Primary Duties and Responsibilities:
Provides prompt, efficient and friendly customer service by exhibiting caring, concern and patience in all customer interactions and treating customers as the most important people in the store.
Smiles and greets customers in a friendly manner, whether the encounter takes place in the employee's designated department or elsewhere in the store.
Makes an effort to learn customers' names and to address them by name whenever possible.

Understands and follows local, state, and company policies on sales of tobacco, liquor, and lottery tickets.
Ensures compliance with local, state, and federal guidelines regarding WIC and food stamps.
Required to know value, look-up numbers, department numbers, and features of items for which money is received.
Stocks, faces, rotates merchandise, cleans shelves, understands shelf tags, knows location of merchandise in the store and also in the back room, and takes customers to an item when requested.
Perceives immediate surroundings for customer needs and problems for store safety and security.
Communicates clearly and distinctly on the intercom or telephone system.
Reviews weekly store ad to note price changes and sale items.
Keeps register area clean, ensures adequate supplies are ready and available, reports improper cash levels in cash drawer, and helps maintain the store appearance.
Compares totals on cash register with amount of currency in register to verify balances at the beginning and end of work shift, (where applicable).
May give cash refunds or issue credit memorandums to customers for returned merchandise or know where to direct the customer.
Understands and practices proper sanitation procedures and ensures the work area is always clean and neat.
Maintains strict adherence to department and company guidelines related to personal hygiene and dress.
Adheres to company policies and individual store guidelines.
Reports to work when scheduled and on time.

Knows code dates on perishable items and can explain to customers.

Role Description
The Material Handler accurately moves raw materials to and from the warehouse for use by the production team. This includes issuing and staging raw materials and packaging to a job before production occurs. The Material Handler will assist in the cycle count program.

Role Responsibilities
· Receive all inbound deliveries - Complete all documentation as required.
· Load/unload trucks.
· Prepare all outbound shipments for complete and timely delivery to customer.
· Schedule and follow up on all inbound deliveries and outbound shipments.
· Pick all materials from pick lists and deliver / transfer to production staging areas.
· Maintain perpetual inventory log and confirmations.
· Perform daily forklift checks and equipment within the S/R areas.

The Herdsperson assists other Herd Technicians in routine animal care and general farm tasks to ensure the welfare/well-being of the animals. The Clinical Assessment Manager - Care Delivery serves as a valuable resource to the Intake, Enrollment, and Care Delivery teams by documenting critical clinical enrollment, evaluation, and documentation of correct diagnosis, reassessment, and change in condition information in the electronic record system.
JOB RESPONSIBILITIES:
* Works closely with intake and enrollment and care delivery teams regarding in-home or virtual assessment appointments with prospective or enrolled participants (utilizing the Uniform Assessment System of New York, UAS-NY). During this visit, answers questions about the PACE program and emphasizes the unique benefits of the PACE program.
* Conducts in-home assessments for PACE participants utilizing the UAS-NY to determine initial or continued plan eligibility by the requirement need of Community Based Long Term Care (CBLTC) Services for greater than 120 days, and care needs.
* Evaluates and reviews previous assessments (of all types) to ensure documentation of any/all changes are accurately reflected in EMR. Must include narrative comments and supporting documentation and escalate issues of significant changes to Care Delivery to provide corrected resources.
* Evaluates and reviews previous primary diagnosis and medications at key intervals to ensure updated documentation is accurately reflected in EMR and match the diagnosis entered into UAS. Update and/or modify diagnosis codes and medication updates and ensure the accuracy of information. Must include narrative comments and supporting documentation and escalate issues of changes to ensure proper care and management of cases are followed.
* Promotes and educates potential referral sources about the PACE program as needed to encourage participation in the program through presentations, lectures, meetings, or other support.
* Educates potential/new PACE participants on plan benefits, services, and care delivery model.
* Escalates questions or concerns during the assessment to the quality review team to ensure appropriate completion of UAS-NY and handling of on-site issues that may require intervention.
* Completes and/or reassesses previous PCAT Tasking Tool to determine appropriate home assistance with ADLs/IADLs combined with attendance at the CenterLight PACE site. Evaluates and provides recommendations for modifications, corrections, and/or changes to previous evaluations, as well as escalates issues to the discipline manager.
* Develops the proposed service plan and obtain necessary documentation during home visits.
* Utilizes Lenavi analyzer during either the review process and/or assessment.
* Documents all interactions and outcomes in the designated clinical management/electronic record system.
* Completes all assessments, notes, service plans, and other documentation timely (within 48 hours).
* Works with the quality review team to ensure the accuracy and completeness of all necessary documentation in the Health Commerce System and the electronic record system.
* Maintains knowledge of all applicable CMS/PACE guidelines and relevant organizational and departmental policies and procedures, including but not limited to, the timely conduction, finalization, and documentation of the UAS-NY.
* Remains responsive and available by phone/email for contact by CenterLight staff.

