Job Summary: As a member of the Hartford Hospital Palliative Care Department, the Palliative Care social worker will provide comprehensive clinical and psychosocial assessment and intervention for patients with serious illness. The social worker must be comfortable working in a fast-paced inpatient and outpatient setting where the structure of the day may change depending on patient, family and staff needs. The Palliative Care social worker provides support around adjustment to illness, decision making, eliciting goals of care and family coping along the illness trajectory. In keeping with the NASW Standards for Social Work Practice in Palliative and End of Life Care (***************************** the social worker will play a multidimensional role as clinician, educator, advocate and team consultant. Job Responsibilities: 1. Performs comprehensive psychosocial assessments of hospitalized patients with serious illness. Assists the health care team to integrate psychosocial factors into treatment plans: Ability to analyze significant psychological, physical, emotional, social and economic issues of patients/families and to effectively communicate the relevance of such to other health care providers. Ability to identify, analyze and suggest interventions for difficult patient/family management situations. 2. Provides social work intervention including counseling and support to assist patients/families. Demonstrates ability to form empathic relationships with patients/ families who have significant emotional adjustment and coping issues. 3. Effectively addresses quality of life and end of life issues within a culturally competent framework to support the equitable delivery of excellence in health care to all patients and families. 4. Assess patient/ family's level of understanding, barriers to learning, and to provide age-specific and culturally competent education as needed. Able to effectively assess and discuss the psychosocial issues of palliative care to patients/families 5. Arranges, attends and actively participates in patient/family conferences, in person, as well as via available telehealth resources. 6. Demonstrates commitment to continuous improvement by seeking ways of improving care, effectiveness, and efficiency within the Palliative Care Service through active participation in H3W Huddles and meetings. 7. Maintains patient records and other documentation in a manner consistent with standards of clinical care, hospital, department, third party and regulating agencies' policies and procedures and standards of risk management. 8. Attends Palliative Care Team rounds and all required departmental meetings. 9. Demonstrates awareness of, and adherence to, Hartford Hospital policies and procedures. Maintains confidentiality and complies with professional ethics according to professional departmental and organizational standards and maintains current Connecticut license to practice social work. 10. Participates in continuing education opportunities to enhance and broaden skills relevant to job performance; maintains documentation of continuing education and in-service activities provided. 11. Participates in educational programs for Hartford Hospital staff as an expert on psychosocial issues in palliative care. 12. Works with members of the team to provide bereavement follow up for our palliative care patients. 13. Helps to ensure that we have appropriately documented Serious Illness Conversations, as well as appropriately scanned- in copies of other advance care planning documentation (such as completed health care representative forms, MOLST forms, Living Wills, Guardianship paperwork) into the Electronic medical record for all patients seen by our service for palliative care consultation. Qualifications · Hold a master's degree from a social work program accredited by the Council on Social Work Education. Completion of Post-MSW Fellowship or Certification Program in Palliative Care preferred. · Two years post MSW work experience in a healthcare setting. · Work experience in the field of palliative care, hospice care or relevant disease-specific specialty care. · Licensed Clinical Social Worker (LCSW) with valid licensure in the State of Connecticut. · Seek Advanced Palliative and Hospice Social Worker - Certified (APHSW-C) certification within 2 years. · Bilingual in Spanish and English preferred. Job Summary The Manager Quality Trauma Surgery Program provides leadership and operational support for the St. Vincent's Medical Center (SVMC) Trauma Program's (TP) development, implementation, and assessment to ensure an organized, effective, and efficient quality-focused, approach to providing high-quality trauma care. This position provides multiple services to include the coordination of TP initiatives between Trauma Program leaders, faculty, and HHC facilities, which includes clinical, educational, and research initiative collaboration and information sharing as relevant. This position's continuum of involvement spans departmental, inter-hospital, system, community, state, and national interventions. In addition, this role involves frequent interaction and collaboration with a variety of stakeholders across the HHC System and non-HHC facilities at a variety of professional and clinical levels with an emphasis on high-quality customer service. This position reports to Director of Clinical Operations, Inpatient Surgery with a substantial dyad of accountability to the Trauma Program Medical Director. Essential Functions/Responsibilities 1. Promotes and operationalizes the goals and objectives of the trauma program to achieve its strategic vision for an integrated system. Establishes collaborative partnerships with all relevant parties characterized by vision, leadership and communication. Oversee and manages Trauma Quality Performance Team consisting of a Performance Improvement Nurse, Trauma Registrars, and Injury Prevention Coordinator. Acts as a departmental representative/collaborator with all Injury Prevention Center initiatives. 2. Champions and leads quality performance improvement/evaluation and patient safety efforts that ensure compliance with hospital/department rules and regulations, policies and procedures. Collaborates with other Quality and Safety colleagues/programs to inform planning at the SVMC hospital and System level. Coordinates and manages standing TP meetings, M&Ms, and multi-disciplinary M&M and PIP meetings. 3. Provides leadership in the design and implementation of a systematic process for monitoring, evaluating and improving the quality and effectiveness of trauma patient care. Identifies quality of care issues and brings to closure to ensure quality patient care. Ensures process includes data abstraction and verification, query design, statistical analysis, trending and reporting, to develop data sets into useful information to develop strategies for achieving improved outcomes. 4. Collaborate with and/or supervises appropriate personnel to implement, maintain and administer quality trauma databases and files according to QA protocols, HIPPA standards and legal/risk management laws. Reviews and communicates data to appropriate staff members, regulatory agencies and the national trauma registry to ensure accurate risk-adjusted benchmarking. 5. Develops and implements outcome standards, credentialing standards, policies and procedures and clinical and system process pathways in collaboration with system participants. Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of Outcome and Assessment Information Set (OASIS) assessments by reviewing for regulatory and policy adherence. The individual in this role provides clinical staff with guidance on documentation, utilization, and patient care decisions to support optimal clinical and operational outcomes. They review visit utilization for care appropriateness and collaborate with clinicians on necessary modifications, reporting any potential over or underutilization to leadership. The role requires timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs. Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agency's compliance with data submission requirements for regulatory and payor standards. JOB RESPONSIBILITIES Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability. Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals. Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs. Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings. Qualifications Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Responsibilities: · Facilitates all aspects of set up and development including responsibility and functionality of all electrophysiology (EP) lab equipment and troubleshooting equipment technical problems. Evaluations include: Consideration of developmental, social and/or cultural, substance abuse, psychological, environmental, and medical issues Physical, social, emotional, and familial characteristics Use of evidence-based assessment tools to determine cognitive status and behavioral health needs including Dementia, Depression, Anxiety, etc. Home safety, family dynamics, caregiver strain, and other risk factors including suicide Patient/family understanding of their disease treatment options Strengths, available supports, and barriers to care Determine appropriate level of care including needs for long/short-term skilled-nursing facilities, assisted living facilities, adult day care, etc. Based on a comprehensive, culturally competent assessment collaborates with the patient and family to create a care plan with measureable goals. Implements short-term interventions to address identified barriers and promote health and well-being. Provides clinical treatment services in form of triage, crisis intervention, individual or family therapy, screening and planning Utilize Motivational Interviewing and Coaching skills to assist patients/families in identifying and addressing goals.
