Job Summary The purpose of this position is to connect uninsured hospital patients to programs that will cover their medical expenses. As a Patient Advocate, you will play a critical role in assisting uninsured hospital patients by evaluating their eligibility for various federal, state, and county medical or disability assistance programs through bed-side visits and in-person interactions. Your primary objective will be to guide patients face-to-face through the application process, ensuring thorough completion and follow-up. This role is crucial in ensuring that uninsured patients are promptly identified and assisted, with the goal of meeting our benchmark that 98% of patients are screened at bedside. Essential Duties and Responsibilities * Screen uninsured hospital patients at bedside in an effort to determine if patient is a viable candidate for federal, state, and/or county medical or disability assistance. * Complete the appropriate applications and following through until approved. * Detailed, accurate and timely documentation in both Elevate PFS and hospital systems on all cases worked. * Provide exceptional customer service skills at all times. * Maintain assigned work queue of patient accounts. * Collaborate in person and through verbal/written correspondence with hospital staff, case managers, social workers, financial counselors. * Answer incoming telephone calls, make out-bound calls, and track all paperwork necessary to submit enrollment and renewal for prospective Medicaid patients. * Maintain structured and timely contact with the applicant and responsible government agency, by phone whenever possible or as structured via the daily work queue. * Assist the applicant with gathering any additional reports or records, meeting appointment dates and times and arrange transportation if warranted. * Conduct in-person community visits as needed to acquire documentation. * As per established protocols, inform the client in a timely manner of all approvals and denials of coverage. * Attend ongoing required training to remain informed about current rules and regulations related to governmental programs, and apply updated knowledge when working with patients and cases. * Regular and timely attendance. * Other duties as assigned.
