data collection, and issues and/or concerns.
* Should ensure that the line therapist(s) are generalizing programs throughout multiple settings/environments.
* Should ensure that the line therapist includes maintenance goals within current programming.
* Providing guidance and support to the Line Therapist(s).
* Reporting issues and/or concerns relating to family, client, or team to your supervisor.
* Reporting progress and/or concerns to your Interventionist.
* Receiving parent/guardian feedback, responding to or forwarding concerns to the appropriate person.
Daily Requirements: In the client's home
* Observe the implementation of goals facilitated by the Line Therapist to ensure they follow the ITP.
* Provide guidance and support to the Line Therapist as needed.
* Review data collected by the Line Therapist(s) and parent(s)/guardian(s).
* Communicate with the parent/guardian. Respond to concerns as needed.
* Provide the Interventionist with mastered goals/graphs/ or other data to be updated.
* Model new objectives and/or techniques as necessary.
* Monitor programs to ensure that generalization of skills and maintenance goals are being worked on in multiple settings.
* Inform the Interventionist of any concerns or issues.
Outside the client's home (2 hours/week as needed - based on approval)
* Work related to the client's program, such as visual schedules, token boards, program materials for various skill sets, academic materials, arts/crafts, etc.
* All work done outside the client's home should be approved by the Supervisor and the Interventionist.
