How you'll contribute
Evaluates and conducts medically prescribed physical therapy treatment programs. Provides educational information to patients in regard to injury prevention, ergonomics, restorative therapy and rehabilitation therapy.
Supervises physical therapy assistants and aides in performing treatment.
Performs initial and ongoing assessments of patient's condition.
Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.

As the PhysicalTherapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, you will work to help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team
Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician
Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy
Completes all patient evaluations and develops the PT plan of care within state specific guidelines
Reports outcomes of evaluation, goals, and anticipated projected frequency of care

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of “direct” and “indirect” patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. As a Home Health PhysicalTherapist , you will:
Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.
Test/screen the patient's physical strengths to assist the physician in evaluating the patient's level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.
Develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.
Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plan that reflects current plan of care as related to therapy, supervise/evaluate staffs' performance. Monitor the appropriate completion of documentation by physical therapy assistants and home health aides/personal care workers as part of the supervisory/leadership responsibility.
Accurately, promptly and thoroughly document patients' care observations, interventions and evaluations. Assure that interim (verbal) orders received from the physician and physicaltherapist are promptly and accurately documented, submitted for physician signature and implemented
Report patient's progress to the patient's physician, Clinical Manager, staff, patient and family. Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff.

By providing direct care, ergonomic education, and recovery strategies, the PT helps mitigate mission-impacting injuries, reduces downtime, and enhances aircrew longevity. This role is essential to ensuring that pilots remain physically capable of meeting the intense demands of aerial combat and mission execution.
JOB DUTIES:
Conduct MSK Evaluations and Interventions.
Design, Implement, and monitor individualized injury prevention and rehabilitation programs.
Order Imaging, Labs, and Specialist consultations as needed.
Documentation of work performed in approved electronic record systems.
Educate Aircrew and leadership on injury prevention, ergonomics, and recovery strategies.
Comply with all DHA, DoD, and MTF protocols.
Other related duties as assigned.
QUALIFICATIONS:
Doctor of Physical Therapy from a CAPTE-accredited program
Minimum 5 years of experience as a PhysicalTherapist in Collegiate, Professional, Olympic, Tactical (military), Aerospace industry, or similar high-performance environment (with experience within the last 2 years)
Current CPR and AED certification from the American Heart Association (AHA) or American Red Cross (ARC)
Must be legally authorized to work in the United States without employer sponsorship, now or in the future
