Arnot Health is looking for a Physical Therapist experienced in Pediatrics. This position is full-time, day shift. Main Function Plans and implements specific treatment programs for individual patients according to the principles and practices of Physical Therapy. Collaborates with interdisciplinary treatment team. The Staff Physical Therapist is accountable to the Clinical Coordinator of Rehab Services.
Duties And Responsibilities
1. Reviews requisitions for Physical Therapy.
Interviews patients, evaluates patient's physical condition and needs.
2. Develops a program of corrective exercise and treatment for assigned patients.
Determines proper equipment usage, application and body position, adjusting as necessary in the administration of such treatments as exercise, gait training, soft tissue mobility, whirlpool, hot packs, ultrasound, paraffin, ice packs, electrical stimulation, and traction.
Instructs patients on segments of the program including proper use of wheelchairs, crutches, canes, braces, and prosthetic appliances and devices.
Recommends modification or changes in the patient's program based on own evaluation of progress.
Confers as necessary with physicians concerning alternatives available.
3. Prepares written documentation as required by the profession and the department, such as evaluation results, treatment plans, progress reports and discharge summaries.
4. Communicates observations, facts and comments with nursing and interdisciplinary team members to ensure efficient coordination of the overall health care plan.
5. Provides guidance and direction to those staff members assigned to assist in patient care.
6. Maintains department policies, procedures, objectives, and quality initiative program, safety and infection control.
Adheres to hospital wide safety responsibilities:
Maintains a safe environment for patients, visitors and other employees.
Maintains a working knowledge of all hospital safety policies and procedures (including fire safety and disaster protocol).
Follows all safety practices on equipment.
Uses all safety devices on equipment and insures it is in proper working condition.
Wears safety clothing, glasses, gloves, shoes, etc. when required and respects “Universal Precautions” as indicated.
Reports any unsafe act or condition to his/her supervisor.
Attends safety programs when they are conducted.
Is familiar with and adheres to “Policy and Procedure” manual for the department.
Assists where indicated in assisting the Clinical Coordinator with quality initiative indicators.
7. Assists in care and maintenance of department equipment and supplies.
8. Attends meetings as required and participate on committees as directed.
9. Assists in transport of patients to and from PT Department as the need should arise.
10. Assumes the duties and responsibilities of the Senior Therapists in his/her absence.
11. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, workshops, etc.
Is responsible for attending all annual mandatory educational programs as required by position.
Provides age appropriate care to neonate, infant, children, adolescent, adult and geriatric patients.
Employee understands and demonstrates the importance of satisfying the needs of the customer/patient by interacting with him/her in a friendly and caring way, being attentive to the customer's needs, both psychologically and physically, and by taking the initiative to maintain communication with the customer in order to provide a secure and pleasant experience with the Arnot Health.

Education
EDUCATION:
Must be licensed, or eligible to obtain a license, to practice physical therapy in NYS at the time of hire.
Experience
Experience in a hospital setting preferred.
Cardiopulmonary Resuscitation (cpr) Requirements
Basic Life Support (BLS) required within 90 days of hire date.
Physical Demands
Appreciable physical effort or strain; occasionally repetitive efforts. May involve lifting, pushing, pulling. Must be able to lift at least thirty (30) pounds independently and at least one-hundred and fifty (150) pounds with assistance of another person.
Exposure Category
Category II. Tasks that involve no exposure to blood, body fluids, or tissues. Employment, however, may require performing unplanned Category I tasks.

Physical Therapist:

