Nurse Practitioners & PhysicianAssistants - Job Description:
Conduct in-person home visits to manage acute illnesses and injuries.
Provide treatment and clinical management consistent with evidence-based guidelines.
Work collaboratively with our multi-disciplinary team of field providers and care coordinators to efficiently manage complex care needs in the home.
Assesses patients for risk factors associated with the care of the acute and complex chronically ill patient including: medication compliance and side effects, limited mobility, impaired nutrition, fall safety, and social isolation.
Maintain a safe, secure, equitable and healthy work environment by following safety procedures, quality standards and comply with regulations
Maintain patient confidentiality
Complete patient records in the Electronic Health Record in a timely manner that meets acute care compliance standards and captures patient acuity.
Respond to on-demand and scheduled calls and focus on quality and efficiency metrics to improve patient experience.
Stay current on in-home acute care literature, technology, and regulatory issues.
Nurse Practitioners & PhysicianAssistants - Education and Experience:
Master?s Degree in PA or NP Studies from an accredited university.
Valid PA or NP Certification and license to practice in New York.
Able to meet credentialing requirements, including DEA certificate, and BLS certification.
Experience in providing care in an emergency department or hospitalist medicine practice.
Additional experience in geriatric medicine, hospital at home, or post-acute care a plus.
Physical Requirements:
Daily use of PPE and electronic documentation.
Frequent, prolonged standing, walking, and patient/equipment transfers.
