The Physical Therapist collaborates with the Occupational Therapist to efficiently operate West PACE rehabilitation department and, also, works effectively with colleagues and stakeholders to promote the West PACE values, team culture and mission.
Essential Functions:
Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs(FMP).
Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
Order, track and manage maintenance for equipment.
Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status.
Work with the other members of the IDT to support smooth care transitions between settings (e.g. hospitals, skilled nursing facilities, home,)
In coordination with the Marketing Team, support enrollment of prospective participants into the program.
Participate in end-of-life care coordination and support.
Complete all required documentation in a timely and accurate manner.
Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
Follow West PACE policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.

Education and Certification:
Valid Physical Therapy license in the state of California, or proof of license.
