As an Occupational Therapist, you will provide direct therapy to students in Grades K-12 with special needs including but not limited to development, implementation and maintenance of designated instructional programs, conducting regular assessments, and developing or adjusting individualized education plan (IEP) goals as needed.
‖ Responsibilities Include:
Implementing student-specific occupational therapy services as outlined in each Individualized Education Program (IEP), including adherence to frequency, duration, and service location requirements.
Completing comprehensive assessment reports within state- and district-mandated timelines to support data-driven educational planning.
Communicating evaluation results and individualized therapy recommendations to families, educational staff, students (as appropriate), and related agencies, with appropriate consent from the student's legal guardian.
Planning and developing targeted therapeutic goals that are educationally relevant and seamlessly integrated into each student's academic program.
Conducting ongoing reassessments and modifying therapy objectives based on evolving student needs and progress toward educational outcomes.
Monitoring the fidelity of therapy implementation and collaborating with and training educators, paraprofessionals, and other support staff involved in occupational therapy interventions.
Documenting student progress through timely and accurate reporting in alignment with district standards and program guidelines.
Participating in IEP meetings to share data-informed insights, progress updates, and goal recommendations to support individualized planning.
Creating a supportive, safe, and developmentally appropriate learning environment tailored to each student's sensory, physical, and cognitive needs.
Evaluating progress toward therapy goals and determining when students are ready to transition from or discontinue occupational therapy services.
Facilitating professional development sessions and in-service trainings to enhance the capacity of occupational therapy staff and interdisciplinary team members.
Participating in training school personnel on the effective use of assistive technology and adaptive equipment, while maintaining proper documentation of use and outcomes.
Attending and actively engaging in all required team meetings, trainings, and professional collaboration forums.
Performing additional duties as assigned, contributing positively to the overall success of the student services team.
‖ Qualifications Required:
Master's degree or higher in occupational therapy or a closely related field of study.
Licensed currently or in the process of obtaining a state occupational therapy (OT) credential.
Ability to obtain and maintain certification in company approved crisis management (PCM) training.
Prior experience and/or highly knowledgeable in providing occupational therapy services, preferably in an educational or classroom setting.
Prior experience and/or knowledge in special education services and compliance, particularly for students with severe learning disabilities, autism spectrum disorders, emotional behavioral disorders and/or other related disabilities.
Prior experience and/or knowledge in working with students with individualized education plans (IEP's).
Proven success in developing and implementing effective treatment plans for students with diverse needs.
Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
Ability to think and act quickly and calmly in an emergency and make independent decisions.
Ability to effectively present information and respond to questions from groups of staff members, students, parents and the public.
Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability.
Well-versed in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment.

1. Accepts responsibility for non-patient care duties, e.g. task forces, in-service new staff, organization, which aids in the delivery of care of the department.
2. Administers comprehensive PT evaluations, using tests and measurements appropriate for the patient's diagnosis, age, and disabilities. Completes in a timely manner.
3. Arranges schedules to accommodate patient care when a team member is absent. Coordinates scheduling of patients as appropriate to the service. Assists with basic operational oversight when department manager is absent.
4. Attends, participates, and contributes to in-service educational training.
5. Communicates with patient and family. Educates patient, family, other caregivers in self-care, prevention and wellness, home exercise programs, use of equipment, and safety.
6. Completes required administrative forms for leave of absences, payroll, and statistics. Uses time clock appropriately. Completes annual competencies, health tests, and surveys in a timely manner.
7. Confers individually and/or in team meeting with physicians, nurses, social workers, and other health care team members about patient status and/or discharge planning.
8. Confers with other PT's, assistants, aides, clerical staff and department leader as appropriate for problem solving and program planning.
9. Develops and implements a treatment plan using various physical therapy procedures, equipment, modalities, and techniques to produce positive changes in patient outcomes. Orders, recommends, or obtains referrals for necessary assistive devices, bracing, prosthetics, or continuation of therapy in another setting.
10. Documentation: Writes clear, concise, legible, and objective information on patient evaluations, progress notes, discharge notes, equipment orders, and insurance forms and records accurate daily treatment attendance and charges. Utilizes electronic health record efficiently for information gathering. Completes documentation in a timely manner.
11. Orients and supervises new staff members and students. Proofreads notes and supervises patient care until assured of their competence. Helps evaluate staff PTs, PT Assistants, and PT Aides. Evaluates students fairly and accurately with the approved clinical evaluation tools.
12. Participates in gathering data for Continuing Quality Assurance, Performance Improvement, staff surveys, etc.
13. Uses appropriate safety measures - body mechanics, infection control, environmental hazard control; cleans treatment area after use.
MINIMUM QUALIFICATIONS:
Education: Graduation from an approved school of Physical Therapy.
Minimum Experience: One year of experience as a Physical Therapist as employed by Alameda Health System; OR three to five years or more of recent experience in another clinical setting related to the area of proposed supervision (acute medical, rehab, SNF, or outpatient).
Required Licenses/Certifications: Current Licensure with the California Board of Physical Therapy as a Physical Therapist.
Required Licenses/Certifications: BLS - Basic Life Support Certification - issued by AHA-American Heart Association.
Pay: $66.48 / Hour
The pay range for this position reflects the base pay scale for the role at Alameda Health System. Schedule: Jet Physio believes in a happy, well rested staff. Part of this is having flexibility in your schedule. Essential Functions:
⦁ Has good knowledge of Agency Policies and Procedures, State and Federal Regulations related to Home Health/Hospice services, Clinical Procedures/Protocols, PT Job Description and assigned responsibilities, and acts upon all listed above requirements.
⦁ Assumes responsibility for care of all assigned patients as Physical Therapist and coordinates patient care services with other disciplines of Healthcare team, including RN, OT, ST and MSW, as well as patient's PCP, DPCS and QA RN.
⦁ Assesses and evaluates therapeutic/rehabilitative/functional status of the patient. Assesses patient's muscle strength, mobility, gait, ROM, transfer capabilities and rehab potential.
⦁ Evaluates home environment for safety and educates patient/family/caregivers on Fall precautions/Safety measures.
⦁ Documents assessment findings in Evaluation Visit Form, and submits it in DeVero within 24 hours from the date of evaluation. Makes sure that all fields of evaluation are completed thoroughly and accurately according to the assessed/factual data.

Clinical Care Responsibilities
Provide comprehensive physical therapy, within the scope of credentialed privileges, to Lyon-Martin clients, documenting care promptly and following charting protocols and applicable CA state licensing requirements.
Possesses strong clinical knowledge of and commitment to healthcare for trans, non-binary gender non-conforming, and intersex people, LGBQA+ communities, and cis-gender women.
Develops a continuity panel of clients.
Completes patient sessions in a timely manner.

Work with leadership team to envision and expand Chronic Pain Management and Integrative Care program at the clinic overall.
Work collaboratively with mental health and medical staff to facilitate integration and to improve patient outcomes.
Provides de-escalation to clients who may become triggered during session and link them to care.
Fosters safety, consistency, and connection with clients.
Document visit notes and client interaction completely in an accurate, detailed, and timely manner (within 72 hours, ideally within 24 hours), responses to in basket messages in a timely manner (within 72 hours, ideally within 24 hours), following charting protocols and applicable CA state licensing requirements.
Follows through on commitments with clients and colleagues.

Qualifications
Required Skills, Education and Certifications:

* 2 years of therapy experience strongly preferred
* Must be able to work independently with little to no supervision or training
* Previous home care / private duty experience
* Must be reliable, dependable, and on time
* Basic computer skills as well as overall technical savviness
* Proof of eligibility to work in the U.S.
