The Physical Therapist I (Entry/competent), Outpatient is responsible for the assessment and treatment of functional impairments including orthopedic injuries, neurologic conditions, oncology/lymphedema, and women's/men's health. The goal is to maintain or restore the patients to their maximum functional performance. - Individually responsible for differential diagnosis and supervision of patient's care under direct access law without physician oversight. - Evaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelines. - Administers evidence- based physical therapy treatments appropriate to diagnosis and to APTA guidelines. - Documents and charges for patient care services in accordance with regulatory, licensing, payer, and accrediting requirements. - Completes progress notes/recertification to demonstrate the need for continues skilled services as outlined by federal, state, and insurance regulations. - Completes discharge planning according to APTA guidelines by consulting with physicians, OT, SLP and with the patient/family. - Completes peer to peer insurance review and other required forms for obtaining authorization for services. - Meets productivity/KPI targets. Competent: - Coordinates external resources and support for patients and families. - Completion of yearly mandatory education for employment and VA licensure. - Other duties as assigned. - Participates in at least one community service activity that is Centra sponsored or related to the PT profession. - Serves as a Clinical Instructor for a physical therapy student clinical 1-2, or an intern (greater than 30 hours) applying to PT school. - Provides one in-service annually to staff. - Completes two of the following: - Participates as a member of the APTA with at least 50% attendance. - Participates in efforts to impact/affect legislation in professional area. - Performs clinical activity or special project as pre-approved by manager. - Supervises Licensed Physical Therapy Assistants, rehab assistants, and other support staff. Required Education: Graduate of an accredited PT program (Bachelor's, Master's, or Doctorate Program) Preferred Education: Masters Degree in Physical Therapy or Doctoral Degree in Physical Therapy Preferred Experience: At least 1 year clinical experience in outpatient setting but will consider new graduates. Required Certifications and Licensures: Current Virginia PT licensure or PT Compact license privileges.

Job Description We are hiring a PRN Physical Therapist for our outpatient clinic. The assistant will implement physical therapy programs and activities, follow developed recovery plans, and deliver therapy treatments. The assistant is equally adept at addressing the whole patient and seeing them as individuals. The assistant understands that the goal is to execute a program that restores, reinforces, and enhances the patient's physical abilities. Responsibilities: Provides patient care by established protocols and patient care guidelines. Responsible for timely documentation, including daily notes, weekly workers' compensation reports, progress reports, and incident reports. Provide comprehensive patient care, administer active and passive manual therapeutic exercises, therapeutic massage, heat, light, sound, water, and electrical modality treatment such as ultrasound, electrical stimulation, ultraviolet, infrared, and hot and cold packs. Administers traction to relieve neck and back pain. Instruct, motivate and assist patients in learning and improving functional activities such as perambulation, transfer, ambulation, and daily living activities. Observe patients during treatments, compile data on patients' responses to treatments and progress, and report orally or in writing to the Physical Therapist and/or Clinical Director. Fits patients for, adjusts, and trains patients in the use and care of orthopedic braces, prostheses, and supportive devices such as crutches, canes, walkers, and wheelchairs. Confers with physical therapy staff members and other health team members, individually and in conference, to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating treatment programs. Measures the patient's range of joint motion, length and girth of body parts, and vital signs to determine the effects of specific treatments or to assist the Physical Therapist in compiling data for patient evaluations. Qualifications Graduate from CAPTE-accredited Physical Therapy program. Current state of Virginia Physical Therapy license and CPR certification. Able to create and implement outpatient balance and vestibular rehabilitation treatment plans. Ability to work efficiently/effectively in an autonomous environment. Possesses basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) systems, Database software, and MS Office. Tests patient's physical abilities analyzing rehabilitation goals; consult with rehab team in order to coordinate the most effective physical therapy program. Organizational Relationships: Supervises: Responsible for the care delivered by PTAs, Rehab Technician. Essential Job Responsibilities: - Evaluate patients/residents within 24 hrs of physician referral. - Develop effective treatment plans and obtain approval for services from referring physician, and treat patients/residents according to physician's treatment plan. - Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff on the patient's treatment plan, progress, and prognosis. - Record evaluations, daily treatment notes, weekly progress notes, 14 day progress report to the physician, and discharge summaries per policy and procedure. - Instruct patient/resident's families or nursing staff in maintenance program. - Participate in discharge planning. - Secure necessary durable medical equipment for patients/residents to facilitate independence in mobility: i.e. orthoses, walkers, canes, elevated toilet seats, and grab bars. - Submits logs and all billing information on a timely basis. - Reports any problem areas/equipment within each facility to the appropriate supervisor immediately. - Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines. May perform other duties as assigned.
