We often see patients being admitted for sepsis, chronic wound care, psychosocial behaviors, complications or side effects of treatment/therapy, bleeding, respiratory complications, goals of care conversations, and so much more. Patient care is a collaborated approach with our multi-disciplinary and consultation teams. Our unit shares space with the ICU which allows for a close working relation with our ICU partners. We are a patient centered team with an extreme compassion to support and advocate for our patients. Our unit has allowed us to provide individualized care to our patients and their caregivers with the installment of new medical equipment and amenities. Given the size of our unit and the patient population we serve, we are given the opportunity for new learning experiences that will provide personal growth within our nursing profession. The Clinical Nurse I provides professional nursing care through assessment and accurate documentation, diagnosis, planning, implementation and evaluation for the comfort and well-being of patients. Carries out a plan of care specific to unit competencies for the target patient population. Coordinates care delivery and education through collaboration with patient, family and all health care team providers to ensure safe, effective and quality patient care. Veterans are scheduled for appointments at contracted facilities for compensation and pension exams and embeds practitioners in facilities as needs warrant. Dedicated facilities are in areas where there is a large Veteran population with the potential for high volume. The compensation and pension exam helps the VA answer questions about Veterans' current health. The practitioners will not decide the percent of disability or rating, develop treatment plans, or prescribe medication. Completed documentation is sent to the VA for final determination on all disability claims. Prior to the exam, the practitioner reviews a Veteran's medical or mental health history, using electronic bookmarks that highlight the portions applicable to the Veteran's claimed condition.
