Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.
Fabricate splints and instruct patients on using adaptive equipment to enhance function.
Provide training in the use of prosthetic and/or orthotic devices.
Maintain accurate clinical records and report any changes in the patient's condition to the physician.
Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
Work alongside physicians and discharge planners to provide health education to patients and families/caregivers.
Identify patient and family/caregiver needs for additional services and make appropriate referrals.
Confirm daily visit schedules to ensure seamless coordination with other staff.
Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.
Qualifications
Must have a current OT license (state-specific).
Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.
Registered with the National Registration Examination of the American Occupational Therapy Association.
Valid BLS certification.
Minimum two years of OT experience (home health experience preferred).
Excellent communication, organizational, and time management skills.
Must have a valid driver's license and access to a reliable and insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Confirm daily visit schedules to ensure seamless coordination with other staff.
