Responsibilities
Our PhysicalTherapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. PhysicalTherapist Lakewood CO (10 miles SW of Denver) 4.5-day work week We are looking for a motivated PhysicalTherapist for our growing, multi-disciplinary pain management and orthopedic practice in Lakewood CO. This position is 100% outpatient care, providing exemplary patient care for established and new patients. Must be an excellent team player and work well with other providers in our team-based treatment approach. PT duties include conducting patient evaluations, treating a variety of musculoskeletal issues, ordering, and interpreting diagnostic tests, and communicating well with other providers to assist in the best care for patients. The ideal candidate will have experience in orthopedics, but recent graduates will be considered! A bonus would be someone trained in or willing to be trained in pelvic floor physical therapy.

Duties:

Requirements:
Graduation from an accredited Physical therapy Program (DPT preferred)
National PT Board Certification
Experience working in out-patient, orthopedic care preferred but will consider recent grads
PT license in CO
Pelvic floor PT experience or certification a huge plus (or those interested in training)
Schedule:
Monday - Friday (office hours end at 1pm on Fridays)
Full time- no weekends
Compensation (range):
$85,000 - $110,000 plus bonus compensation

The Staff PhysicalTherapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patient's physical abilities analyzing rehabilitation goals; consult with rehab team in order to coordinate the most effective physical therapy program.
Organizational Relationships:
Supervises: Responsible for the care delivered by PTAs, Rehab Technician.
Essential Job Responsibilities:
- Evaluate patients/residents within 24 hrs of physician referral.
- Develop effective treatment plans and obtain approval for services from referring physician, and treat patients/residents according to physician's treatment plan.
- Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff on the patient's treatment plan, progress, and prognosis.
- Record evaluations, daily treatment notes, weekly progress notes, 14 day progress report to the physician, and discharge summaries per policy and procedure.
- Instruct patient/resident's families or nursing staff in maintenance program.
- Participate in discharge planning.
- Secure necessary durable medical equipment for patients/residents to facilitate independence in mobility: i.e. orthoses, walkers, canes, elevated toilet seats, and grab bars.
- Submits logs and all billing information on a timely basis.
- Reports any problem areas/equipment within each facility to the appropriate supervisor immediately.
- Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines.
May perform other duties as assigned.
