Key Responsibilities:
Assist in delivering client care services as directed by the Occupational Therapist (OT).
Support the OT in evaluating and developing the rehabilitative care plan and in periodic re-evaluations.
Help prepare clinical and progress notes, ensuring OT review and submission per company policies.
Collaborate with the OT to instruct clients and families on exercise programs and therapeutic activities.
Guide clients in self-care and creative activities aimed at improving their independence, physical, and mental functioning.
Report any adverse drug reactions, allergies, or medication concerns to the OT.
Record client activities and report findings to the OT and physician, noting any changes in the client's condition.

Support discharge activities and contribute to the Occupational Therapy Discharge Summary with the OT.
Confirm visit schedules with the Supervisor/Director of Clinical Services to ensure proper coordination.
Assist in timely reassessments and alert the office if assessments or recertifications are due.
Qualifications:
Graduation from an accredited Occupational Therapy Assistant program.
Current certification or license, as required by state law.
Current BLS certification.
Six months to one year of Home Health experience (preferred).
Strong verbal and written communication and organizational skills.
Valid driver's license with a reliable, insured automobile.
Strong problem-solving, critical thinking, and interpersonal communication skills.
