Position Summary:

Your contribution to the team includes:
* Provision of comprehensive primary care services for all ages in a primary medical care ambulatory clinic setting, and in Health care for the Homeless facilities working independently and interdependently with other HealthPoint providers.
* Provide care consistent with evidence-based medicine.
* Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
* Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
* Complete accurate and timely documentation in patient records and superbills.
* Participate in patient triage system.
* Participate in a shared call system with other providers.
* Perform minor outpatient procedures in accordance with the scope of practice and HealthPoint privileging policy.
* Provide consultation to other clinic providers either in person or by telephone.
* Consult with clinic physicians, specialists, or other health care professionals as appropriate.
* Administer medications in clinic as needed.
* Assist with meeting clinic patient care team productivity and quality care standards.
* Participate in development and utilization of HealthPoint practice guidelines and procedures.

* Maintain credentials essential for practice, to include licensure, board certification, society memberships and CPE.
Must have's you'll need to be successful:
* Graduate from an accredited nurse practitioner or physicianassistant program.
* Washington State nurse practitioner or PhysicianAssistant license. Board certification required.
* Demonstrated operating knowledge of computers: intermediate level ability with Word, Outlook, and other required software programs; prior experience with electronic medical records preferred.

Responsibilities
What You'll Do:
We are hiring a Nurse Practitioner or PhysicianAssistant who will direct, coordinate, and provide medical/clinical services to specified individuals within the facility, including, but not limited to intervention, psychiatric evaluation and assessment, differential diagnosis, treatment planning, medication management, engagement, family meetings, supportive psychotherapy, individual education, risk assessment, and discharge planning.
* Serves as the attending clinician to specified individuals. Completes initial psychiatric assessments, crisis interventions, family interventions, focused crisis diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, individual education, progress toward treatment goals, discharge planning including medical, psychological, social, behavioral and developmental aspects of the individual's care, treatment, and services.
* Documents individual's progress in the clinical record including admission orders, progress notes, and discharge orders. Oversees delivery of medication education and consent, according to established policy.
* Orders laboratory tests according to standards of clinical practice. Reviews the health assessment completed by nursing staff, obtains a past and current medical history, and conducts a focused physical examination as indicated, according to established guidelines.
* Initiates contact with Primary Care Physicians and Emergency Room clinicians according to established guidelines.
* Assesses individual's needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with Outpatient Behavioral Health Provider according to established guidelines. Reviews and utilizes all available documents from the crisis clinical team.
* Provides necessary information to clinical team regarding inpatient utilization. Utilizes appropriate medication based on best practices.
* Collaborates with other personnel to ensure that each treatment modality is appropriate for the individual in addition to working with outside psychiatric providers in crisis and inpatient services. Works with the community to review involuntary petitions, assess involuntary patients, and coordinate with the legal system to follow established guidelines for Involuntary Evaluation and Treatment.
* Participates in medical staff meetings including regional and quarterly meetings; participates in all required meetings; and participates in any meeting as requested by Medical Leadership.
* Performs all other duties as assigned.
Qualifications
What You'll Bring:
* NP: Unrestricted Registered Nursing license in the State. Completion of an accredited Nurse Practitioner program with a Master's level certification in Psychiatric Mental Health (PMHNP).
* PA: Completion of an accredited PhysicianAssistant program.
* Licensed in State of employment and National Certifications.
* Experience in the delivery of crisis mental health and/or delivery of family health treatment to children, adolescents, and/or adults.
* Unrestricted DEA license.
* Must be able to perform Safe Clinch patient restraint techniques.

Supervision:
The APP (Advanced Practice Provider), ARNP or PA, will work under the supervision of the Attending Physician on service and work collaboratively with physicians in post-graduate training programs (i.e. residents and fellows in Internal Medicine and Critical Care Medicine)
Patient population:
Critically ill patients with complex medical, surgical or neurological problems. Typical issues may include respiratory failure, ARDS (including extracorporeal life support), sepsis, cardiac arrest and complications of chronic medical conditions.
Responsibilities to include:
(Note: responsibilities may evolve over time depending on the needs of the service, demand in other critical care services and outreach roles)
1. Patient Care:
* Admit new patients (e.g. write admission note, develop initial assessments/plans, place admission orders in EMR)
* Provide frontline coverage for admitted patients (e.g. "cross-cover" / manage patients admitted by other providers; either daytime or nighttime)
* Participate in daily morning and afternoon rounds (e.g. present updated clinical information and problem-based plan of care on multidisciplinary ICU rounds, share tasks with other members of ICU team such as completion of daily rounding checklist, entering orders in EMR, documenting, etc.)

* Facilitate transitions of care to other service lines (e.g. documentation and direct hand-off for discharge from ICU to Acute Care, skilled nursing facility, surgical/anesthesia services for OR cases)
* Participate in family meetings and discussions of goals of care as member of critical care team
* Participate with team in critical care outreach activities for patients on Acute Care, Emergency Department or other ICU services depending on clinical need (e.g. responding to "Code Blue", Rapid Responses, support other ICU services with high clinical loads)
* Potential to work in other ICUs within HMC depending on clinical need (e.g. Neurocritical Care)
2. Procedures
* Assist or perform bedside procedures for hospitalized patients (e.g. central venous catheters, arterial lines, thoracentesis, paracentesis, lumbar puncture, point of care ultrasound, etc.) under the direction and supervision of the attending physician.

