Responsibilities
Our PhysicalTherapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. The Role
We are seeking compassionate and skilled PhysicalTherapists to join our team- PRN to full time opportunities available. You'll work with patients in the Longmont area, helping them regain mobility and improve their quality of life. This role is ideal for a therapist who enjoys building strong patient relationships and providing personalized, hands-on care.
What You'll Be Doing
Conducting thorough assessments and ongoing evaluations of patients physical conditions and functional abilities using OASIS
Developing and implementing individualized treatment plans tailored to each patients needs
Providing therapeutic interventions, including manual therapy, exercises, and education on injury prevention
Monitoring patient progress and making adjustments to care plans as needed
Building supportive relationships with patients and their families, helping them through each step of their recovery journey
Collaborating with the care team to ensure coordinated, comprehensive care
What Makes You a Great Fit
Degree in Physical Therapy from an accredited program
Valid ColoradoPhysicalTherapist license
Valid BLS Certification
Experience in home health settings preferred
Exceptional communication skills and a patient-centered approach to care
Licensed driver with current auto insurance and good driving record
