The Registered Nurse (RN) in the Emergency Room (ER) provides critical care to patients in urgent or emergent conditions. The RN in the ER works in a fast-paced, high-pressure environment, delivering immediate care to patients suffering from injuries, illnesses, or trauma. They assess, triage, and treat patients in collaboration with physicians, nurse practitioners, and other healthcare professionals, ensuring the best outcomes in a dynamic setting.
Key Responsibilities:
Triage and Patient Assessment:
Perform initial assessments of patients presenting in the ER, including triaging based on the severity of their condition.
Evaluate patient history, conduct physical assessments, and identify urgent health issues to prioritize care.
Collect patient vital signs (blood pressure, temperature, pulse, respiration) and other relevant data to determine immediate care needs.
Emergency Care and Treatment:
Administer medications, fluids, and treatments as prescribed, including intravenous (IV) fluids, antibiotics, pain management, and other emergency medications.
Perform wound care, splinting, suturing, and other interventions as needed for trauma or injury patients.
Provide emergency life-saving interventions, including CPR, advanced cardiac life support (ACLS), and trauma care.
Assist in resuscitation efforts, including managing ventilators, intubation, and defibrillation when necessary.
Collaboration with Medical Team:
Work closely with physicians, nurse practitioners, and other team members to develop and implement patient care plans.
Assist with diagnostic testing, such as lab work, imaging (X-rays, CT scans), and electrocardiograms (ECGs), based on patient needs.
Communicate clearly and efficiently with the healthcare team, updating them on changes in patient status and discussing care options.
Monitoring and Documentation:
Continuously monitor patients' conditions, noting changes in vital signs, mental status, and symptoms.
Document all patient assessments, treatments, and outcomes accurately in the electronic health record (EHR).

Patient and Family Communication:

The Clinical Nurse Specialist/Nurse Practitioner is a registered nurse, who, as a result of graduate education and in-depth clinical experience possesses the advanced knowledge and clinical skills necessary to provide expert nursing care. The major functions of the CNS/NP include education, consultation, clinical practice, research, and leadership. The primary responsibility is the direct application of clinical expertise to patients/families and patient care staff. The CNS/NP plans, implements, coordinates and evaluates nursing care and clinical programs for patients. In addition, the CNS/NP advances nursing practice and optimizes patient outcomes through education, leadership and participation in clinical research in collaboration with Service Line Leader and Physicians. The CNS/NP practices in accordance with the California Nurse Practice Act, ANA Code of Ethics, and the ANA Bill of Rights for Nursing as well as the Hospital/Department policies. The CNS/NP provides care in a collaborative manner with other health team members to ensure optimal, individualized patient care and outcomes.
Key Responsibilities:
Weekly patient rounding-coordination of care and communicating plan of care to the patient/family and multidisciplinary team.
Navigate the continuum of care for patients including the coordination of post-op/procedure visits, Acute Rehab, Home Care and/or Skilled Nursing Facility (SNF).
Optimize post hospital care coordination including Tele-monitoring and specialist referrals.
Collaborate with the Enterprise wide teams to improve patient satisfaction and patient care outcomes.
Liaison to Clinical Data Specialists of the applicable data registries.
Perform preliminary/concurrent case reviews to support cardiac quality assurance, quality improvement activities and outcomes management

Qualifications
Master's degree in Nursing.
Must have at least 24 months clinical nursing experience in area of specialty.
Demonstrated ability to produce results and coordinate projects
Experience of expertise in quality improvement and evidenced based practice.
Prior CNS, NP or nursing education experience preferred.

The Sepsis, Quality Outcomes Nurse will collaborate with the Quality Department, Sepsis Team, and multidisciplinary teams to:
Review patient charts and treatment plans in partnership with multidisciplinary teams to monitor patient outcomes.
Track and report the utilization of evidence-based clinical practices and identify opportunities to improve patient care.
Develop systems to identify specific inpatient populations according to evidence-based guidelines.
Identify areas for improvement based on evidence-based practices, regulatory standards, and accreditation requirements.
Analyze data to detect trends, gaps, and opportunities in high-risk, high-volume, or problem-prone processes.
Conduct chart abstraction and review for key regulatory reporting, including The Joint Commission, SEP-1 Core Measures, and other indicators.
Create tools to track and analyze trends in patient outcomes (e.g. mortality, readmission, SEP-1 compliance, etc.).
Design and implement new processes and clinical care initiatives to continually improve patient outcomes and achieve performance targets.
Communicate findings and provide feedback to physicians and nurses regarding compliance with evidence-based practices and outcome measures.
Conduct and report results of clinical outcomes and process monitoring; design and develop dashboards and innovative data visualizations to improve care.
Advise on the appropriate use of quality improvement tools and methodologies, such as PDCA, Lean, Six Sigma, and Statistical Process Control Analysis.
Apply performance improvement methodologies (e.g., Lean, Six Sigma, FMEA) to quality improvement projects.

Qualifications:
Registered Nurse (RN) with current state licensure.
Bachelor's degree in nursing (BSN) required; Master's degree preferred.

Existing knowledge of SEP-1 core measure is preferred.
