Job Summary
Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients/residents.
May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Will be interacting with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. Will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership. Performs other related duties as requested by manager.
Based on facility needs and assigned locations, may focus on those receiving intervention in the post-acute, long term care and home health care environments.
Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.
When applicable, may require valid driver's license.
Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

The Department of Cardiovascular Diseases at Mayo Clinic in Rochester, MN, is seeking a Vascular Medicine Specialist to join our team. The Department of Cardiovascular Diseases is comprised of over 130 specialists of which 13 participate in the Division of Vascular Cardiology. The division is based in the Gonda Vascular Center, a multidisciplinary practice involving 49 physicians from disciplines including vascular surgery, vascular medicine, vascular radiology, cardiology, hematology, physical medicine, rheumatology, and general internal medicine. Within this space, the Vascular Medicine practice is comprised of multiple clinics and laboratories (vascular medicine, thrombophilia, the non-invasive vascular physiologic laboratory, vascular ultrasound, vein clinic, wound care clinic, vasculitis clinic, and vascular medicine hospital consult service). There is an active educational program including teaching learners at all stages of education including the Mayo Medical School (with 160 students), Internal Medicine residency program (residents participate in elective 1 month rotations in Vascular Medicine), Mayo School of Graduate Medical Education (Vascular Medicine and Cardiology), and the Mayo School of Continuous Professional Development. Additionally, several members of the section have active translational and epidemiologic research programs. This is a full-time position with a significant allocation to the clinical practice. This would include participation in the various Vascular Center clinics as well as patient care assignments to the Vascular Medicine hospital consultative practice. The position carries an academic appointment to the Mayo Clinic College of Medicine.
Qualifications
License/certification: Candidates must hold an MD or equivalent degree and be board-certified in internal medicine and BC/BE in vascular medicine. Board certification in Cardiology is an advantage.

Role of a Physical Therapist:
Physical Therapists (PT) are health care professionals who diagnose and treat individuals of all ages, from newborns to the very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.

The Physical Therapist reports directly to the Clinical Supervisor.
SCHEDULE
This is a full time or part time position with a flexible schedule during office hours Monday - Friday.
This position will require you to travel to client's homes to provide 1:1 client care that may take 1-3 hours per client.

This position reports to the Mankato, MN office and provides services in Owatonna, Waseca, Faribault, and surrounding cities.

MAJOR RESPONSIBILITIES
Administers physical therapy evaluations. Participates in the development of the Plan of Care.
Provides physical therapy treatments, procedures, evaluations and diagnostic tests for clients under the direction of their physician for whom therapy has been medically prescribed.
Translates all exercises into functional activities or activities of daily living.
Provides instructions in the use and care of assistive devices.
Develops needed plans for modifying equipment, appliances and the physical surroundings in client's homes.
Prepares clinical and progress notes. Reports to physician and the Case Manager the client's response to treatment or change in condition.
Maintains appropriate records including frequency of visits and client's response.
Instructs and supervises PTAs, and HHAs when appropriate.
Provides in-service education programs for company staff as requested.
Participates in Case Conferences as indicated.
Provides families with information, support and encouragement to help motivate clients in their progress. Instructs client's family on home physical therapy program.
Assists individuals and families to accept and adjust positively to physical, mental and social limitations.
Participates in the development of community resources to meet the needs of clients.
Maintains positive and effective communication with all staff and others. Understands and participates in team concept.
Maintains absolute confidentiality of all information pertaining to clients, families and employees.
Maintains a safe client environment and identifies and reports to the office any suspected vulnerable client abuse, neglect, or financial exploitation.
Performs other related duties and responsibilities as assigned.
PHYSICAL/ENVIRONMENTAL DEMANDS
Heavy to Very Heavy. Involves lifting clients, bending, stooping, stretching, and setting up equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.
Exerting in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, and in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for heavy work.
QUALIFICATIONS
Graduate of an accredited Physical Therapy program; currently licensed as a Physical Therapist in the state of Minnesota. The license must not have been revoked, suspended, and without limitations or restrictions.
Have at least two (2) years experience in a health care setting or equivalent experience. Home Health Care experience preferred.
Strong interpersonal communication and teaching skills.
Strong organizational skills and the ability to work independently.
Excellent written and oral communication skills.
Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.
A valid driver's license and proof of car insurance when using a personal vehicle for company business.
Proof of negative mantoux or documentation of negative chest x-ray.
Current CPR certification is highly recommended.
Have U.S. Citizenship or evidence of valid Alien Work Permit.

* Assumes Positions to complete the patient/resident treatments which may require holding a position or a patient in a position for period of time (5 minutes duration)
* Reaching horizontally for patient care, treatment or assessment at 6-10" frequently and occasionally up to 22-24" to reach across the bed. May over-reach to retrieve blood pressure cuff while pulling patient curtain
* May assume squatting or stooping position for emptying a Foley catheter or donning shoes and socks or adjusting the wheelchair foot rests.
* Bending duration of 7-56 seconds, depending on the cares. Frequent bending throughout the shift.
* Transport patients/resident in beds, carts, wheelchairs. Vertical handle height at 36". Firm grasping required. Push/pull force for beds=35-45#, for Wheelchairs: 15-50#. Tipping or pushing down on the patient scale to move it = 35# force

Required Work Schedule:
General working hours range within 7:00a.m. and 5:00p.m. Monday through Thursday and 7:00a.m. and 4:30p.m. Friday depending on assigned shift and departmental needs. Weekends on a rotational basis in inpatient acute care setting.

Fusion Medical Staffing is seeking a skilled Physical Therapist for a 13-week travel assignment in Rochester, Minnesota. Required Qualifications:
Valid Physical Therapy license in compliance with state regulations
Current BLS certification (AHA/ARC)
Preferred Qualifications:
Doctorate degree in physical therapy
Physical Therapy experience, but New Grads are welcome to apply
Other certifications or licenses may be required for this position
Summary:
The Physical Therapist evaluates, diagnoses, and treats patients with physical impairments to improve mobility, reduce pain, and enhance quality of life. Working in settings such as hospitals, clinics, and rehabilitation centers, they create personalized, evidence-based treatment plans using exercise and manual therapy to help patients recover and become independent. This role requires strong clinical skills, effective communication, critical thinking, and a commitment to high-quality, safe patient care.
Essential Work Functions:
Evaluate patients' physical conditions using standardized tests, clinical examinations, and patient history to identify functional limitations and treatment need
Develop personalized rehabilitation programs based on assessment findings and patient goals
Implement physical therapy treatment plans using various techniques to improve patient mobility and reduce pain
Operate specialized physical therapy equipment and modalities safely and effectively, ensuring proper maintenance and calibration of devices
Assess patient progress, adjust treatment plans as necessary, and accurately document evaluations, interventions, and outcomes in the medical record
Work closely with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care and optimize treatment outcomes
Educate patients and caregivers on home exercise programs, proper body mechanics, injury prevention strategies, and self-management techniques to promote long-term functional recovery
Maintain a safe treatment environment by adhering to infection control protocols, safety guidelines, and regulatory standards while addressing patients' comfort needs during therapy sessions
Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings
Perform other duties as assigned within scope of practice
Required Essential Skills:

The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills

**Responsibilities**
Provides athletic training services in the interscholastic, intercollegiate, elite amateur, professional and recreational setting including event coverage. Participates in outreach activities as a representative of Mayo Foundation for Medical Education and Research. Delivers treatment to patients under the supervision of the physician in accordance with quality standards of the department. Assists in duties in physician clinic as directed by provider. Functions independently in the clinical setting in the Mayo Clinic Sports Medicine Center, and participates in other Mayo outreach activities, as directed. Provides one-on-one clinic care as needed, under the supervision of a physician, and in accordance with the quality standards of the department including (taking a history, performing an initial evaluation, proxy orders, fitting of braces and splints per protocols, providing therapy per protocols and assisting with outpatient procedures such as injections). Individuals must have the knowledge base to be able to evaluate and report findings to the physician. Delivers treatment and care to patients under the supervision of the physician in accordance with quality standards of the department. Further the role of the clinical athletic trainer at the Sports Medicine Center by continuing to improve the efficiency of the physicians they are working with. The individual must be able to be involved with the formation of new programs and materials focused on wellness, injury prevention, and fitness for physically active patients as time allows.
**Qualifications**
A Master's degree in athletic training or a related medical field and certification by the Board of Certification and National Athletic Trainer's Association, along with Minnesota Athletic Trainer's license is required. CPR certification is required. Must be able to demonstrate clinical proficiency with good manual dexterity in order to manipulate treatment equipment. Must have strong communication and human relation skills to interact with clinicians, administrators, students, coaches, parents, etc. Require at least two years experience working as a clinical athletic trainer in a clinical setting with patients in a one-on-one situation.
**Additional Qualifications**

Provides athletic training services in the interscholastic, intercollegiate, elite amateur, professional and recreational setting including event coverage. Provides athletic training services to athletes to include evaluation, treatment, management of injuries,, and injury prevention strategies. Works independently to provide emergent care of injuries and make decisions for proper medical referral as indicated. Communicates and collaborates with patient, physician, physical therapist and other team members regarding patient progress, response to treatment, goals, and program changes.