JOB OVERVIEW:
This role will provide direct clinical care within the DESC Opioid Recovery and Care Access (ORCA) Center. The ORCA Center is estimated to start clinical services in late summer to fall 2025 and will eventually be open 24 hours per day, 7 days per week, including holidays. It will offer four overlapping types of services, delivered by a team of prescribers, nurses, medical assistants and peer specialists, with operational leadership support:
Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments.
Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the “72-hour rule”, or Code of Federal regulations Title 21 §1306.07 (b)), for people with opioid use disorder (OUD), regardless of whether an opioid overdose has recently occurred.
Harm reduction counseling and OUD-related physical and behavioral health services, such as hepatitis C testing and treatment and HIV pre-exposure prophylaxis.
Follow-up care, at the ORCA Center or at times field-based, with individuals who have started MOUD to ensure support in MOUD continuation.
Before the ORCA Center opens, this role will focus on supporting the DESC Opioid Treatment Network team, an outpatient OUD treatment team, to provide care for clients with OUD and related health conditions. After the ORCA Center's opening, responsibilities will transition to the Center's clinical operations.
This is a clinical role without administrative or supervisory duties.
PRIMARY RESPONSIBILITIES:
Clinical Care
Provide evidence-based treatment for OUD, including initiation and management of buprenorphine (sublingual and long-acting injectable) and, according to DESC protocols, methadone initiation.
Conduct harm reduction counseling and provide services related to infectious diseases such as syphilis, hepatitis C, and HIV (including PrEP).
Manage and treat conditions commonly seen in individuals with OUD, such as skin and soft tissue infections and linkage to mental health care.
Perform initial health assessments and follow-up visits for individuals starting or continuing MOUD.
Educate patients on health-related topics, treatment plans, and harm reduction strategies.
Document clinical care in accordance with DESC and regulatory standards.
Care Coordination
Collaborate with an interdisciplinary team of clinicians, peer specialists, and social workers to provide holistic care.
Ensure continuity of care for individuals transitioning between ORCA Center services and other health care providers.
Professional Development
Stay updated on best practices in addiction medicine and harm reduction through continuing education.
Participate in in-service training and case discussions to maintain high clinical standards.
Requirements
MINIMUM QUALIFICATIONS:
Licensed as a Nurse Practitioner (Family or Adult NP) or PhysicianAssistant in the State of Washington.
Registration with the Drug Enforcement Agency.
Experience or training in addiction medicine and harm reduction.
Strong commitment to serving individuals with complex needs, including homelessness and co-occurring behavioral health conditions.
Familiarity with trauma-informed care, harm reduction principles, and equity frameworks.
Ability to remain open and receptive to constructive feedback.
Strong ability to approach those with differing opinions with curiosity & active listening.
Ability to work collaboratively in a multidisciplinary team environment.

**$10,000 Sign-on Bonus for eligible new part-time and full-time employees joining UW Medicine** **Harborview Medical Center** has an outstanding opportunity for **APP (Nurse Practitioner or PhysicianAssistant)** . **WORK SCHEDULE** + 90% FTE (36 hours per week), Days, 12 hour shifts (and including some nights, week-ends and holidays).
Supervision:
The APP (Advanced Practice Provider), ARNP or PA, will work under the supervision of the Attending Physician on service and work collaboratively with physicians in post-graduate training programs (i.e. residents and fellows in Internal Medicine and Critical Care Medicine)
Patient population:
Critically ill patients with complex medical, surgical or neurological problems. Typical issues may include respiratory failure, ARDS (including extracorporeal life support), sepsis, cardiac arrest and complications of chronic medical conditions.
Responsibilities to include:
(Note: responsibilities may evolve over time depending on the needs of the service, demand in other critical care services and outreach roles)
1. Patient Care:
- Admit new patients (e.g. write admission note, develop initial assessments/plans, place admission orders in EMR)
- Provide frontline coverage for admitted patients (e.g. "cross-cover" / manage patients admitted by other providers; either daytime or nighttime)
- Participate in daily morning and afternoon rounds (e.g. present updated clinical information and problem-based plan of care on multidisciplinary ICU rounds, share tasks with other members of ICU team such as completion of daily rounding checklist, entering orders in EMR, documenting, etc.)
- Daily management of ICU patients (e.g. evaluate and manage ICU patients, write daily progress notes, formulate and implement plans of care, communicate with consultants, update family/caregiver daily, work collaboratively with other members of multidisciplinary team including physicians in training)
- Facilitate transitions of care to other service lines (e.g. documentation and direct hand-off for discharge from ICU to Acute Care, skilled nursing facility, surgical/anesthesia services for OR cases)
- Participate in family meetings and discussions of goals of care as member of critical care team
- Participate with team in critical care outreach activities for patients on Acute Care, Emergency Department or other ICU services depending on clinical need (e.g. responding to "Code Blue", Rapid Responses, support other ICU services with high clinical loads)
- Potential to work in other ICUs within HMC depending on clinical need (e.g. Neurocritical Care)
2. Procedures
- Assist or perform bedside procedures for hospitalized patients (e.g. central venous catheters, arterial lines, thoracentesis, paracentesis, lumbar puncture, point of care ultrasound, etc.) under the direction and supervision of the attending physician.
- Procedures may be referred from Medical ICU patients or patients on other clinical units (e.g. Hospital Medicine, Surgical services)
3. Quality Improvement, Research and Scholarship
- APPs are expected to be active participants in research, QI projects and regular QI meetings
o Ideally participation involves contributing to (or initiating) projects that leverage existing resources and ongoing initiatives that will result in measurable improvements in processes of care and/or scholarly output (e.g. presentation, publication, etc.)
o Mentorship is available from faculty and staff
- Attendance and participation in educational conferences
**REQUIREMENTS**
*Nurse Practitioner:
- Masters or Doctorate Degree in nursing from an accredited school of nursing
- AND Washington State Advanced Registered Nurse Practitioner License with prescriptive privileges
- AND National certification as an Acute Care Nurse Practitioner
*PhysicianAssistant:
- Licensure as a Physician's Assistant in the State of Washington
