The Department of Infectious Disease within the Integrated Hospital-Care Institute at Cleveland Clinic has openings for individuals to join our or team within the Cleveland Clinic Health System. The Cleveland Clinic Department of Infectious Disease at main campus is a robust clinical group comprised of 35 staff members. It provides full spectrum infectious disease care for a 1,200 bed referral center with our unique model of 8 inpatient clinical services and our outpatient department. Members of the department pursue specialty interests including solid organ transplant / BMT, bone and joint infections, neurological infections, care of the patient with HIV, granulomatous infections, and endocarditis and infected cardiac devices. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient. It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not). The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs. The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market. PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company. Performs other duties as assigned and modified at manager's discretion. KNOWLEDGE, SKILLS & ABILITIES: Competencies for Success Availability and Accessibility for patients to build trust from their patients. It is expected that PCP's will make themselves as available to their patients as possible by being open and available for walk-in visits and answering phone calls and messages in a timely manner. Evidence Based Medicine - The PCP remains updated on evidence-based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. They specialize in providing care for the whole family and as such deliver comprehensive, ongoing care to patients regardless of age or gender as well as deliver comprehensive, ongoing pre and post natal care to pregnancy patients, and provide care for the general health and well-being of the female patients.
