The Role
As an Occupational Therapist (OT), you will be responsible for evaluating residents' functional abilities and designing individualized treatment plans to improve independence in daily activities. You'll provide hands-on therapy, collaborate with interdisciplinary teams, and ensure high-quality rehabilitation services that support our residents' goals.
Key Responsibilities
· Evaluate residents' functional, cognitive, and psychosocial needs using standardized assessments.
· Develop and implement personalized treatment plans to improve daily living skills, mobility, and safety.
· Provide therapeutic interventions including adaptive equipment training, therapeutic exercise, and functional retraining.
· Educate residents, families, and staff on strategies to support rehabilitation goals.
· Document evaluations, progress notes, and discharge summaries accurately and timely in accordance with regulatory and company standards.
· Collaborate with physical therapists, speech-language pathologists, nurses, and other team members to deliver coordinated care.
· Maintain compliance with all applicable state and federal regulations.
Qualifications
· Current licensure as an Occupational Therapist (OT) in the state of practice.
· Graduation from an accredited Occupational Therapy program.
· Previous experience in long-term care, skilled nursing, or rehabilitation preferred.

· Successful completion of State Provider Training
· Demonstrated competency in CPR for children

Job Summary:
The Staff Physical Therapist (P.T.) is an employee of The Physical Therapy Specialty Center (PTSC), a division of Primary Care Partners (PCP). The P.T. is responsible for direct patient care.
Duties/Responsibilities:
Performs comprehensive patient evaluations:
Primary patient care responsibilities will be outpatient physical therapy intervention.
Patients are scheduled for evaluation in such a manner as to not negatively impact other clinical staff yet meet the patient's needs.
Evaluations include medical history, results and interpretation of appropriate functional tests and measurements such as (but not limited to): range of motion, muscle strength, sensory/perceptual deficits, coordination, functional mobility, pain scales, and social needs.
Responsible for providing comprehensive patient care for P.T. patients.
Modifies treatment plan according to patient response to best meet established goals. Documents treatment progression and/or modifications according to departmental/employer standards.
Recognizes adverse reaction to treatment and initiates effective action immediately.
Ensures appropriate patient referrals and discharge.
Administers proper use of modalities
Submits legal and ethical billing/coding which supports treatment and services rendered
Responsible for participating and contributing to the advancement of knowledge and techniques related to P.T. and other related topics.
Identifies, collects and analyzes appropriate patient care information to provide feedback for performance improvement activities.
Maintains required completion of continued education credits as directed by state regulations.
Responsible for maintaining open communication with patients, families, staff and others.
Seeks clarification, listens and conveys information clearly, in written and verbal forms.
Demonstrates the ability to identify and resolve interpersonal or professional conflicts when dealing with patients and members of the organization. Communicates effectively with our patients and families, and recognizes/reports potential cases of patient abuse.
Demonstrates tact, courtesy and a positive approach to communication and interaction with other employees, visitors, physicians, and the public.
Follows the Standards of Ethical Conduct as defined in PT codes of conduct, complies with HIPAA standards.
Supervisory Controls:
Utilize and supervise Physical Therapy Assistants (PTA) within the Physical Therapy Practice Act
Customer Services:
Meets the needs of customers while supporting PTSC/PCP missions. Consistently communicates and treats customers (patients, their representatives, visitors, and all staff) in a courteous, tactful, and respectful manner. Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately.

State licensure in Colorado
Eligibility for insurance credentialing
Further Qualifications Standards:
Education, Experience and Licensure. In addition to meeting the basic requirements, candidates must have 1 year of post degree progressively responsible experience, or a master's degree in physical therapy or a directly related field.
Knowledge of anatomy, physiology, physics, kinesiology, psychology, pathophysiology and surgical procedures as they relate to human function. Knowledge of contemporary and evidence-based PT practice
Knowledge of and ability to apply clinically appropriate assessment techniques and treatment interventions to address human pathophysiology and dysfunction.
Ability to communicate, both orally and in writing, with patients, families, caregivers, and other healthcare professions.
Skill in utilizing appropriate screening and evaluation techniques required to appropriately provide direct patient care.
Knowledge of clinical signs and symptoms to make appropriate referrals for further medical attention/assessment as indicated.
