The Role
As an Occupational Therapist (OT), you will be responsible for evaluating residents' functional abilities and designing individualized treatment plans to improve independence in daily activities. You'll provide hands-on therapy, collaborate with interdisciplinary teams, and ensure high-quality rehabilitation services that support our residents' goals.
Key Responsibilities
· Evaluate residents' functional, cognitive, and psychosocial needs using standardized assessments.
· Develop and implement personalized treatment plans to improve daily living skills, mobility, and safety.
· Provide therapeutic interventions including adaptive equipment training, therapeutic exercise, and functional retraining.
· Educate residents, families, and staff on strategies to support rehabilitation goals.
· Document evaluations, progress notes, and discharge summaries accurately and timely in accordance with regulatory and company standards.
· Collaborate with physicaltherapists, speech-language pathologists, nurses, and other team members to deliver coordinated care.
· Maintain compliance with all applicable state and federal regulations.
Qualifications
· Current licensure as an Occupational Therapist (OT) in the state of practice.
· Graduation from an accredited Occupational Therapy program.
· Previous experience in long-term care, skilled nursing, or rehabilitation preferred.
· Strong communication, documentation, and organizational skills.

The PhysicalTherapist will provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician's orders. To provide leadership and supervision of physical therapy services to other physicaltherapists, assistants and aides. The PhysicalTherapist evaluates and treats clients, communicates with families, physicians and other health team members and maintains appropriate documentation of services and complying with State and Federal regulations. Participates in on-going agency meetings and conducts in-service and training for agency staff on ongoing basis.
QUALIFICATIONS:
Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a PhysicalTherapist in state where practicing.
Able to understand medical terminology and how it applies to physical therapy, to communicate with residents, staff, physicians, and to prepare meaningful reports.
Spatial and form perception are necessary to visualize anatomical structure of the disabled part of the body and the relation to therapeutic devices in remedial actions.
Coordination and dexterity are necessary when treating clients and handling therapeutic equipment.
Ability to communicate with clients to establish favorable attitude and to motivate the clients to desired cooperation and effort in his/her rehabilitation development.
Possesses adequate strength and endurance to meet the physical demands involved in moving clients and equipment into position for treatment and in manipulating equipment and clients.

Be willing to travel and work at other VOANS facilities, as needed.

Re-evaluates the client in conjunction with the attending physician.
Supervises the operation of physical therapy services to ensure an acceptable level of performance from assistants and support staff.
Prepares and maintains treatment records with evaluation and progress notes, including:

Other pertinent information as required by agency's policies.

Participates in client care planning conferences, initial conferences, for residents on physical therapy programs, and when requested by nursing and/or therapy department.
Participates in other agency meetings as directed by Director of Clinical Services.
Communicate with Director of Clinical Services and other health team members regarding client progress, problems and plans for specific clients.

Monitors wait times of both in-house and community services. * Oversees the direct patient care, and effective case management of Physical Therapy patients in all inpatient locations at the Grand Junction VAMC, including Medical Intensive Care Unit, Surgical Intensive Care Unit, Medical, Surgical, Inpatient Mental Health, and Community Living Center Beds.
* Participates as a member of interdisciplinary inpatient care coordination teams. Coordinates outpatient services with the local PM&R department, other VA facilities, Fee Based referral sources for PM&R disciplines or home health rehabilitation services.
* Coordinates PM&R focused in-service training opportunities for PM&R and the facility as a whole.
* Monitors and performs all data/metric gathering assessments on inpatients with appropriate diagnosis.
* Coordinates additional staffing based on inpatient census.
* Coordinates weekend coverage of inpatient services with weekend call therapist.
* Acts as the subject matter expert regarding inpatient Physical Therapy services, and mentors other staff as appropriate.
* Primary patient care responsibilities will be inpatient physical therapy intervention. Ancillary responsibilities may include outpatient physical therapy responsibilities, as directed by the Supervisory of Physical Therapy.
* Patient medical records are reviewed to assess appropriateness of physical therapy intervention, note any potential risks or contraindications to treatment and to assist in guiding the evaluation process.
* Evaluations include medical history, results, and interpretation of appropriate functional tests and measurements such as: range of motion, muscle strength, sensory/perceptual deficits, coordination, functional mobility, pain scales, and social needs.
* Evaluations address rehabilitation potential, behavior, motor skills, and psychosocial development at an age appropriate level.
* Determines patient's rehabilitation candidacy for transfer to an appropriate rehabilitation setting.
* Evaluation findings are used to develop and implement comprehensive patient treatment plans including objective, measurable goals, and timelines for completion.
* Demonstrates a full professional understanding of the theories or treatment of physical therapy and their proper application through the use of therapeutic physical rehabilitation and reconditioning procedures.
* Recognizes adverse reaction to treatment and initiates effective action immediately.
* Ensures appropriate patient referrals and discharge planning in conjunction with the PhysicalTherapist and Medical Center staff.
* Provides Physical Therapy specific wound care and prevention for all inpatient units.
* Identifies, collects and analyzes appropriate patient care information to provide a data base for performance improvement activities within the VA system.
* Investigates current concepts and scientific principles in Physical Therapy practice through literature reviews, study of current research, clinical practice, and through attendance at continuing education courses and conferences.

POSITION SUMMARY
Evaluates and treats patients and communicates with patients, clients, and team members. Maintains documentation of services in the medical record. Conducts in-services and training for patients, clients, and team members on an ongoing basis in restorative techniques. Completes all duties in accordance with established medical practices, governmental requirements and Company policies and procedures.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS
Graduate of an accredited college or university with a valid state license or eligibility to obtain license in state where services are provided.
High school diploma or equivalent required.

PT PT PhysicalTherapist SNF 1+ to 2 years of experience The PhysicalTherapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. S/he identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. S/he implements individualized therapy programs designed to restore, reinforce, and enhance performance. The PhysicalTherapist also instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in
1. Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services. Services such as therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training; functional training; manual therapy; airway clearance techniques; integumentary repair and protection; use of electrotherapeutic, physical agent and mechanical modalities.
2. Supervises Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.
3. Designs, fits and/or applies adaptive devices, and trains patients in the use of adaptive devices including orthotics and prosthetics.
4. Attends and contributes to patient care, staffing conferences and other related meetings.
5. Puts Patient Service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
6. Instructs patient in the skills and techniques required for participation in mobility activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential.
QUALIFICATIONS:
1. Hold a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
2. A Master's degree in Physical Therapy or a related field.
a. For individuals who completed appropriate Physical Therapy education prior to January 1, 2002, with a valid Physical Therapy license in the state(s) of practice or proof of license eligibility, a Bachelor?s degree in Physical Therapy (or related field), plus five (5) years of progressively responsible experience in Physical Therapy will be accepted in lieu of the Masters degree.
b. A foreign-degree equivalent to the first professional degree, consistent with state licensing rules, will be accepted in lieu of Masters degree.
