How you'll contribute
A Physical Therapist who excels in this role:
Performs initial and ongoing assessments of patient's condition.
Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.
Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient.
Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication.

Expectation and Care Management - 60%
* Initiates contact with physician for orders and treatment plan formulation and/or revision as the patient's condition requires.
* Participates in the development and implementation of the patient OASIS assessments, Evaluations, and discharge OASIS documentation.
* Performs physical therapy treatments as prescribed by the physician, evaluating and treating patients by applying appropriate therapy techniques and modalities in client's home environment.
* Recommends modifications or changes in the patients' Plan of Care based on evaluation of their progress toward the patient goals.
* Educates/instructs/recommends patient, family members and other health team members of the proper techniques, for exercise program, precautions and self care activities to assure safety for caregivers and patient.
* Participates in the interdisciplinary team meeting (ITM) by exchanging team information on the progress and discharge plan in order to provide the highest level of continuity of care for the patient.
* Communicates with client, family members, clients' advocate(s), caregivers and physicians on patient's progress and discharge plan on a regular basis.
* Conducts home visits to implement the patient's Plan of Care in use of therapeutic and self care activities for the purpose of improving function and safety in order to allow the patient to return home at their highest level of function and independence.
* Participates in discharge planning by exchanging information with staff members, family members and physician to promote continuity of client care, in accordance with corporate policies, federal, state and local guidelines.
* Ensures that all work areas, treatment areas and equipment are maintained in a clean, sanitary and safe manner.
Administration and Reporting - 20%
* Maintains current and accurate patient records through use of personal EMR device, and paper/chart documentation in accordance with company, federal, state, and local guidelines.
People Management and Development - 10%
* Coaches and influences the work of the Licensed Physical Therapy Assistants (LPTA) regarding the performance expectations of their position.
* Fosters a positive work environment that attracts, retains, and motivates employees.
* Interviews, selects, and orients new employees.
* Defines, establishes, and promotes performance expectations so staff understands and demonstrates excellence through high quality and consistent performance.
* Evaluates job performance of staff in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth.

Experience

**Responsibilities And Duties:**
- Completes intake evaluation including clinical assessment, exercise tolerance test (stress test or 6 minute walk test), testing evaluation, documentation and billing
- Develops exercise prescription & progression for patients in all 3 programs
- Case management of patients enrolled in Cardiac, pulmonary and/or PAD SET Rehab. Includes completion and ongoing monitoring of the individualized treatment plan, referral & consultation with dietitian and stress management, smoking cessation as needed, etc. Includes communication with Medical Director and/or referring physician. All documentation completed in CareConnect
- Circulation during exercise classes & CareConnect documentation of exercise, progression, vitals & other pertinent information
- Ability to develop exercise prescriptions to teach and demonstrate strength based exercises, including hand weights, body weight and stretching to progress patients in cardiac, pulmonary and PAD SET rehab.
- Specialized knowledge related to billing for monitored/non monitored cardiac rehab; pulmonary rehab & PAD SET
- Understanding, monitoring and troubleshooting continuous cardiac monitoring
- May provide inpatient Cardiac Rehab teaching in the hospital (Phase I)

**SPECIALIZED KNOWLEDGE**
Experience in cardiac, pulmonary and/or SET for PAD Rehab including an internship in cardiopulmonary rehab. The position functions in partnership with a multi-disciplinary team to provide direct clinical management of patients referred to a cardiopulmonary rehab setting. Work requires a high level of problem-solving skills, analysis and interpretation of clinical problems related to cardiac disease, pulmonary disease and/or peripheral arterial disease. Position requires excellent communication skills (verbal and written), active listening skills, problem solving, independent decision making and managing stressful situations. Individuals in this position are required to respond to emergency situations appropriately. Clinical care is documented in the medical record in the form of individualized treatment plans, creation of exercise prescriptions and general health care coordination.

Clinical Cardiac Electrophysiologist Opportunity
Youngstown, OH
Mercy Health Physicians-Youngstown is seeking a board certified or board eligible clinical cardiac electrophysiologist to join our Heart & Vascular Institute. Primary responsibilities include inpatient and outpatient clinical care and procedural competence in diagnostic electrophysiology studies, catheter ablations, and device implantation. Competency in complex ablation, lead extractions, and left atrial appendage closure is not required, but will be looked upon favorably. Candidates are expected to have completed fellowship training in clinical cardiac electrophysiology and be board certified or board eligible by the American Board of Internal Medicine.
The Mercy Health Physicians Youngstown Heart & Vascular Institute is the largest and highest volume electrophysiology practice in the region, with a total of 15 employed cardiologists including 5 interventional cardiologists and 3 EP specialists. There are 13 advanced practice clinicians supporting delivery of care in both inpatient and outpatient settings. The position will be considered 100% electrophysiology, and the call model is 1:4 electrophysiology-only call. A newly built state-of-the-art electrophysiology lab and hybrid operating room, both equipped with 60 Vantage View monitors, have been added to an existing fully functional electrophysiology lab. Both electrophysiology labs are equipped with 3D mapping, with the team currently using Carto Biosense. Clinical research is ongoing and expanding, and our brand-new electrophysiology lab also includes a separate viewing room equipped with 2 viewing screens for education and training purposes.
The Mercy Health Heart & Vascular group also includes 3 board certified cardiothoracic surgeons and 4 board certified vascular surgeons, who collaborate with interventional cardiology and electrophysiology as part of a structural heart team. Electrophysiology and structural heart cases are supported by 3 cardiothoracic anesthesiologists who are board certified in TEE, as well as a team of CRNAs. The service line performs high volumes of cardiac, thoracic, and vascular cases annually. This specialist team is highly integrated and includes a multidisciplinary valve clinic that supports a robust TAVR and MitraClip program, and the electrophysiology group provides the only left atrial appendage closure and lead management programs in the region. The main campus hospital is a level 1 trauma center, is a certified stroke center, is chest pain certified with PCI, and is the only sepsis center of excellence in Ohio.

Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community.
Design/train staff and family members to carry out the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings.
Coordinate/oversee/supervise/instruct and evaluates Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are rendered. Ensure that interim (verbal) orders received from the physician are accurately documented and implemented. Submit evaluation, treatment plans and discharge summary to the supervisor and care management staff.

Required Experience/Skills:
Degree from an accredited Occupational Therapy Program
A minimum of six months of occupational therapy experience preferred
Home Health experience a plus
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
A valid driver's license, auto insurance, and reliable transportation are required.
