Key Responsibilities and Requirements Patient Care & Visits Conduct timely and thorough evaluations and treatments for new admissions and readmissions. Reliably perform follow-up visits based on practice guidelines and patient needs. Address acute changes in patient conditions, ensuring optimal care to prevent unnecessary hospitalizations. Medical Competency & Treatment Accurately assess patients and develop management plans that are consistent with current standard of care and our group's practice guidelines. (Practice guidelines and other reference resources will be provided by the practice upon hiring). Prevent unnecessary emergency room visits and hospitalizations by promptly addressing medical concerns such as infections, dehydration, volume overload, uncontrolled diabetes, acute kidney injury, and uncontrolled hypertension. Committed to lifelong learning. Communication Reliably and promptly respond to calls, texts, and other requests from the center's nursing staff during required hours. There will be no overnight call duties. Dedicate an appropriate amount of time to patients and/or families during medical encounters to ensure comprehensive care. Clearly and effectively explain care plans to patients, families, and the directors of nurses the long-term care communities. Professionalism & Teamwork Establish and maintain strong rapport with patients, families, and long-term care center's administrative and clinical leadership teams. Exhibit a courteous and professional attitude when communicating with collaborating physicians, nurses, other healthcare providers, patients, and families. Treat all parties with respect and demonstrate openness to constructive feedback to foster a positive working environment. Documentation & Compliance Promptly complete progress notes per practice requirements. (The EHR system has been customized for the group to maximize efficient charting.) Qualifications Active Nurse Practitioner license or Physician Assistant license in the state of Texas. Clinical experience as an NP or PA in one of the following settings: assisted living communities, skilled nursing facilities / nursing homes, hospital (such as hospitalist team or general medicine consult team), primary care clinic, or internal medicine subspecialty clinic (such as cardiology, pulmonology, or endocrinology). Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions. Verbal and written communication skills. Interpersonal skills. Critical thinking and complex problem-solving skills. Skill in the use of computers and related software applications. Ability to develop and implement comprehensive outcomes-based patient treatment plans. Ability to counsel patients concerning medical and psychological/psychiatric conditions, treatment plans, and behavior modification. Ability to interpret and evaluate laboratory and other diagnostic tests. Current license to practice as a Physician Assistant in the state of Texas. Basic Life Support within 30 days of hire or transfer.
