Summary:
The APP, in collaboration and consultation with physicians, the Lead Nurse Practitioner, Care Coordinators, staff RNs and other health care professionals, provides holistic, compassionate care to Medicare Advantage plan members and families. The APP practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. Essential Duties & Responsibilities:
Performs in-depth health risk assessments on all assigned plan members
Works collaboratively with physicians, Care Coordinators, facility staff, family members, key care givers and any appropriate ancillary medical personnel as appropriate
Responsible for identifying overall quality improvement activities through assessing and planning health care which is individualized to the member and consistent with policies and protocols
Reviews and interprets the medical, surgical and social history of members
Performs appropriate screening procedures for members
Interprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up, reconciles medications
Responsible for prescribing or performing various treatment modalities - including, but not limited to: medications, injections, wellness counseling, suturing and ancillary testing
Is compliant with OSHA, State Board of Nursing

Summary:
The primary purpose of the facility-based APP is to administer Provider-level direct patient care services within the Post-Acute Care setting.
Essential Duties & Responsibilities:
Conduct clinical interventional and episodic direct care visits, and appropriate re-evaluation of care visits.
Provide ongoing evaluation and treatment of chronic, co-morbid illnesses, as medically appropriate
Evaluate/treat residents for changes in condition.
Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately for all patients under their care, as allowed and/or requested by collaborating physicians.
Coordinate care with collaborating physicians as indicated for overall medical management.
Conduct Annual Wellness Visits on all long-term Nursing Facility residents and may perform Annual History and Physicals as requested by Facilities.

Coordinate care with other disciplines, such as Therapists, Specialists, and Social Workers.
Maintain communication with families/attend family conferences when possible.

Perform return to work assessments to ensure fitness for duty and a safe workplace environment.
Perform fitness for duty evaluations.
Assist with coordination of care, physician referrals, transitional duty program.
Provide medical review of surveillance and immunization records of contracted employees, physicians, volunteers and other affiliates.
Assist with TB Surveillance and Hepatitis B immunization programs.
Maintain professional growth and development through approved seminars, workshops and professional affiliations.
Review and sign all EKG, pulmonary function tests, visual exams and lab tests as required.
Review and sign wellness center medical authorizations
Perform other work related duties as assigned or requested.

Education:
Graduate of an accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program.
Licensures/Certification:
National Board Certification.
Current state level Medical license in good standing.
Experience:
Previous experience practicing in Occupational Medicine, Internal Medicine, Family Practice, or Emergency Medicine is desirable.
Previous experience in working with Workers Compensation, DOT, and OSHA Regulated Exams desirable.
Valid motor vehicle license and clear driving record.
