Hours may vary. POSITION SUMMARY The Case Manager is responsible for providing case management services to adults living in the behavioral health residential group homes. These services include resource and referral, life skills training, group counseling, mental health assessment, treatment planning and monitoring, advocacy and support services, case collaboration with other providers at Pioneer Center and other community organizations. As a part of the Residential department of the Behavioral Health pillar, the Case Manager will adhere to the Mission, Vision, and Values of the Pioneer Center and philosophy of the Behavioral Health pillar. This philosophy emphasizes Trauma-Informed Care, Evidence-Based Practices, Strength-Focused Treatment, and Client-Centered Services. To achieve these standards, the Case Manager and all members of the Behavioral Health pillar must attend ongoing internal and external trainings and contribute to a culture of collaborative learning. Consistently spends 60% of time providing direct service to clients (typically 24 of 40 hours per week) Complete all documentation accurately and concurrently, which is defined by within 24 hours of service delivery. Learns and implements Trauma-Informed Care and Evidence-Based Practices Facilitate groups for residential clients on topics such as community living, healthy relationships, assertive communication, emotional regulation, and independent living skills. Maintain a caseload and provide case management services in compliance with Medicaid Rule 132/140 Uses strength-focused treatment planning to collaboratively create goals and objectives with clients. Completes IM+CANS (Illinois Medicaid Comprehensive Assessment of Needs and Strengths) with all clients minimally every 180 days. Travels to clients' homes and other community locations to meet with clients. Participates in on-call shifts in rotation with other case managers, coordinators, and managers. Makes referrals and advocates for clients in need of assessments, treatment, appointments, and other services. Assists clients in accessing public benefits (TANF, SNAP, SSDI, etc.) Teaches and models healthy social skills and independent living skills. Develops linkages in the community by networking with other service providers. Facilitates or participates in case consultation or staffing's with other service providers. Provides service in a professional manner and communicates with staff, clients, families, and other agencies involved with the client. Completes required agency training. Provides service according to policy and procedure (HIPAA, Confidentiality, Ethics, etc.). Regular, reliable, predictable attendance. Performs all other duties as assigned by Supervisor. Chance for day shifts after 6 mos-1 year! What You'll Do Partner with patients and families in care decisions. Develop and document individualized care plans using evidence-based practices. Perform safe hand-offs using SBAR and bedside reporting. Communicate effectively with physicians and the multidisciplinary team. Stay engaged in unit huddles, process improvement projects, and continuing education. Maintain certifications and competencies to deliver the highest standard of care. Qualifications Education: Graduate of an accredited nursing program (ADN, Diploma, or BSN). Licensure: Current RN license in Missouri or compact license. Certifications: Required: BLS (American Heart Association) within 30 days of hire. Preferred: ACLS, NRP, C-EFM, and others as applicable. Experience: Previous RN experience required; Women's Health experience preferred but not mandatory.