* Provides significant clinical information to the Care Delivery team in advance of enrollment to ensure a smooth transition and appropriate opportunity for Care Delivery preparation of new participants with complex health conditions.

The Sanitation Supervisor is responsible for overseeing and coordinating all sanitation activities within the facility to ensure compliance with FDA regulations, cGMPs (Current Good Manufacturing Practices), SQF, HACCP, and company standards. This role ensures the plant is maintained in a clean, safe, and audit-ready state at all times, while supervising the sanitation team and partnering with Production, Quality, and EHS to achieve operational excellence.
Responsibilities
Sanitation Operations
• Supervise and direct daily sanitation activities across production areas, packaging, warehouses, and common spaces.
• Ensure cleaning and sanitizing of equipment, tools, and production areas are completed according to Master Sanitation Schedule (MSS) and Standard Sanitation Operating Procedures (SSOPs).
• Monitor and verify pre-operational inspections prior to start-up of production shifts.
• Maintain sanitation records and logs required for FDA, USDA, SQF, or customer audits.
Compliance & Food Safety
• Ensure all sanitation activities comply with FDA, OSHA, FSMA, and site food safety standards.
• Partner with Quality Assurance to implement and maintain HACCP and preventive controls related to sanitation.
• Support internal and external audits; take corrective action on findings related to sanitation.
• Enforce chemical handling, labeling, and storage procedures in compliance with OSHA and company policies.
• Assists with CIP/COP Validation and Verification activities.
• Assists with plant dedicated footwear and uniforms, ensuring employees are compliant with all internal GMP policies.
Leadership & Training
• Supervise sanitation crew, including scheduling, timekeeping, coaching, and performance management.
• Train employees on sanitation procedures, chemical safety, and PPE requirements.
• Drive a culture of accountability, safety, and continuous improvement within the sanitation team.
Continuous Improvement
• Recommend and implement improvements to sanitation methods, equipment, and efficiency.
• Collaborate with Maintenance, Production, and Quality to minimize downtime and improve sanitation outcomes.
• Participate in Lean/5S, Kaizen, or continuous improvement initiatives impacting sanitation and hygiene standards.
• Track and trend Sanitation KPI's, report data and information on a regular basis to the management team and hourly employees.
Preferred Education/Qualifications
• High school diploma or GED required.
• Associate or bachelor's degree in food science, Microbiology, Industrial Management, or related field preferred.
Work Experience
• 2-4 years of sanitation experience in a food, beverage, or FDA-regulated manufacturing environment.
• 1-3 years of supervisory experience, preferred.
• Strong knowledge of FDA, cGMPs, HACCP, SSOPs, chemical safety, and sanitation best practices.
Skills & Abilities
• Ability to lead and motivate non-exempt sanitation employees across multiple shifts.
• Knowledge of cleaning chemicals, sanitation equipment, and safe handling practices.
• Strong organizational skills with attention to detail and documentation accuracy.
• Ability to work flexible hours, including nights and weekends, as needed to support production schedules.
• Proficiency in MS Office and experience with ERP or sanitation management systems.
• Excellent communication and organizational skills.
• Ability to work in fast-paced environment.
