Education, Experience, and Licensure Requirements
Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)
Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
One (1) year experience in post acute care or related setting preferred
CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Position Overview

Setting: Outpatient with crossover to Skilled Nursing Facility (SNF) and Inpatient Rehab
Position Overview
We are seeking a licensed PhysicalTherapist (PT) for a contract assignment supporting primarily outpatient rehabilitation services, with additional coverage in SNF and inpatient settings as needed. This role requires flexibility, strong clinical skills, and the ability to manage a diverse adult patient population across multiple care environments.
Under general supervision, the PhysicalTherapist performs evaluations, develops and implements treatment plans, and delivers comprehensive therapy services focused on habilitation and rehabilitation.
Key Responsibilities
Perform diagnostic assessments and develop individualized physical therapy treatment plans
Deliver outpatient therapy services with occasional inpatient and SNF coverage
Provide rehabilitation services for adult patients across varying acuity levels
Educate patients and caregivers on therapeutic exercises and recovery plans
Maintain accurate and timely documentation in the electronic medical record
Collaborate with interdisciplinary healthcare teams
Provide coverage across shifts, weekends, and holidays as required by departmental operations
Equipment & Systems
Meditech electronic medical record (EMR)
Required Qualifications
Active HawaiiPhysicalTherapist license required by assignment start
Candidates without an active license must apply upon offer acceptance
Minimum 6 months of experience working with adult patients in at least one of the following settings:
Acute Care
Skilled Nursing Facility (SNF)
Rehabilitation Hospital
Outpatient Therapy
Strong clinical judgment and adaptability across care settings
Certifications
BLS/BCLS required
Schedule Details
Department provides 7-day coverage
Typical schedule includes 5 working days per week
Possible schedules include Sunday-Thursday or Tuesday-Saturday
Orientation & Training
Department and EMR orientation provided
Additional Requirements
Must arrive the Thursday prior to assignment start to complete employee health requirements on Friday
Bilingual skills not required
