Internal Medicine - Unit 53
Our unit is extremely focused on teamwork and the best patient care can only happen with teamwork. We love having new people join our team and do this by having a very open and welcoming culture. We support and develop all members of our team creating a culture of continuous learning. We opened this unit as a team in November 2017 which was an amazing experience. We have all learned and grown so much in our nursing careers and got to do this together. We are so fortunate to care for such a wide variety of patients. Every day we are all seeing or learning something new to make us better nurses and caregivers.
This medical surgical 28 bed unit provides advanced and specialized care to patients with a wide variety of medical conditions including renal failure, liver failure, respiratory complications, infections and many other diagnoses. Comorbidities and psychosocial backgrounds in turn creates individualized and customized care for each of our patients.
This unit is identified as a dedicated unit for the patient population it serves, allowing for close collaboration with the physicians and other ancillary team members.
Responsibilities and Essential Job Functions

* Assists the surgeon during operations by opening locations for surgery, operating laparoscopic cameras, suturing wounds, using surgical staplers, providing suction or anything else that the surgeon may need.
* Assists surgeon by maximizing patient volume, monitoring, and contacting high risk patients, returning patient calls in a timely manner, and providing follow up. Coordinates with other departments for continuity of patient care. Interprets radiology results to determine surgical plan.
* Develops with the patient and/or caregivers, a mutually acceptable cost-aware plan of care that maximizes health potential. Formulates a treatment plan which includes ordering diagnostic tests, selecting appropriate pharmacologic and non-pharmacologic interventions, develops a patient education plan, and obtains appropriate consultation or referral.
* Consults with physician appropriately regarding patient care issues and concerns. Communicates clinical findings to, and works collaboratively with, physicians to develop patient treatment and education plans.
* Performs procedures independently in the office. Conducts wound care for ongoing surgical wounds, requiring debridement in the office setting.
* Reviews all referrals for soft tissue tumors, skin lesions and properly delegates to self or surgeon for excision. Completes all discharge summaries for physicians.

The Ward Family Heart Center at Children's Mercy Kansas City is the premier high-volume pediatric heart program in the Midwest. Our focused, interdisciplinary care of children with complex congenital and acquired heart disease brings to bear the expertise of board-certified pediatric cardiologists, congenital cardiothoracic surgeons, cardiac intensivists, anesthesiologists, and advanced practice pediatric critical care nurses. Our facilities include a new state-of-the-art Cardiac Intensive Care unit in 2021 and the most advanced technology available for diagnosis, treatment, and translational research for our patients, who range in age from fetal to young adult.
The Acute Care Cardiology Unit (ACCU) is the inpatient cardiology service. The Acute Care Cardiology APPs provide 24/7 coverage to all high acuity cardiac patients which includes but is not limited to, single ventricle patients, transplant patients, post-operative and post- cardiac catheterization patients. The APP would participate in patient care rounds (with direct patient management), order and interpret laboratory tests and diagnostic procedures, analyze and prioritize complex data, develop differential diagnoses, and implement plans of care. The APP would be responsible for daily progress notes as well as family education. The APP is a resource for residency staff and a collaborator with the multiple cardiac sub-specialties. The APP would also work with the CV surgeons on post-operative care, including but not limited to incisional management, removal of chest tubes and pacer wires, and wound vac therapy.
PhysicianAssistants II (PA-II) provide proficient, comprehensive health care services to a defined patient population; obtaining the health history; performing a physical examination, diagnostic, and therapeutic procedures; formulating and implementing a plan of care; and participating in patient and family education. Each PA II demonstrates commitment to professional leadership and provides leadership for quality & outcomes improvement activities, clinical research or evidence based practice, and organizational development.

This role will also support inpatient care needs in addition to daytime triage for Emergency and community care providers. The PhysicianAssistant/Nurse Practitioner provides primary health care services including acute, chronic, and preventative medical care, with an emphasis on chronic disease management and pediatric care using the Patient Centered Medical Home model of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES
* Provides diagnostic and therapeutic services to patients including performing history and physical exams, prescribing medications, managing acute, chronic, and preventative care, making referrals for hospital and specialty services when applicable under the direction of a collaborating physician
* Reviews and interprets received laboratory, radiology, and consultation reports in a timely manner and provides direction to nursing staff or directly contacts the patient with results and/or treatment recommendations
* Provides quality care to patients by following the most current evidence-based guidelines, best practices, and state, local, and federal guidelines
* Provides ongoing patient education regarding current diagnosis and plan of care
* Assists in establishing standards of care for management of patients, in collaboration with other providers and staff
* Provides accurate documentation regarding the reason of the visit, medical history, current medication list, recommended plan of care, orders for medications and/or labs, referrals, transition of care, and follow-up
* Identifies barriers to patient care and delivery of services and assists with formulating the plan of care and coordination of care by making referrals to internal support services or outside collaborating organizations
* Collaborates with the Chief Medical Officer (CMO), Chief Clinical Officer (CCO), other providers, and other staff to evaluate services as well as plan and develop programs
* Participates and provides input in monthly Medical Executive meetings and other committee meetings as well as participating in Quality Improvement (QI) activities, Peer reviews, Pharmacy and Therapeutics, and evaluation and implementation of clinical programs
* Reviews current literature to remain current with advances in therapeutic interventions
* Ensures participation in continuing education requirements
* Maintains patient confidentiality at all times in accordance with HIPAA regulations
MINIMUM REQUIREMENTS
* · Graduated from an accredited Nurse Practitioner or PhysicianAssistant program
* Spanish bilingual required
* · Licensed in the State of Missouri
* · Basic Life Support (BLS) Certification
PREFERRED REQUIREMENTS
* Active Missouri DEA and BNDD Licensure
* Two years' experience as a PA or Nurse Practitioner in an Federally Qualified Health Center
* Experience with eClinical Works (eCW)
