Responsibilities:
You'll achieve success by:
Treating patients in a one on one private treatment room forminutes
Reviewing patients medical history
Diagnosing patients by observing their movements and listening to their concerns
Developing individualized treatment plans for patients with clear goals and expected outcomes
Using exercises, stretching, equipment, and hands-on manual therapy to manage patients' pain, increase mobility, and prevent further pain and injury
Recording patient progress and modifying the plan of care as needed
Qualifications:

The Role

Responsibilities
Our PhysicalTherapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. The Role
As an Occupational Therapist (OT), you will be responsible for evaluating residents' functional abilities and designing individualized treatment plans to improve independence in daily activities. You'll provide hands-on therapy, collaborate with interdisciplinary teams, and ensure high-quality rehabilitation services that support our residents' goals.
Key Responsibilities
· Evaluate residents' functional, cognitive, and psychosocial needs using standardized assessments.
· Develop and implement personalized treatment plans to improve daily living skills, mobility, and safety.
· Provide therapeutic interventions including adaptive equipment training, therapeutic exercise, and functional retraining.
· Educate residents, families, and staff on strategies to support rehabilitation goals.
· Document evaluations, progress notes, and discharge summaries accurately and timely in accordance with regulatory and company standards.
· Collaborate with physicaltherapists, speech-language pathologists, nurses, and other team members to deliver coordinated care.
· Maintain compliance with all applicable state and federal regulations.
Qualifications
· Current licensure as an Occupational Therapist (OT) in the state of practice.
· Graduation from an accredited Occupational Therapy program.
· Previous experience in long-term care, skilled nursing, or rehabilitation preferred.
· Strong communication, documentation, and organizational skills.

Responsibilities
Our PhysicalTherapists (PTs) examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. Our PhysicalTherapists (PTs) use our Specialty Programs, including Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program to treat our patients.
Qualifications
PhysicalTherapists (PTs) must meet the following requirements:
