● Reviews the quality and appropriateness of individual physical therapy programs for effectiveness and efficiency, using predetermined criteria.
● Adapts treatment to meet the individual needs of the patient.
● Interprets and communicates evaluation findings to patients, families, referring providers, and other team members. .
● Develops appropriate home programs to maintain and enhance the performance of the patient.
● Documents results of patient's assessment, treatment, follow-up, and discharge from services.
● Records billable treatment times according to Clinic procedures.
● Accurately documents patient/family involvement in the medical record as per clinic policy.
● Completes daily/weekly progress notes and patient reassessments with changes noted in functional and physical status, goals and treatment plans.
● Ensures all documentation is completed timely.
● Provides/recommends/fabricates adaptive devices or other equipment.
● Trains patients, families and appropriate staff in the use or application of adaptive equipment and/or devices to ensure optimum safety and outcomes.
● Complies with all Infection Control, Universal Precautions and OSHA standards for the healthcare professional.
● Demonstrates understanding of CMS and other regulatory requirements for the delivery of patient care.
● Manages clinical care to achieve the Quality Index Minimal Productivity Standards (QIMPS).
● Demonstrates knowledge of compliance policies and emergency and disaster procedures of the facility.
● Participates in relevant in-service training sessions.
● Promotes and protects patients' rights (including HIPAA); treats patients with dignity and respect; reports suspected abuse or neglect.
Education and Experience:
● Bachelors, Masters or Doctoral degree in physical therapy as evidenced by college diploma and current state licensure required.
● Current, unencumbered, active license to practice as a PhysicalTherapist in the state employed.

Requirements:
ESSENTIAL FUNCTIONS OF THE JOB
Evaluate Patients for rehabilitation services within 24 to 48 hours of physician's orders.
Develop effective treatment plans.
Treat patients according to the treatment plan of care.
Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.
Communicate with physicians, clinical managers, and other health team members, patients, family and facility staff regarding patient progress, problems and plans.
Participate in Plan of Care Meetings upon request and Interdisciplinary Team Meetings.
Achieve or exceed the minimum productivity standard established by CRI.
Participate in in-service training programs for other staff in the facility.
Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
Instruct patient's families and/or facility staff in maintenance programs.
Participate in discharge planning.
Recommend any needed equipment to the appropriate personnel which would serve to facilitate improved delivery of clinical service.
Report any problems with department equipment to the facility personnel.
Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.
Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.
Consistently portray the mission, vision, core values, cornerstones and professional image of Concept Rehab, exercise good judgment in the performance of the job.
Special projects and other duties as assigned.
