What you'll be working on:
Treating patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups
Continuous learning during weekly Clinical Rounds and through other modalities
Ongoing collaboration with both virtual and in-office teammates via daily huddles
Utilization of your specific clinical training and opportunities to give exceptional care to patients virtually
Education, licenses, and experiences required for this role:
Completed an accredited PA program with a national certification
In the past 5 years, practiced as an Advanced Practitioner for at least:
2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
Currently licensed in NY with ability to obtain additional state licenses
Ability to work weekday and weekend shifts (every other Saturday AND or Sunday required)
Current shifts range from (6am-5pm EST, 7am-6pm EST, 8am-7pm EST, 11am-10pm EST, 12pm-11pm EST, 1pm-12am EST)

The base hourly range for this role is $59.10 to $65.50 per hour based on a full-time schedule plus evening and weekend differential. Position Overview
Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services
Responsibilities include:
Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.
Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.
Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.
Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care.
Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate.
Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented.
Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.
Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags).
Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care.
Ensure that all progress notes and orders are signed before the end of the shift.
Respond to emergencies in a timely and professional manner.
Notify the staff MD and Urgicare about 3 hour runs and EMS activation.
Complete special housing rounds when assigned.
Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards.
Maintain clinical competency by participating in all CME and CHS training and in-service requirements.
Maintain your schedule as directed with particular attention to punctuality and timely notification of absences.
Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed.
Complete tasks as delegated by a Site Medical Director or other supervising clinical team member.
Maintain and provide documentation of all required credentials.

HPSA score places group in a good position for loan repayment locally, statewide, Federally and Immigration support.
Small hospital in Upstate NY, 55 miles from Albany, 45 minutes from Saratoga.
Low volume labor and delivery (500 per year). The low volume can be attributed to demographics.
This year group has seen a 20% upswing by end of June.
Group has a strong commitment to deliver this service and a beautiful birthing center.
Currently run Womens Health out of 3 of 11 Primary/Specialty Care Centers.
One is in Gloversville and is attached to the hospital, another in Perth, NY which is 9.2 miles away and the last in Fonda, NY. (recently opened - August 2017).
4 MD group with 2 midwives and 3 WH NP.
Office practices are quite strong.
Four day week and a one in 4 call.
Nurse midwives who take 1st call when they are assigned for call. Which means that you would be called by doctors, ER or the birthing unit in those circumstances. but not
patients. (and, yes, in full disclosure, the math dictates that there are times when a midwife will not be on call with you, but it is fairly rotated and at those times, gyn patientcalls go to our other NP call rotation).
We would assume that you will do deliveries and surgeries.
Salary is based on the Median for our Geographic region.
This assumes a 4 day week and a one in 4 call obligation.
Should a physician wish to raise the number of days in the week, or other varying factors, we can always talk about salary changes.

Compensation depends on the conditions you choose.
Group starts at slightly higher than the MGMA Salary median for your specialty
8 weeks PTO, with one week per year placed in an extended sick time bank (EST) which could be accessed after 7 calendar days of continuous documented
illness.
CME reimbursement stipend these days for OB/Gyn is 4500.00.
Group pays approximately 90% of the premium for Family Health Insurance.
There are 7 major holidays during which the offices are closed. You would be paid for these. These days
still need to be covered in the 1 in 4 call fashion.
Malpractice. We offer Individual Occurrence and this affords you the ability to take the risk management course and get the excess layer.
Additional Perks
We pay Dea, licensing, society dues, etc. and we throw in about 400.00 for whatever other things you want. Some use them for their internet bill at home since
they use remote access, or to buy and new smart phone, etc. we pay 38.00 per month for you to use your personal phone for call.
There is a sign on. We look at the market at the time. I am seeing 30-35k as prevailing.
The physician may also avail themselves of a 10% salary lump sum disbursement at the start of employment. Such 10% is then subtracted from your rate of pay in 52 equal
payments beginning in the first payroll period.
After the 52 weeks, the weekly salary reverts to the regular salary rate.
We pay reasonable moving expenses.'
In the header to this informational page, I listed that you would have great colleagues. I cannot stress enough what high quality individuals we have in that sector. I think the most difficult thing for any Physician to deal with is difficult or low performing colleagues. You would never have that concern here. State Level Programs
REGENT PHYSICIAN PROGRAM is for Primary Care Providers. The provider can apply up to 4 years for 10k each year. The setting on this does not matter. They may work in OP or IP.
DANY PROGRAM (Doctors Across NY) applies to any specialty or PCP which is in short supply. This provides as much as 120K for a one time 3 year commitment and is tax free.
PHYSICIAN PRACTICE SUPPORT PROGRAM makes 100k available to either the Practice (hospital) or the Physician to support a new practice. Should the hospital use the
funds, the Physician may apply after 2 years.
US PRIMARY CARE SERVICE CORP is for APP and mental health providers. Provides 30k a year for up to 5 years. The provider, however, has to do 50% practice in Mental Health and be able to provide documentation. The benefit to support Licensed Social Worker training is being fleshed out. This one would be competitive.
National Level Programs
NATIONAL HEALTH SERVICE CORP provides 50K tax free for a 2 year commitment, 20k for another year per year up to 16 years or up to 370K or when the loan is
repayed.
The site must have a site certification (we do, except for our newest 3 and we are working on that). This benefit is for Physician and NP.
NURSING CORP provides funds for nurses going back to school, makes scholarships available.
VISA SUPPORT - provides grants for immigration work, gives J1 advise to the hospital or employer from an immigration attorney.
Participation in the HPSA ACUMEN program puts us also on grant watch for programs which arise from which either our providers or our hospital would benefit.
