Responsibilities:
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities

Supports the patient and family by explaining the perioperative experience with specific instructions to the patient and family regarding their responsibility in the outpatient process
May or may not coordinate the surgical schedule considering special needs of the patient
Performs within guidelines of specific privileges approved by the governing body and delegated by the Medical Director
May or may not confer with the surgeon about the planned anesthetic to be administered
Documents orders and considerations specified by surgeon and carried out by Physician
Monitors patients and administers medications according to Facility policy
Performs preoperative activities according to Facility policy
Provides support to medical and office staff
Knowledge, Skills and Abilities
Ability to assess situations, resolve within scope of ability, or escalate as appropriate
Ability to respond effectively to emergencies, actual events or drills.
Ability to follow instructions with little supervision
Ability to maintain professional demeanor in all aspects of role
May or may not make hospital rounds for physicians when indicated
Assists in completing paperwork for patients as indicated
Travels between facilities when indicated
Strong customer service skills and excellent communication skills
Job Requirements
Completion of an accredited Physician Assistant Program is required.
Must maintain CEU's in pertinent clinical practice according to licensure requirements.
BLS Certification must be maintained.
Prefer 2 years of experience. Orthopedic experience highly desired. However, new graduates are encouraged to apply