Position Summary: The certified athletic trainer is a licensed allied healthcare professional whose primary concern is the optimal health of student athletes and physically active individuals. Under the direction of a physician, they specialize in injury prevention, injury evaluation and management, emergency situations, rehabilitation, and returning the athlete to play safely and quickly. The certified athletic trainer will be assigned to provide outreach athletic training services to high school & Jr high school athletics at contracted schools.
Education, License & Cert: The incumbent shall, at a minimum, be a Graduate of a CAATE accredited bachelor's or master's degree program, and BOC certified in good standing. Current licensure in the state of primary practice is required. Current dual licensure in New York and Pennsylvania is preferred. Current BLS Certification for the professional mandated.
Experience: Education/experience in working with adolescent athletic population preferred
Essential Functions: Communicates and demonstrate a professional image/attitude for patients, families, clients, athletic directors, coaches, coworkers, and others. Provides high quality care to athletes/patients to include prevention, immediate care, assessment, rehabilitation, referral and communication with providers. Including monitoring athletic activities and events for potential safety hazards or unsafe practices and notify the appropriate supervisor. Making appropriate recommendations to athletic directors or coaches pertaining to protective equipment. Working with coaches to develop and implement effective and appropriate training and conditioning programs for student athletes.
Provide initial assessment and care for emergent, acute and chronic sports related injuries and illnesses and make effective & timely determination if a higher level of care is appropriate. Communicate with appropriate school officials and parents/guardians of athletes regarding injuries/illness, care provided or recommendations for care while working closely and cooperatively with Medical Director/physician(s)/provider(s) to treat and rehabilitate injured student athletes. Develop individualized treatment plans as needed and appropriate interventions under the supervision of the Medical Director or athlete's physician/provider. May provide onsite rehabilitation program at school or recommendation of therapy referral to outpatient facility.
Documentation in athlete Electronic Medical Record to include health status, injury, illness, immediate care, treatment, rehabilitation and/or encounters. Document in Presagia EMR software the injury, treatment and encounter status of all student athletes for both home and away teams. Document at the beginning of each sports season all athletes' demographic information, team/sport participation and grade. All Injuries to athletes must be documented in Presagia within 24 hours. New injuries or illnesses should be created in the “Unresolved Injuries” or “Unresolved Illness” section and must include date and time of injury, body part, side, type of injury, severity, mechanism of injury, and injured while at. The injury must be linked to the appropriate team & sport at the time of injury. Initial management and recommendations as well as relevant history of the injury must also be thoroughly documented on the “Unresolved Injuries” or “Unresolved Illnesses” page when the injury is created. Initial assessment must be added as a note with note type “Initial Injury Assessment” and should document your thorough assessment of each injury or illness to include, history, observation, palpation, ROM, MMT, Neuro Eval, special tests, and differential diagnosis.
Provide return-to-play guidance to athletes, coaches and parents/guardians following injury under the supervision of Medical Director/ physician/provider. 7. Maintains appropriate level of communication with Medical Director and consults with Medical Director / provider regarding referrals for other services as needed. 8. Participating in continuing education/professional development activities. Attends/presents at departmental in-service sessions as scheduled. Attends external professional growth activities as scheduled and communicates information to the team.
Adhere to policies and practices of applicable federal and state guidelines, professional organizations, entity facilities and entity specific policies. Document in Presagia EMR software the injury, treatment and encounter status of all student athletes for both home and away teams. Document at the beginning of each sports season all athletes' demographic information, team/sport participation and grade. All Injuries to athletes must be documented in Presagia within 24 hours. New injuries or illnesses should be created in the “Unresolved Injuries” or “Unresolved Illness” section and must include date and time of injury, body part, side, type of injury, severity, mechanism of injury, and injured while at. The injury must be linked to the appropriate team & sport at the time of injury. Initial management and recommendations as well as relevant history of the injury must also be thoroughly documented on the “Unresolved Injuries” or “Unresolved Illnesses” page when the injury is created. Initial assessment must be added as a note with note type “Initial Injury Assessment” and should document your thorough assessment of each injury or illness to include, history, observation, palpation, ROM, MMT, Neuro Eval, special tests, and differential diagnosis.
Provide return-to-play guidance to athletes, coaches and parents/guardians following injury under the supervision of Medical Director/ physician/provider. 7. Physical Therapist - Visiting Nurse Service (VNS) - Full Time

Under the general guidance of the Director of Rehabilitation Services, provides physical therapy services to patients of the agency, under the direction and written orders of a physician. The physical therapist (PT) works within the scope of practice as defined by the NYS Education Department for physical therapists. The PT practices his/her duties in a manner that is compliant with federal, state, local codes, rules and regulations, and law.
Essential Duties and Responsibilities
Evaluates the degree of function and disability of patients unless contraindicated by his/her professional judgment or a doctor's written order.
Develops a treatment plan based on patient evaluation and physician orders, designed to maximize independent, functional mobility.
Performs physical therapy procedures upon the written order or referral of a licensed physician.
Records on patients charts all physical therapy evaluations, re-evaluations, progress notes, consultation and discharge summaries within the time frames established by policy and procedure. Completes documentation by the end of the day on which the visit was made.
Performs the Comprehensive Assessment (OASIS) at the appropriate time points, and performs aide orientations and biweekly supervisions.
Assesses the need for adjunct services, if not already involved, to achieve desired patient outcomes, e.g. SLP, OT, MSW, aide, skilled nursing.
Educates the patient and family in the therapy process and treatment.
Participates with other personnel in the formulation of the patient's total plan of care, and confers with the clinical team.
Consults with the physician on a regular basis regarding the therapy plan of care and patient progress.
Develops or assists in the development of physical therapy policies and procedures.
Assists in the instruction of agency staff as needed.
Performs other appropriate duties as needed.
Other Responsibilities:
Timely and accurate documentation as evidenced by clinical record audits and the appropriate submission of required documentation.
Follows infection and exposure control protocols.
Meets with direct supervisor during scheduled times and attends required agency and departmental staff meetings.
Responsible for keeping current in therapy practice and knowledge. Participates in in-service training programs.
Actively participates in VNS's quality assurance and quality improvement programs
Complies with agency policies and procedures.
Follows the Code of Conduct.
Undertakes special activities as assigned and other duties as appropriate.
Qualifications:
Graduate of an accredited program in physical therapy.
Licensed and currently registered by New York State Education Department as a Physical Therapist.
At least two years of supervised clinical experience.
Understands the principles, techniques, and practices of physical therapy.
Understands the operation and maintenance of apparatus and equipment used in physical therapy.
Understands the pathological conditions and socioeconomic factors affecting a patient's disabilities.
Competent clinical skills as evidenced by the successful completion of a skills checklist appropriate to the area of work, upon hire and every three years therafter.
Possess a current NYS driver's license and reliable transportation to/from work assignments
Must have some familiarity with medical data base systems.
Must provide immunization record and physician's statement of health at hire and upon return to work from illness-related absence of more than 3 days.
High degree of confidentiality required.
