Under the supervision of the Medical Director, this physician will provide primary medical care to participants in the Habitat Health program, in collaboration with an Interdisciplinary Team (“IDT”). Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested), and documents appropriately in the medical record as required. Provides ongoing and longitudinal primary care using evidence-based practice; is an active member of the IDT, communicates participant changes to team members, and participates in IDT meetings. Provides updates to the IDT on a regular basis. Assists in the development, implementation, and evaluation of the participant plan of care as described in PACE policy. Develops care planning goals based on the expressed wishes and preferences of participants within the context of their medical and psychosocial conditions. Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy.
